Proses penyusunan formularium di RSUD Arga Makmur Bengkulu Utara

Latar Belakang : Rumah Sakit Umum Daerah Arga Makmur berdiri tanggal 12 Juli 1978, rumah sakit kelas C, memiliki 70 buah tempat tidur, dalam memberikan pelayanan kesehatan dilengkapi dengan Instalasi Farmasi. Selama tahun 2003 sampai Juni 2004, obat yang ada berjumlah 178 item, resep banyak tak terlayani atau lebih dari 36 % resep keluar, hal ini terjadi oleh karena kurangnya obat di IFRS selama 6 bulan terahir.Sebagian jenis obat tak digunakan, penulisan resep tidak sesuai dengan standar terapi lebih 2 %. PFT telah dibentuk tahun 2002, akan tetapi formularium belum terbentuk, pahal penyusunan formularium adalah tugas dari PFT. Formularium dapat membantu dokter dalam merubah pola peresepan dan standar terapi yang sudah ada. Penelitian ini bertujuan untuk mengetahui tersusun dan terbentuk serta diketahuinya formularium di RSUD Arga Makmur Metode Penelitian : kualitatif dengan action research, melalui intervensi edukasi agar formularium segera terwujud. Intervensi edukasi persuasif kepada Direktur, agar menginstruksikan secara tertulis kepada PFT untuk menindak lanjuti SK. PFT tahun 2002 yang lalu, untuk menyusun formularium. Sedangkan intervensi edukasi kepada anggota SMF, melalui DKT dengan memberi masukan untuk menyamakan persepsi tentang arti dan manfaat formularium. Selanjutnya membuat kesepakatan untuk menggunakan formularium yang ada. Intervensi edukasi dan upaya regulasi kepada PFT, agar menyusun formularium yang mengacu SK. Dirjen Pelayanan Medik tahun 1998. Hasil penelitian: Dengan melalui intervensi edukasi dan upaya regulasi terhadap PFT, maka formularium RSUD Arga Makmur tersusun, serta disahkan Direktur rumah sakit tanggal 12 Agustus 2004. Formularium tersebut berisi 273 item obat, obat generik 203 item (74,4 %), lebih banyak dibanding dengan daftar obat yang ada, obat dengan nama dagang 70 item (25,6 %). Formularium sudah dilengkapi dengan kelas terapi, dosis, indikasi, kontra indikasi dan kolom keterangan serta protocol khusus. Kesimpulan : Setelah dilakukan intervensi edukasi dan upaya regulasi kepada PFT, maka PFT baru dapat menyusun formularium rumah sakit yang disahkan tanggal 12 Agustus tahun 2004

Background: Arga Makmur district hospital that was established on July 12, 1978 was c-class hospital that has 70 beds. In providing health service, the hospital was completed by pharmacy installation. During 2003 to June 2004 it was found that the existed drugs were 178 items, more than 36 % prescription were not served or it was taken outside because the hospital pharmacy installation lacked of drugs in the last six years. Some kind of drugs were not used, 2% of prescription written was not adjust to therapy standard. Therapy and pharmacy committee was established on 2002 but the formulary was not formed yet, whereas it was the duty of them. Formulary can help doctor in changing the existing therapy standard and prescription manner. The study was aimed to find out composition, formulation, and knowledge of formulary in Arga Makmur district hospital. Method: The method of the study was qualitative with action research with education intervention to form formulary. Education intervention was persuasive to director to instruction written to the head of therapy and pharmacy committee in conducting therapy and pharmacy committee decree on 2002 to arranged formulary, while education intervention was conducted by focus group discussion to the member of functional medical staff in socializing the same perception toward the meaning and the use of formulary. It was continued by agreement to use existed formulary. Education intervention and regulation was obtained to the head of therapy and pharmacy committee to arrange formulary that referred to decree director general of medical service in 1998. Result: By education intervention toward therapy and pharmacy committee, the Arga Makmur district hospital formulary was arranged and legally validated by director on August 12, 2004. That formulary contains of 273 items of drugs, 203 of them were generic drugs (74.4%) and others were patent drugs (25.6%). The formulary was completed by therapy, doses, indication, contra indication, information column, and protocol. Conclusion: After obtaining education intervention by persuasive, and regulation to therapy and pharmacy committee, they arranged hospital formulary and it was validated on August 12, 2004.
