LIFE EXPERIENCE IBU HAMIL DENGAN DIABETES MELLITUS GESTATIONAL DALAM PENCAPAIAN PERAN IBU DI YOGYAKARTA

Latar belakang: Diabetes Mellitus adalah kelainan kadar glukosa dalam darah atau hiperglikemia. Pada wanita hamil terjadi perubahan fisiologis yang berpengaruh terhadap metabolisme karbohidrat karena hormon plasenta bersifat resistensi terhadap insulin. Dari hasil studi pendahuluan pada ibu dengan DMG terdapat pernyataan ibu yang takut dengan perannya sebagai seorang ibu karena menderita DM selama hamil. Hal ini menunjukkan adanya pengaruh pengalaman ibu ketika menderita DMG terhadap pencapaian perannya sebagai seorang ibu. Di Indonesia, prevalensi GDM sekitar 1,9-3,6%. Data dari RSUP Dr. Sardjito Yogyakarta, Sub Bagian Rekam Medik pada indeks rawat inap penyakit DMG pada 10 tahun terakhir dari tahun 2003-2012 ada 94 pasien. Tujuan: mengeksplore life experience ibu DMG dalam pencapaian peran ibu di Yogyakarta. Metode: Penelitian ini merupakan penelitian kualitatif dengan pendekatan fenomenologi. Partisipan yang digunakan dalam penelitian ini adalah ibu yang sudah melahirkan maksimal 1 tahun dengan riwayat DMG. Jumlah partisipan dalam penelitian ini 5 orang. Teknik pengambilan sampel dengan purposive sampling yang bersifat criterion sampling. Analisis data menggunakan metode Colaizzi (2011). Pengambilan data di lakukan pada bulan Desember 2013-Februari 2014. Etika penelitian meliputi informed consent dan perijinan dari komite etik FK UGM. Teori keperawatan Maternal Role Attainment digunakan sebagai acuan untuk menganalisis terhadap life experience ibu dengan DMG. Hasil: Pada penelitian ini, didapatkan sebanyak 8 tema dari sub kategori-sub kategori yang telah tersusun, yaitu pengalaman ibu saat didiagnosis DMG; pengaruh DMG terhadap kehamilan; pengalaman ibu dalam berusaha secara ekstra untuk menjaga kehamilannya; variasi life experience ibu selama hamil, melahirkan, dan pasca melahirkan; persepsi ibu terhadap perannya selama menjalani perannya; faktor-faktor yang mendukung pengembangan dan pencapaian peran ibu; pengalaman ibu dalam mengintegrasikan identitas perannya sebagai ibu; kesenjangan dan keinginan ibu penderita DM dalam menerima pelayanan kesehatan. Life experience ibu DMG dalam mencapai perannya sebagai ibu terdapat keberagaman. Kesimpulan: Life experience ibu hamil dengan DMG menunjukkan keberagaman dan life experience ibu dalam pencapaian perannya sebagai ibu dengan DMG kurang optimal. Kategori-kategori yang didapatkan mendukung tahapan Maternal Role Attainment.

Introduction: Diabetes Mellitus is a disorder of the blood glucose levels or hyperglycemia. On physiological changes of pregnant women who have an effect on carbohydrate metabolism of hormone the placenta is owing to resistance to insulin. A study on the prelude with DMG there were mother statements who fear the maternal role attainment because suffering DM during pregnant. It shown any influences of mother experience when suffer DMG towards maternal role attainment. According to Medical Record Department, Sardjito hospital Yogyakarta, In Indonesia, the prevalence of DMG is approximately 1,9-3,6 on Inpatient disease index of DMG in the last 10 years from 2012-2013. This hospital treat 94 patients of hyperglycemia0. Objective: To explore Yogyakartanese Moms’ experiences coping DMG. Methods: This research was a phenomenological approach qualitative research. Participants in this research was the mother who gave birth to a maximum of 1 year with a history of DMG. The number of participants in this study was 12 peoples. Purposive sampling technique with sampling were the sampling criterion. Data analysis using the method of Colaizzi (2011). Data was collected between December 2013 and February 2014. Research ethics included an informed concent and ethics committe’s approval. Nursing theory of maternal role attainment (Mercer, 2011) used as a reference to analyze life experience of mother with DMG. Results: There were 8 categories i.e the experience of the mother while in diabetes mellitus gestational diagnose; the influence DMG of the pregnancy; mother's experience in trying to be extra to keep her pregnancy; a variation of life experience of mother during pregnancy, childbirth, and post delivery; the perception of the mother of his role being a mother, the factors that support the development and achievement of the maternal role attainmnet; the mother's experience in integrating the identity of her role as mother; gap and the desire of the mother with DMG in a receiving health services. Life experience mother DMG in achieving its maternal role attainment there is diversity. Discussion: Life experience pregnant mothers with DMG show various diversity and mother life experiences in the role attainment with DMG less than optimal, and those categories acquired is supportive toward stage of maternal role attainment
