Uji diagnostik mammografi, ultrasonografi payudara dan kombinasi mammografi ultrasonografi payudara pada massa payudara palpabel

Penelitian ini bertujuan untuk mengetahui validitas hasil pemeriksaan mammografi, USG payudara dan kombinasi mammografi dengan USG payudara pada massa payudara palpabel. Hipotesis yang diajukan adalah pemeriksaan mammografi dan USG payudara memiliki sensitivitas 80% dan spesifisitas 85% dalam mendiagnosis massa payudara palpabel. Kombinasi USG payudara dengan mammografi diharapkan akan meningkatkan sensitivitas dan spesifisitas. Desain penelitian ini adalah uji diagnosis dengan membandingkan hasil pemeriksaan mammografi dan USG payudara dengan pemeriksaan histopatologi sebagai baku emas. Pengumpulan data dilakukan di RSUP Dr. Sardjito antara Maret 2005 sampai September 2007. Subyek yang memenuhi kriteria inklusi dan eksklusi dilakukan pemeriksaan mammografi, USG payudara dan histopatologi. Subyek penelitian ini berjumlah 44 subyek yang memiliki hasil mammografi dan USG payudara. Dilakukan analisis secara deskriptif dan analisis statistik uji nonparametrik McNemar Test dengan tabel 2x2, mempergunakan perangkat lunak komputer serta penghitungan nilai sensitivitas, spesifisitas, nilai duga positif (NDP), nilai duga negatif (NDN), rasio kecenderungan positif (RK positif) dan rasio kecenderungan negatif (RK negatif). Hasil penelitian ini menunjukkan bahwa mammografi untuk mendiagnosis massa payudara palpabel memiliki sensitivitas 90,47%, spesifisitas 91,30%, NDP 90,47%, NDN 91,30%, RK positif 10,4 dan RK negatif 0,104 . USG payudara untuk mendiagnosis massa payudara palpabel memiliki sensitivitas 85,71%, spesifisitas 91,30%, NDP 90%, NDN 87,50%, LR positif 9,9 dan LR negatif 0,16. Kombinasi mammografi dan USG memiliki sensitivitas 95,23%, spesifisitas 91,30%, NDP 90,90%, NDN 95,45%, LR positif 10,94 dan LR negatif 0,05. Mammografi dan USG payudara memiliki validitas yang tinggi untuk mendiagnosis massa payudara palpabel. Kombinasi mammografi dengan USG payudara meningkatkan sensitivitas.

The aim of this study was to measure validity of mammography, breast sonography and combination of mammography and breast sonography in diagnosing palpable mass of the breast. Hypothesis of this study was sensitivity and specificity of mammography and breast sonography in diagnosing palpable mass of the breast are 80% and 85%. Combination of mammography and breast sonography will increase the sensitivity and the specificity. Study design for this study was a diagnostic test study that compared mammography and breast sonography with histopatologic result as a gold standard. Study was performed between March 2006 and September 2007 in Dr. Sardjito Central General Hospital, Yogyakarta. There were 44 subjects had both mammograph and breast sonograph. Descriptive analysis with McNemar non parametric test performed by a computer software program to calculated sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (LR positive) and negative likelihood ratio (LR negative) of mammography and breast sonography in diagnosing palpable mass of the breast. Sensitivity, specificity, PPV, NPV, LR positive and LR negative of mammography in diagnosing palpable mass of the breast were 90,47%; 91,30%; 90,47%; 91,30%; 10,4 and 0,104 consecutively. Sensitivity, specificity, PPV, NPV, LR positive and LR negative of breast sonography in diagnosing palpable mass of the breast were 85,71%; 91,30%; 90%; 87,50%; 9,9 and 0,16 consecutively. Combination of mammography and breast sonography had sensitivity 95,23%, specificity 91,30%, PPV 90,90%, NPV 95,45%, LR positive 10,94 and LR negative 0,05. Mammography and breast sonography have high validity in diagnosing breast palpable mass. Combination of mammography and breast sonography increase the sensitivity.
