EVALUASI ALGORITMA RDW-RBC (RED CELL DISTRIBUTION WIDTH-RED BLOOD CELL) SEBAGAI PEMBEDA BETA THALASSEMIA TRAIT ATAU HEMOGLOBIN E DARI DEFISIENSI BESI

Latar Belakang: Penyebab paling umum kelainan eritrosit mikrositik hipokromik adalah hemoglobinopati dan defisiensi besi. Beta Thalassemia Trait (BTT) atau Hemoglobin E (HbE) adalah bentuk hemoglobinopati yang lazim di Indonesia yang dapat dikonfirmasi dengan analisis hemoglobin. Algoritma RDW-RBC merupakan algoritma yang disusun dari parameter hematologi dengan menggunakan model decision tree untuk membedakan BTT atau HbE dari defisiensi besi pada populasi mikrositik hipokromik. Tujuan: Mengevaluasi kemampuan algoritma RDW-RBC sebagai pembeda BTT atau HbE dari defisiensi besi pada populasi mikrositik atau hipokromik. Metode: Penelitian observasional potong lintang dengan rancangan uji diagnostik, melibatkan peserta skrining talasemia di Departemen Patologi Klinik dan Kedokteran Laboratorium pada bulan Juli – Desember 2021. Kriteria inklusi adalah usia >12 tahun, hemoglobin 9-16 gr/dL, Mean Corpuscular Volume (MCV) <80 fL dan/atau Mean Corpuscular Hemoglobin (MCH) <26 pg. Penetapan diagnosis hemoglobinopati (BTT atau HbE) atau defisiensi besi didasarkan pemeriksaan referensi yaitu algoritma pemeriksaan meliputi feritin, CRP, elektroforesis hemoglobin. Evaluasi kemampuan algoritma RDW-RBC sebagai pembeda BTT dan HbE dari defisiensi besi dibandingkan pemeriksaan referensi menggunakan tabel 2x2, dinyatakan dalam sensitivitas, spesifisitas, Nilai Ramal Positif (NRP), Nilai Ramal (NRN), akurasi, Youden index, Positive Likelihood Ratio (PLR), dan Negative Likelihood Ratio (NLR). Nilai p<0,05 ditetapkan bermakna secara statistik. Analisis data ini menggunakan SPSS versi 25. Hasil: Subjek penelitian sejumlah 118 orang memiliki median usia 22 (12-39) tahun, perempuan 106 orang (89,8%) dan laki-laki 12 orang (10,2%). Pada penelitian ini didapatkan rerata nilai hemoglobin 11,97±1,39 g/dL, MCV 74,15 (56,60-81,30) fL, MCH 23,70 (16,30-27,00) pg, dan RDW 16,25 (12,20-24,80) %. Berdasarkan algoritma pemeriksaan referensi, didapatkan kelompok BTT atau HbE sejumlah 46 orang dan defisiensi besi sejumlah 72 orang. Subjek dari kelompok BTT atau HbE terdiri dari BTT 14 orang dan HbE 32 orang, memiliki median usia 26 (12-36) tahun, terdiri dari laki-laki 12 orang (26,1%) dan perempuan 34 orang (73,9%). Subjek defisiensi besi memiliki median usia 19 (12-39) dan seluruhnya perempuan (100%). Kelompok BTT atau HbE memiliki jumlah eritrosit, hemoglobin, hematokrit, MCHC yang lebih tinggi, serta MCV dan RDW yang lebih rendah dibanding defisiensi besi, p<0,05. Performa diagnostik dari algoritma RDW-RBC sebagai pembeda BTT atau HbE dari defisiensi besi pada populasi mikrositik atau hipokromik memiliki sensitivitas 91,3% (83,16%-99,45%), spesifisitas 70,83% (60,33%-81,33%), NRP 66,67% (55,03%-78,31%), NRN 92,73% (85,86%-99,59%), akurasi 78,81% (71,44%-86,19%), Youden index 0,62 (0,53-0,71), PLR 3,13 (2,16-4,54), dan NLR 0,12 (0,05-0,32). Simpulan: Algoritma RDW-RBC dapat dijadikan pembeda BTT atau HbE dari defisiensi besi pada populasi mikrositik hipokromik, memiliki sensitivitas 91,3%, spesifisitas 70,83%.

Background: The most common causes of hypochromic microcytic erythrocyte disorders are hemoglobinopathy and iron deficiency. Beta Thalassemia Trait (BTT) or Hemoglobin E (HbE) is a form of hemoglobinopathy that is common in Indonesia which can be confirmed by hemoglobin analysis. The RDW-RBC algorithm is an algorithm developed from hematological parameters using the decision tree model to distinguish BTT or HbE from iron deficiency in hypochromic microcytic populations. Objective: To evaluate the ability of the RDW-RBC algorithm to differentiate BTT or HbE from iron deficiency in microcytic or hypochromic populations. Methods: A cross-sectional observational study with a diagnostic test design, involving participants in thalassemia screening at the Department of Clinical Pathology and Laboratory Medicine in July – December 2021. Inclusion criteria were age >12 years, hemoglobin 9-16 gr/dL, Mean Corpuscular Volume (MCV) <80 fL and/or Mean Corpuscular Hemoglobin (MCH) <26 pg. Determination of the diagnosis of hemoglobinopathy (BTT or HbE) or iron deficiency is based on a reference examination, namely the examination algorithm includes feritin, CRP, hemoglobin electrophoresis. Evaluation of the ability of the RDW-RBC algorithm as a differentiator of BTT and HbE from iron deficiency compared to reference checks using a 2x2 table, expressed in sensitivity, specificity, Positive Predictive Value (PPV), Predictive Value (NPV), accuracy, Youden index, Positive Likelihood Ratio (PLR), and Negative Likelihood Ratio (NLR). The value of p<0.05 was determined to be statistically significant. This data analysis uses SPSS version 25. Results: A total of 118 subjects had a median age of 22 (12-39) years, 106 women (89.8%) and 12 men (10.2%). In this study, the average hemoglobin value was 11.97 ± 1.39 g/dL, MCV 74.15 (56.60-81.30) fL, MCH 23.70 (16.30-27.00) pg, and RDW 16.25 (12.20-24.80) %. Based on the reference test algorithm, there were 46 people in the BTT or HbE group and 72 people with iron deficiency. Subjects from the BTT or HbE group consisted of 14 BTT and 32 HbE people, had a median age of 26 (12-36) years, consisted of 12 men (26.1%) and 34 women (73.9%). The iron deficiency subjects had a median age of 19 (12-39) years and all of them were female (100%). The BTT or HbE group had higher erythrocyte counts, hemoglobin, hematocrit, MCHC, and lower MCV and RDW than iron deficiency, p<0.05. The diagnostic performance of the RDW-RBC algorithm has sensitivity of 91.3% (83.16%-99.45%), specificity of 70.83% (60.33%-81.33%), NRP of 66.67% (55.03%-78.31%), NRN of 92.73% (85.86%-99.59%), accuracy of 78.81% (71.44%- 86.19%), Youden index of 0.62 (0.53-0.71), PLR 3.13 of (2.16-4.54), and NLR of 0.12 (0.05-0.32) to differentiate BTT or HbE from iron deficiency in the microcytic or hypochromic population Conclusion: The RDW-RBC algorithm can be used to differentiate BTT or HbE from iron deficiency in hypochromic microcytic populations, has sensitivity of 91.3%, specificity of 70.83%.
