Uji diagnostik skor rinitis alergi

Beberapa alat skrining telah dikembangkan untuk mempermudah mengetahui prevalensi rinitis alergi. Alat skrining yang dikembangkan berdasar kondisi di Indonesia belum ada. Skor rinitis alergi disusun untuk penilaian secara kuantitatif dengan menggunakan cut-off point, sehingga interpretasi hasil lebih mudah. Tujuan penelitian: Menetapkan nilai cut-off skor rinitis alergi dan mengetahui nilai diagnostik skor rinitis alergi. Manfaat penelitian: Sebagai alat skrining yang sederhana dan mudah pemakaiannya. Disain penelitian: uji diagnostik. Tempat: Bagian rawat jalan Penyakit Telinga Hidung dan Tenggorok RS Dr. Sardjito Yogyakarta. Bahan dan cara: Jumlah sampel dalam penelitian ini adalah 78 orang. Baku emas yang dipakai adalah skin prick test terhadap alergen hirup. Dilakukan uji signifikansi hubungan 12 variabel dengan rinitis alergi menggunakan uji X2. Variabel yang mempunyai hubungan bermakna dipakai untuk menyusun skor rinitis alergi. Setiap variabel yang dijumpai pada subyek diberi nilai 1 sedangkan bila tidak dijumpai diberi nilai 0, kemudian dihitung nilai total. Cut-off point skor rinitis alergi ditentukan dengan kurva Receiver Operator Characteristic (ROC). Nilai diagnostik dihitung berdasar cut-off point yang terpilih. Analisa statistik : Uji X2 dan kurva Receiver Operator Characteristic (ROC). Hasil: sembilan variabel (pilek encer, bersin, hidung tersumbat, debu rumah, bulu binatang, riwayat atopi keluarga, konka nasales hipertrofi, konka nasales livide, discharge serous) terpakai untuk skor rinitis alergi dengan cut-off point ³ 6 (sensitivitas 88,71%, spesifisitas 93,75%, nilai duga positif 98,21%, nilai duga negatif 68,18%, likelihood ratio positive 14,19, likelihood ratio negative 0,12). Kesimpulan: Hasil uji diagnostik menunjukkan bahwa skor rinitis alergi valid sebagai alat skrining untuk rinitis alergi.
Background: Allergic rhinitis may decrease quality of life. Many screening tools have been developed to know prevalence of allergic rhinitis easily. Screening tool based on condition in Indonesia has not been developed yet. Score for allergic rhinitis was made for quantitative assessment using cut-off point, so make interpretation of the result easier. Objective: to determine cut-off point score for allergic rhinitis and to know diagnostic value of score for allergic rhinitis. Advantage: as a screening tool that is simple and easy to use. Research design: diagnostic test. Location: ENT clinic in Dr. Sardjito hospital, Yogyakarta. Material and methods : Seventy eight persons followed this research. Skin prick test for inhalant allergen was used as gold standard. To perform significance test for correlation of 12 variables with allergic rhinitis, X2 test was used. Variable that has significance correlation was used for score for allergic rhinitis. Every variable that is met in subject pointed as 1 and if it is not met the subject pointed as 0, then total score counted. Receiver Operator Characteristic (ROC) curve was used to determine cut-off point of score for allergic rhinitis. Diagnostic value counted based on cut-off point. Statistic analysis: X2 test and Receiver Operator Characteristic (ROC) curve. Result: Nine variable (rhinorhea, sneezing, nasal obstruction, house dust, animal dander, history of family atopic, turbinate hypertrophy, livide turbinate, discharge serous) used for score for allergic rhinitis with cut-off point ³ 6 (sensitivity 68,18%, specificity 93,75%, positive predictive value 98,12%, negative predictive value 68,18%, likelihood ratio positive 14,19 and likelihood ratio negative 0,12). Conclusion: The result of diagnostic test for score for allergic rhinitis show that score for allergic rhinitis valid as a screening tool.
