PENGARUH APLIKASI TOPIKAL CASEIN PHOSPHOPEPTIDE AMORPHOUS CALCIUM PHOSPHATE (CPP-ACP) TERHADAP KADAR KALSIUM, FOSFAT DAN PH SALIVA (Kajian pada white spot)

White spot merupakan lesi karies dini email, sering dijumpai pada anak. Salah satu perawatan white spot adalah aplikasi topikal yang mengandung kalsium dan fosfat. Casein Phospopeptide-Amorphous Calcium Phosphate (CPP-ACP) merupakan bahan yang mengandung kalsium dan fosfat tinggi. Kalsium dan fosfat dilepaskan CPP-ACP yang berdifusi dalam saliva diharapkan dapat meningkatkan remineralisasi, menghambat demineralisasi. Fosfat juga diharapkan mampu berperan sebagai penyangga saliva. Tujuan penelitian ini untuk mengetahui pengaruh aplikasi topikal CPP-ACP terhadap kadar kalsium, fosfat dan pH saliva. Subyek penelitian terdiri 22 anak, dengan white spot gigi desidui rahang atas. Aplikasi topikal CPP-ACP dilakukan sekali sehari, sebelum tidur malam. Pengukuran kadar kalsium, fosfat dan pH saliva dilakukan sebelum aplikasi dan setelah 7, 14, 28 kali aplikasi. Kadar kalsium saliva diukur dengan Induktively Cauple Plasma, kadar fosfat saliva dengan Spektrofotometri DR 2000, pH saliva dengan Dental saliva pH indicator. Analisa data dengan Friedman dilanjutkan Wilcoxon. Hasil penelitian menunjukkan kadar kalsium, fosfat dan nilai pH saliva sebelum aplikasi topikal CPP-ACP terendah (43.7891 ±11.16495 ppm untuk kalsium; 28.3445±6.38469 ppm untuk fosfat; 6.1091±.10193 untuk pH). Kadar kalsium, fosfat dan pH saliva tertinggi setelah 28 kali aplikasi (113.3832 ±1.80684 ppm untuk kalsium; 51.5959±4.21489 ppm untuk fosfat; 7.7273±.14535 untuk pH). Uji Friedman dilanjutkan Wilcoxon menunjukkan perbedaan bermakna antar aplikasi (p<0.05). Kesimpulannya semakin sering aplikasi topikal CPP-ACP, semakin tinggi kadar kalsium, fosfat dan pH saliva. Kadar kalsium, fosfat dan pH saliva tertinggi setelah 28 kali aplikasi.

White spot is an early email carious lesions, often found in children. One of white spot treatment is a topical application that contains calcium and phosphate. Casein- Amorphous Calcium Phosphate Phospopeptide (CPP-ACP) is a substance contains high calcium and phosphate. We expect that it will release of calcium and phosphate into saliva, might improve remineralization and inhibiting demineralization. Phosphate released from CPP-ACP, might supports salivary buffer capacity. The aim of this study is to investigate the influence of topical application of CPP-ACP to calcium, phosphate level and pH value in saliva. The study subjects consisted of 22 children with white spot on maxillary deciduous teeth. Topical application of CPP-ACP was once a day, before bedtime. Measuring the salivary levels of calcium, phosphate and pH taken before, after 7, 14 and 28 times the application. The salivary calcium levels were measured by Inductively Couple Plasma, phosphate levels by Spectrophotometry DR 2000, pH values by Dental Saliva pH Indicator. Data were analyzed by Friedman resumed Wilcoxon. The result of the study showed that calcium and phosphate level, pH value in saliva before topical application of CPP-ACP was lowest (43.7891 ±11.16495 ppm for calcium; 28.3445±6.38469 ppm for phosphate; 6.1091±.10193 for pH) and after 28 times application was highest (113.3832 ±1.80684 ppm for calcium; 51.5959±4.21489 ppm for phosphate; 7.7273±.14535 for pH). The result of Friedman resumed Wilcoxon showed that those were significant differences (p<0.05). In conclusion, the more frequent topical application of CPP-ACP, the higher salivary calcium, phosphate levels, pH values. Salivary calcium and phosphate levels, pH values were highest after 28 times application.
