PERBANDINGAN RESPON TERAPI PASIEN KANKER SERVIKS UTERI JENIS KARSINOMA SEL SKUAMOSA DAN ADENOKARSINOMA PADA STADIUM LANJUT (IIB-IVB) DI RSUP DR. SARDJITO

Latar belakang : Prognosis dari kanker serviks ditentukan oleh beberapa faktor, salah satunya jenis histopatologi. Secara histopatologi, kanker serviks terdiri dari beberapa jenis tergantung dari sel yang mengalami abnormalitas. Jenis yang paling banyak ditemukan adalah karsinoma sel skuamosa dan adenokarsinoma. Dari beberapa penelitian, perbedaanya masih tidak konsisten. Oleh karena itu perlu diteliti lebih lanjut adakah perbedaan respon terapi antara kedua jenis sel kanker tersebut. Tujuan : Mengetahui perbedaan respon terapi pada pasien kanker serviks stadium lanjut (IIB-IVB) jenis karsinoma sel skuamosa dan adenokarsinoma. Metode : Penelitian memiliki desain cohort retrospektif pada subjek pasien kanker serviks rawat inap dan rawat jalan RSUP Dr. Sardjito. Data diperoleh dari Instalasi Catatan Medis RSUP Dr. Sardjito Yogyakarta. Data yang dicatat dari periode tahun 2011 - 2013 meliputi umur, tinggi badan, berat badan, jenis histopatologi, stadium kanker serviks, terapi yang diberikan dan hasil pap smear evaluasi terapi. Data dianalisis dengan komputer menggunakan uji hipotesis chisquare. Hasil : Penelitian tidak menunjukkan adanya perbedaan bermakna secara statistik antara respon terapi dengan jenis histopatologi, usia, stadium, kadar hemoglobin, dan indeks massa tubuh (p > 0,05). Namun didapatkan perbedaan secara klinis. Kesimpulan : didapatkan perbedaan respon terapi secara klinis pada penderita kanker serviks berdasarkan usia, stadium, kadar hemoglobin, indeks massa tubuh dan tipe histologi sel kanker, namun tidak didapatkan perbedaan bermakna secara statistik.

Background : The prognosis of cervical cancer is determined by several factors, one of which type of histopathology. By histopathology, cervical cancer consists of several types based on the cell abnormalities. The most common type is squamous cell carcinoma and adenocarcinoma. From several studies, the difference is still inconsistent. Therefore, it needs further investigation whether there any difference in treatment response between the two types of cancer cells. Objective : To know the therapy response differences in patients with advanced cervical cancer (IIB-IVB) type squamous cell carcinoma and adenocarcinoma. Methods : A retrospective cohort study design on the subject of cervical cancer inpatient and outpatient hospital Dr. Sardjito. Data were obtained from the Medical Records Installation hospital of Dr. Sardjito. Recorded data were obtained from the period 2011 - 2013 include age, height, weight, type of histopathology, stage of cervical cancer, treatment given and the results of pap smear evaluation of therapy. Data were analyzed by computer using chi-square test. Results : The study showed no statistically significant difference between the response to therapy with histopathologic type, age, stage of cervical cancer, hemoglobin levels, and body mass index (p > 0,05). But the difference is found clinically. Conclusions : Clinically, there are differences found in therapy response in patients with advanced cervical cancer based on age, stage, hemoglobin level, body mass index and histological types of cancer cells, but no statistically significant difference found (p > 0,05).
