Validitas pemeriksaan mamografi dengan sistem BI-RADS TM untuk deteksi kanker payudara di RS DR Sardjito Yogyakarta

2008 | Tesis | S2 PPDS I-Ilmu BedahLatar Belakang Kanker payudara merupakan penyebab kematian nomor enam di Indonesia, sebagian besar akibat keterlambatan diagnosis dan pengobatan. Mamografi merupakan pemeriksaan yang lebih akurat dibandingkan pemeriksaan klinis untuk mendeteksi kanker payudara, dengan nilai positif (true positif) mencapai 90%, sehingga dapat menurunkan mortalitas akibat kanker payudara. Ketidakseragaman pembacaan mamogram dapat menimbulkan keraguan terhadap interpretasi hasil dan rekomendasi penatalaksanaan kanker payudara. The American College of Radiology Breast Imaging and Data System (BI-RADSâ„¢) membuat suatu metode standar untuk mengorganisir hasil mamografi, menggambarkan temuantemuan dan mengelompokkan kesimpulan interpretasi. Di rumah sakit Dr Sardjito Yogyakarta belum tersedia data validitas pemeriksaan mamografi menggunakan metode sistem BI-RADSâ„¢. Metode Penelitian Penelitian ini merupakan penelitian cross sectional study dengan rancang bangun uji diagnostik Tujuan Penelitian: Mengetahui validitas pemeriksaan mamografi menggunakan sistem BI-RADSâ„¢ dibandingkan pemeriksaan patologi anatomi dalam mendeteksi kanker payudara pada pasien dengan kecurigaan kanker payudara. Mengetahui nilai sensitivitas, spesifisitas, nilai ramal negatif, nilai ramal positif, rasio kecenderungan positif dan rasio kecenderungan negatif pemeriksaan mamografi dengan sistem BI-RADS dibandingkan pemeriksaan patologi anatomi. Hasil Validitas pemeriksaan mamografi dengan sistem BI-RADS dibandingkan histopatologi dalam mendeteksi kanker payudara dapat dihitung, dengan hasil sensitivitas 87,50%, spesifisitas 93,75%, nilai duga positif (PPV) 96,55%, nilai duga negatif (NPV) 78,95%, rasio kemungkinan positif (LR+) 14, rasio kemungkinan negatif (LR-) 0,1333, prevalensi 66,6%, , dan akurasi 89,58% Kesimpulan Pemeriksaan mamografi dengan sistem BI-RADS di RS Dr Sardjito Yogyakarta mempunyai validitas yang tinggi
Background: The breast cancer is the second most common cancer among women on the world and the sixth common cause of mortality in Indonesia. The diagnosis of breast cancer may upheld with physical and imaging examinations, but the definitive diagnosis should performed by histopathologic examination. Mammography is an easy, non invasive imaging method to detect breast cancer with good accuracy so it could decrease the mortality of breast cancer. The lack of uniformity in mammography repoting has led to confusion as to the interpretation and management recommendation. The American College of Radiology Breast Imaging Reporting and Data System (BI-RADS) provides a standardized reporting system for mammography. The validity of this system at Dr Sardjito Hospital Jogjakarta has not been evaluated. Objective : To defining the sensitivity, specificity, positive and negative predictive values, and accuracy of the BI-RADS system mammography to diagnose breast cancer. Methods: Analytic study was done with cross sectional approach using diagnostic test. All medical records of the patient with breast lump that performed mammography and histopatology examination at Dr Sarjito Hospital Jogjakarta between December 2003- March 2008, have been analized retrospectively. Results: BI-RADS system mammography at Dr Sardjito Hospital Jogjakarta have sensitivity 87,50%, specificity 93,75%, positive predictive value 96,55%, negative predictive value 78,95%, positive likelihood ratio 14, negative likelihood ratio 0,1333, prevalence 66,6%, and accuracy 89,58%. Conclusion: BI-RADS system mammography at Dr Sardjito Hospital Jogjakarta has high validity, so it can be used as supporting diagnostic test to detect breast cancer.
