INTISARI Latar Belakang: Low back pain (LBP) adalah salah satu masalah kesehatan yang paling umum terjadi. Penyebab LBP salah satunya adalah intervertebral disc degeneration (IDD) pada regio lumbosacral dan Modic Changes (MC). Dikatakan IDD dapat memicu perubahan struktur disekitarnya dan menyebabkan LBP. MC adalah variasi sinyal dari endplate dan bone marrow vertebra yang terlihat pada pemeriksaan MRI. MC diasumsikan sebagai fitur degeneratif dari endplate dan bone marrow vertebra dan memiliki 3 tipe yang menggambarkan proses patologis yang terjadi. Pada beberapa literatur, terdapat klasifikasi baru untuk menilai derajat keparahan dari suatu lesi MC. Beberapa klasifikasi tersebut adalah klasifikasi oleh Udby et al., untuk tinggi MC (TMC) dan oleh Mu X., et al., untuk luas MC (LMC). Derajat keparahan ini berkorelasi dengan rasa nyeri pada pasien LBP, walaupun demikian klasifikasi tersebut menjadi salah satu parameter yang belum banyak di teliti dan dikorelasikan dengan IDD pada pasien LBP. Modalitas pencitraan standar untuk mendeteksi IDD dan MC adalah MRI Tujuan: Penelitian ini bertujuan untuk mengetahui ada korelasi antara degenerasi DIV (IDD) dengan ukuran MC pada pemeriksaan MRI pasien LBP. Material dan Metode: Penelitian ini menggunakan metode observasional analitik korelasi (cross-sectional) menggunakan data sekunder pada 30 subjek dengan klinis LBP yang telah menjalani pemeriksaan MRI lumbosacral di bagian radiologi RSUP Dr.Sardjito Yogyakarta pada bulan Januari 2021-Juni 2022. Dari 30 subjek didapatkan 150 sampel sesuai level DIV lumbosacral yang terlibat. Dengan menggunakan DICOM MRI, dilakukan evaluasi IDD dengan klasifikasi Schneiderman dan dinilai korelasinya dengan tinggi serta luas MC terlibat. Hasil penelitian dianalisis menggunakan korelasi Spearman. Hasil: Dari 150 sampel, didapatkan 103 sampel dengan IDD dan 89 sampel dengan MC. Pada IDD grade 1, terdapat TMC Negatif: 9 buah (6%), A: 4 buah (2.66%), B: 7 buah (4.66%) dan C: 4 buah (2.66%). Pada IDD grade 2, TMC Negatif:10 buah (6.66%), A: 8 buah (5.33%), B: 17 buah (11.33%) dan C: 8 buah (5.33%). IDD grade 3, TMC Negatif: 0 buah (0%), A: 4 buah (2.66%), B: 12 buah (8%), C: 20 buah (13.33%). IDD grade 0, TMC Negatif: 42 buah (28%), A: 0 buah (0%), B: 4 buah (2.66%) dan C: 1 buah (0.66%). Pada IDD grade 1, didapatkan LMC Negatif: 9 buah (6%), A: 11 buah (7.33%), dan B: 4 buah (2.66%). Pada IDD grade 2, LMC Negatif:10 buah (6.66%), A: 20 buah (13.33%), dan B: 13 buah (8.66%). Pada IDD grade 3, LMC Negatif: 0 buah (0%), A: 12 buah (8%), dan B: 24 buah (16%). LMC C: 0 (0%) pada seluruh sampel. Berdasarkan uji korelasi didapatkan hasil bahwa terdapat korelasi signifikan (p<0.001) antara IDD dengan TMC (r=0.683) maupun LMC (r=0.699) dengan arah positif dan kekuatan korelasi yang kuat. Kesimpulan: Terdapat korelasi yang bermakna secara statistik antar variabel dan semakin besar derajat IDD semakin tinggi pula derajat ukuran MC yang terlibat (berdasarkan klasifikasi) Kata Kunci: Modic Changes, MC, Degenerasi DIV, Korelasi, MRI lumbosacral

CORRELATION OF LUMBAL INTERVERTEBRAL DISC DEGENERATION WITH MODIC CHANGES LESION SIZE ON MRI EXAMINATION OF LOW BACK PAIN PATIENTS Dito Pondra Dharma1, Yana Supriatna2, Hesti Gunarti2 Department of Radiology, Faculty of Medicine, Public Health and Nursing, Universitas Gadjah Mada / Dr. Sardjito General Hospital ABSTRACT Background: Low back pain (LBP) is one of the most common health problems. One of the causes of LBP is intervertebral disc degeneration (IDD) in the lumbosacral region and Modic Changes (MC). It is said that IDD can trigger changes in the surrounding structure and cause LBP. MC is a variation of the signal from the vertebral endplate and bone marrow seen on MRI. MC is assumed to be a degenerative feature of the vertebral endplate and bone marrow and has 3 types that describe the pathological process that occurs. In some literature, there are new classifications to assess the severity of an MC lesion. Some of these classifications are the classification by Udby et al., for MC Height (TMC) and by Mu X., et al., for MC Area (LMC). This degree of severity correlates with pain in LBP patients, although this classification is one of the parameters that has not been widely studied and correlated with IDD in LBP patients. The standard imaging modality for detecting IDD and MC is MRI Objectives: This study aims to determine the correlation between IVD degeneration (IDD) and MC size on MRI examination of LBP patients. Materials and Methods: This study used a correlation analytic observational method (cross-sectional) using secondary data on 30 subjects with clinical LBP who had undergone lumbosacral MRI examinations at the radiology department of Dr. Sardjito Hospital Yogyakarta in January 2021-June 2022. Out of 30 subjects 150 samples were obtained according to the involved lumbosacral IVD level. Using DICOM MRI, IDD was evaluated with the Schneiderman classification and its correlation with the height and area of MC involved was assessed. The results of the study were analyzed using Spearman's correlation. Results: From 150 samples, 103 samples with IDD and 89 samples with MC were obtained. In IDD grade 1, there were Negative TMCs: 9 (6%), A: 4 (2.66%), B: 7 (4.66%) and C: 4 (2.66%). In IDD grade 2, TMC is negative: 10 (6.66%), A: 8 (5.33%), B: 17 (11.33%) and C: 8 (5.33%). IDD grade 3, TMC Negative: 0 pieces (0%), A: 4 pieces (2.66%), B: 12 pieces (8%), C: 20 pieces (13.33%). IDD grade 0, TMC Negative: 42 pieces (28%), A: 0 pieces (0%), B: 4 pieces (2.66%) and C: 1 piece (0.66%). In IDD grade 1, negative LMCs were obtained: 9 (6%), A: 11 (7.33%), and B: 4 (2.66%). At IDD grade 2, LMC was Negative: 10 (6.66%), A: 20 (13.33%), and B: 13 (8.66%). At IDD grade 3, LMC Negative: 0 pieces (0%), A: 12 pieces (8%), and B: 24 pieces (16%). LMC C: 0 (0%) in all samples. Based on the correlation test, it was found that there was a significant correlation (p<0.001) between IDD and TMC (r=0.683) and LMC (r=0.699) with a positive direction and strong correlation strength. Conclusion: There is a statistically significant correlation between variables and the greater the degree of IDD, the higher the degree of MC size involved (based on classification) Key words: Modic Changes, MC, IVD Degeneration, Correlation, lumbosacral MRI
