PERBEDAAN EKSPRESI INTERLEUKIN -8 MUKOSA GASTER ATROPI DAN TANPA ATROPI PADA GASTRITIS KRONIK INFEKSI HELICOBACTER PYLORI

2016 | Tesis-Spesialis | SP Ilmu Penyakit DalamLatar belakang: Pada negara berkembang nfeksi Helicobacter pylori (HP)terjadi pada 80% populasi dan < 40% pada negara maju. Infeksi Helicobacter pylori menyebabkan inflamasi mukosa lambung dan epitel lambung serta melepaskan interleukin-beta, interleukin-6, interleukin-8, dan tumor necrosis factor alfa. Infeksi Helicobacter pylori berkaitan dengan resiko penyakit saluran cerna seperti gastritis kronis, ulkus duodenum, ulkus gaster, dan adenokarsinoma gaster dan merupakan penyebab utama infectionrelated cancer. Belum ada penelitian tentang perbedaan ekspresi interleukin-8 mukosa gaster atropi dan tanpa atropi pada gastritis kronik dengan infeksi Helicobacter pylori di RSUP Dr. Sardjito Tujuan Penelitian: Untuk membuktikan adanya perbedaan ekspresi interleukin-8 pada mukosa gaster atropi dan tanpa atropi pada gastritis kronik dengan infeksi Helicobacter pylori. Metode: Penelitian menggunakan desain potong lintang dengan pendekatan retrospektif sampel biopsi bulan Januari 2009-Desember 2014. Klasifikasi mukosa atropi dan tanpa atropi diperoleh melalui laporan histopatologi. Data numerik-kategorik 2 kelompok, tidak berpasangan. Uji statistik untuk distribusi tidak normal menggunakan uji Mann-Whitney. Perbedaan dianggap bermakna jika didapatkan p< 0,05 dengan interval kepercayaan 95% Hasil: Didapatkan 40 sampel preparat biopsi gaster dengan infeksi H.pylori, terdiri dari 12 sampel mukosa atropi dan 28 sampel mukosa tanpa atropi. Nilai median kadar ekspresi IL-8 pada semua sampel adalah 82,50(19-96), nilai median ekspresi IL-8 pada mukosa atropi sebesar 80(20-92) dan non atropi sebesar 83(19-96). Dilakukan uji komparasi dengan uji Mann Whiytney. Dari perhitungan statistik tidak terdapat perbedaan bermakna ekspresi IL-8 mukosa atropi dan non atropi dengan nilai p=0,516. Kesimpulan: Tidak terdapat perbedaan ekspresi interleukin-8 pada mukosa gaster atropi dan tanpa atropi pada gastritis kronik dengan infeksi Helicobacter pylori.
Background: The prevalence of Helicobacter pylori infection is 80% in developing countries while in developed countries the prevalence is < 40%. Helicobacter pylori infection is causing gastric mucosal inflammation, gastric epithelium and releasing interleukin beta, interleukin-6, interleukin-8, and tumor necrosis factor alfa. Helicobacter pylori infection is associated with the risk of gastrointestinal disease such as chronic gastritis, duodenal ulcer, gastric ulcer and gastric adenocarcinoma and a major cause infection-related cancer. There has been no research about differences in interleukin-8 expression of gastric mucosal atrophy and non-atrophy in chronic gastritis with helicobacter pylori infection in the Hospital Dr. Sardjito Research Purposes: Knowing the differences expression of IL-8 mucosal atrophy and non-atrophy in chronic gastritis with helicobacter pylori infection Methods: This study used a cross-sectional design. This study examined the expression of interleukin-8 of the biopsy sample HP (+) from January 2009 to December 2014. The classification of mucosal atrophy and non-atrophy obtained through endoscopy report. The analysis of categorical and numerical data of more than 2 groups, unpaired, abnormal distribution using Mann-Whitney test. The differences were considered as significant if the obtained p <0.05 with 95% of confidence intervals. Results: There were 40 samples of gastric mucosal biopsy preparations, comprising 12 samples of gastic mucosal atrophy and 28 samples of gastric mucosal non atrophy. The median value of the expression of IL-8 in all samples is 82,50 (min 19-mak 96), while median value of IL-8 expression in mucosal atrophy is 80 (min 20-mak 92) and median value expression IL-8 in mucosal non atrophy is 83 (min 19-mak 96). On a comparative test using Mann Whitney U test, the result shows that statistically there is no significant differences in the expression of IL-8 mucosal atrophy and non-atrophy with p value = 0.516. Conclusion: There is no differences expression interleukin-8 gastric mucosal atrophy and non-atrophy in chronic gastritis with Helicobacter Pylori infection.
