PENGARUH PEMBERIAN TERAPI ANGIOTENSIN RECEPTOR NEPRILYSIN INHIBITOR DIBANDINGKAN DENGAN ANGIOTENSIN CONVERTING ENZYME INHIBITOR DAN ANGIOTENSIN RECEPTOR BLOCKER TERHADAP KEJADIAN HOSPITALISASI DAN EFEK SAMPING OBAT PADA PASIEN GAGAL JANTUNG KRONIS DENGAN PENURUNAN FRAKSI EJEKSI DI RUMAH SAKIT JANTUNG BINAWALUYA JAKARTA TIMUR

Latar Belakang: Angiotensin Receptor Neprilysin Inhibitor (ARNI) direkomendasikan pada pasien gagal jantung kronis dengan penurunan fraksi ejeksi ventrikel kiri (HFrEF) yang masih bergejala walaupun sudah mendapatkan terapi optimal dengan Angiotensin Converting Enzyme (ACEI) atau Angiotensin Receptor Blocker (ARB). Tujuan Penelitian: Penelitian ini bertujuan mengkaji kejadian hospitalisasi dan efek samping obat pada pasien HFrEF dengan terapi ARNI dibandingkan terapi ACEI/ARB di RS Jantung Binawaluya. Metode: Penelitian ini menggunakan rancangan penelitian kohort retrospektif yang melibatkan 127 pasien HFrEF yang menerima terapi ARNI (17 orang) dan ACEI/ARB (110 orang) di RS Jantung Binawaluya. Data penelitian diperoleh dari rekam medis pasien tahun 2018-2019. Luaran yang diamati adalah kejadian hospitalisasi akibat sebab kardiovaskuler dan kejadian efek samping obat. Hubungan antara jenis terapi dengan kejadian hospitalisasi dianalisis menggunakan analisis kesintasan metode Kaplan-Meier dan cox regression. Hasil Penelitian: Kejadian hospitalisasi teramati pada 47,06 % subjek kelompok ARNI dan 51,82 % subjek kelompok ACEI/ARB. Mean survival time kejadian hospitalisasi akibat penyakit kardiovaskuler kelompok ARNI 254 hari dan ACEI/ARB 297 hari, dengan penurunan risiko hospitalisasi pada kelompok ARNI namun tidak signifikan secara statistik (p=0,142; adjusted-HR= 0,779; 95% CI 0,294-2,061). Terdapat perbedaan signifikan frekuensi hospitalisasi pada kelompok ARNI dibandingkan kelompok ACEI/ARB (p=0,016). Efek samping obat yang ditemukan pada kelompok ARNI yaitu hipotensi (10 kejadian), hiperkalemia (1 kejadian), dan peningkatan serum kreatinin (4 kejadian). Kesimpulan: Tidak terdapat perbedaan yang signifikan dari kejadian hospitalisasi pada pasien HFrEF dengan terapi ARNI atau ACEI/ARB di RS Jantung Binawaluya pada besar sampel minimal. Kelompok ARNI menunjukkan perbedaan frekuensi hospitalisasi dibandingkan kelompok ACEI/ARB. Proporsi kejadian hipotensi sebanyak dua kali lipat pada kelompok terapi ARNI dibandingkan ACEI/ARB namun tidak terdapat perbedaan yang signifikan pada seluruh kejadian efek samping obat di kedua kelompok.

Background: Angiotensin Receptor Neprilysin Inhibitor (ARNI) is recommended in chronic heart failure with reduced ejection fraction (HFrEF) patients who are still symptomatic despite receiving optimal therapy with Angiotensin Converting Enzyme (ACEI) or Angiotensin Receptor Blocker (ARB). Objectives: This study aims to assess the incidence of hospitalization and adverse drug reactions in HFrEF patients receiving ARNI compared to ACEI/ARB. Methods: This study used a retrospective cohort study design involving 127 HFrEF patients who received ARNI (17 people) and ACEI/ARB (110 people) at the Binawaluya Hospital. Data was obtained from the patient's medical records from 2018-2019. The outcomes were the incidence of cardiovascular hospitalization and the incidence of adverse drug reactions. The association between the type of therapy and the incidence of hospitalization was analyzed using survival analysis Kaplan-Meier and Cox regression. Results: Hospitalization was observed in 47.06% of subjects in the ARNI group and 51.82% in the subjects of the ACEI/ARB group. Mean survival time of cardiovascular hospitalization in the ARNI group was 254 days and 297 days in the ACEI/ARB group with hospitalization risk reduction but not significant (p=0.142; adjusted-HR = 0.779; 95% CI 0.294-2.061). There was a significant difference in the frequency of hospitalization in the ARNI group compared to the ACEI/ARB group (p=0.016). The adverse drug found in the ARNI group were hypotension (10 events), hyperkalemia (1 event), and increased creatinine serum (4 events). Conclusions: There was no significant difference on the incidence of hospitalization in HFrEF patients with ARNI or ACEI/ARB therapy at Binawaluya Hospital with minimum sample size. The ARNI group showed differences in the frequency of hospitalization compared to the ACEI/ARB group. The proportion of the incidence of hypotension was doubled in the ARNI group compared to ACEI/ARB group but there was no significant difference in the incidence of all drug side effects.
