Pengaruh plat palatal lepasan dengan button-like stimulator terhadap kelemahan otot-otot bibir dan lidah pada anak cerebral palsy tipe spastik

Cerebral palsy merupakan kelainan sistem saraf sentral yang manifestasinya disfungsi neuromuskular. Kelainan tersebut menyebabkan perkembangan otak terhambat sehingga kemampuan dasar seperti berdiri, berjalan dan berbicara tidak berkembang sepenuhnya. Demikian juga dengan koordinasi abnormal lidah (tongue thrust), bibir, dan pipi sehingga mengganggu penelanan dan pengunyahan, air liur yang terlalu banyak (drooling), tongue thrust yang persisten, asimetri wajah dan gangguan artikulasi. Tujuan penatalaksanaan kasus ini adalah untuk mengetahui pengaruh penggunaan plat palatal lepasan dengan button like stimulator terhadap kelemahanan otot-otot bibir dan lidah pada anak cerebral palsy tipe spastik. Pasien perempuan usia 15 tahun menderita cerebral palsy tipe spastik, mampu berkomunikasi, dapat berjalan sendiri dengan alat bantu. Dari hasil pengamatan, kekurangan pada kemampuan oralnya adalah lidah tidak bisa menyentuh langit-langit, drolling, tidak mampu berkumur-kumur, posisi istirahat mulut dalam keadaan terbuka dengan bibir bawah miring ke kanan. Pasien ini di terapi dengan menggunakan plat palatal lepasan dengan button like stimulator yang kemudian dievaluasi lagi pada bulan ke-3. Pengukuran kelemahan otot-otot bibir dengan menggunakan ganiometri dan kelemahan otot-otot lidah dengan menganalisa perubahan pengucapan huruf L, R, K, G dengan MATLAB 7.0.1 . Penatalaksanaan kasus ini menghasilkan pengurangan sudut 0-sto-chA, 0-sto-chB dan atau selisihnya pada ganiometri. Pada pemeriksaan pengucapan terjadi peningkatan amplitudo saat pengucapan huruf L, R, K, G. Dari hasil tersebut di atas dapat disimpulkan bahwa plat palatal lepasan dengan button like stimulator mampu meningkatkan kekuatan otot-otot bibir dan lidah.

Cerebral palsy is a central nerves system disorder, one of many its manifestation is neuromuscular dysfunction. This disorder affecting the immature brain at a development stage, so the basic abilities like standing, walking, and talking are not fully developed. Thus, the abnormal coordination of tongue, lips and cheeks that makes swallowing an chewing disturb, drooling, persistent tongue thrusting, asymmetry of the face and articulation disorder. The aim of this case management is to present the effect of removable palatal plate with button like stimulator to mouth and tongue muscle flaccidity of child with spastic type cerebral palsy. This case is presented of a 15-years-old-girl with spastic type cerebral palsy, she is able to communicate, able to walk by herself with helping tool. From the observation, her oral skill inadequacy are: the tongue cannot reach the palate; drolling; not able to gargle; when at rest, the mouth is open with the lower lip at an angle to the right side. This patient is then given therapy by using removable palatal plate with button-like stimulator, evaluated in the third month. The labial muscle flaccidity is measured by using ganiometry, while the tongue muscle flaccidity is measured by analyzing change on pronounciation of letters: L, R, K, G with MATLAB 7.0.1. This case management delivers the reduction of the angle 0-sto-chA, 0- sto-chB or difference in ganiometry. In the examination of the pronounciation, there is rise of amplitude when pronouncing letters: L, R, K, G. From the above results, it can be concluded that the removable palatal plate with button-like stimulator is able to increase the labial and tongue muscles strength.
