Evaluasi soket mata pada pemakaian protesa mata Non Fabricated pasien pasca bedah eviscerasi karena trauma kecelakaan :: Laporan kasus

Trauma kecelakaan pada mata dapat mengakibatkan kerusakan bola mata, saraf mata dan rongga mata, oleh karena itu diperlukan tindakan pembedahan untuk mengambil bola mata dan jaringan sekitarnya. Untuk mengembalikan efek psikologis dan esthetik dapat dibuat protesa mata. Laporan kasus ini bertujuan untuk mengevaluasi perubahan soket mata pada pemakaian protesa mata non fabricated pasien pasca bedah eviscerasi. Laporan kasus dilakukan pada seorang laki-laki, umur 19 tahun pasca bedah eviscerasi karena kecelakaan. Pemeriksaan wajah menunjukkan muka asimetris. Pada mata sebelah kanan tampak bekas luka operasi, tidak ada perdarahan, luka sudah kering, tidak terasa sakit. Perawatan dilakukan dengan pembuatan protesa mata non fabricated dengan tahap-tahap: pencetakan mata dengan sendok cetak mata perorangan dan pengisian hasil cetakan terdiri dari dua bagian, yang pertama diisi dengan gips keras sampai bagian terlebar dari cetakan dasar soket dan dibuat tiga retensi sebagai kunci, kedua sampai menutupi seluruh hasil cetakan. Pembuatan model malam sklera, mencoba pola malam sclera dan packing model malam sklera. Deflaskiing & polishing untuk membuat sklera akrilik, mencoba sklera akrilik dan penentuan lokasi/diameter iris, melukis iris & pupil, penyelesaian prothesa mata, packing sklera & iris, deflasking dan polishing untuk membuat prothesa mata serta insersi prothesa mata. Kontrol prothesa mata 1hari, 7 hari, 1 bulan, 2 bulan, 3 bulan. Prognosa baik, karena pasien kooperatif dan motivasi besar untuk dibuatkan protesa mata non fabricated. Evaluasi perubahan soket mata setelah 1 hari, 7 hari, 1 bulan, 2 bulan. 3 bulan menunjukkan tidak ada keluhan rasa sakit, tidak ada peradangan, tidak adanya kotoran dan air mata be rkurang jumlah dan frekuensinya. Kesimpulan yaitu perubahan soket mata pada pemakaian prothesa mata non fabricated pasien pasca bedah eviscerasi dapat diterima dengan baik.

Trauma from accident to the eyes may cause the damaged of eye balls, eyes’s nerves and cavum orbita, that is why we need surgery procedure to take the eye balls and serounding tissues. To get back the psychological effect and esthethics, we can make the eye prothesis. This report is aimed to evaluate the changing of eye socket of non fabricated prothesis on patien post evisceration surgery. This report was based on the case of a man (19 years old) who had undergon eviscerasi post surgery due to accident. Facial examination showed asymmetry. There was wound post surgery on the right eye, no bleeding, dry wound, no pain. There ware some steps of making eye prothesis: taking impression eye by individual eye tray, the impression filled in two parts first with stone gips up to the widest part of the socket base and make three retentions as key, the second part was filled the whole impression. Making wax model of sclera; try in the acrylic sclera and judgment of location / diameter of iris, drawing iris and pupil , finishing eye prothesis, packing sclera and iris, deflasking and polishing to produce eye prothesis , insertion eye prothesis. Controlling the prothesis, 1 day, 7 days, 1 month, 2 mounths, 3 mounths. The prognosis was good because the patient was cooperative and wanted to have non fabricated eye prosthesis. The evaluation of eye socket changes after 1 day, 7 days, 1 mounth, 2 mounths, 3 mounths showed no pain, no inflammation, no debris and decreasing of quantity and frequency of tears. The conclusion of this evaluation is that the eye socket in the use of eye prothesis non fabricated for patient with post evisceration surgery is acceptable.
