Pengaruh Asam Klorogenat (Chlorogenic Acid / CGA) Terhadap Ginjal Tikus Diabetes Mellitus: Kajian Mengenai mRNA TGF-Beta dan Collagen-1

INTISARI Latar Belakang : Diabetes Mellitus merupakan penyakit yang disebabkan oleh kadar glukosa dalam darah yang melebihi ambang batas normal. Kondisi hiperglikemia jangka panjang akan membebani kerja ginjal yang mengarah pada fibrosis ginjal. Asam klorogenat merupakan salah satu antioksidan alami yang dapat menurunkan progresivitas Diabetes Mellitus. Penelitian mengenai pengaruh asam klorogenat terhadap ginjal tikus model DM dengan variasi dosis CGA belum banyak dikembangkan. Tujuan : Penelitian ini bertujuan untuk mengkaji pengaruh CGA terhadap kadar kreatinin dalam serum darah, fibrosis ginjal, ekspresi mRNA TGF-Beta dan kolagen tipe-1 pada ginjal tikus DM. Metode : Desain penelitian quasi eksperimental dengan post-test only with controlled group design. Subjek tikus (Rattus norvegicus) galur Wistar jantan berumur 2 bulan dengan berat badan 150-200 gram dibagi menjadi 6: kelompok kontrol (pemberian saline tanpa STZ dan tanpa CGA); kelompok DM 1,5 (DM 1,5 bulan); kelompok DM 2 (DM 2 bulan); kelompok CGA 1 (DM 1,5 bulan + CGA dosis 12,5 mg/kg/BB); kelompok CGA 2 (DM 1,5 bulan + CGA dosis 25 mg/kg/BB); kelompok CGA 3 (DM 1,5 bulan + CGA dosis 50 mg/kg/BB). Ekspresi mRNA TGF-Beta dan Col-1 dikuantifikasi dengan RT-PCR. Pemeriksaan fibrosis ginjal dilakukan dengan pewarnaan imunohistokimia PDGFR-Beta. Hasil: Gambaran imunohistokimia PDGFR-Beta pada kelompok CGA menunjukkan berkurangnya tampakan sebaran fibroblas dibandingkan kelompok DM. Ekspresi mRNA kelompok CGA 1 dan CGA 2 lebih rendah dibandingkan kelompok DM 1 dan DM 2 (p <= 0,05). Tidak ditemukan perbedaan bermakna pada ekspresi mRNA Col-1 kelompok CGA 1 dan CGA 2 dengan DM. Terdapat peningkatan ekspresi mRNA dan bermakna secara statistik pada kelompok CGA 3 dibandingkan kelompok DM. Kesimpulan: Pemberian CGA menginduksi perbaikan fibrosis ginjal dibandingkan kelompok DM. Pemberian CGA dosis 12,5 mg/kgBB dan 25 mg/KgBB menunjukkan ekspresi mRNA TGF-Beta yang lebih rendah dibandingkan kelompok DM. Pemberian CGA tidak mempengaruhi perubahan ekspresi mRNA Col-1 pada kelompok DM.

ABSTRACT Background: Diabetes Mellitus is a disease caused by glucose levels in the blood that exceed the normal threshold. Long-term hyperglycemia conditions will burden the kidneys, leading to kidney fibrosis. Chlorogenic acid is one of the natural antioxidants that can reduce the progression of Diabetes Mellitus. Research on the effect of chlorogenic acid on the kidneys of DM rats with variations in the dose of CGA has not been developed. Objective: This study aimed to examine the effect of CGA on creatinine levels in blood serum, renal fibrosis, TGF-Beta and Collagen-1 mRNA expression in the kidneys of DM rats. Method: Quasi-experimental research design with post-test only with controlled group design. Rats (Rattus norvegicus) male Wistar strain, aged 2 months, weighing 150-200 grams, were divided into 6: control group (saline administration without STZ and without CGA); group DM 1.5 (DM 1.5 months); DM group 2 (DM 2 months); CGA group 1 (DM 1.5 months + CGA dose 12.5 mg/kg/BW); CGA group 2 (DM 1.5 months + CGA dose 25 mg/kg/BW); CGA group 3 (DM 1.5 months + CGA dose 50 mg/kg/BW). TGF-Beta and Col-1 mRNA expression was quantified by RT-PCR. Renal fibrosis examination was performed by PDGFR-Beta immunohistochemical staining. Results: Immunohistochemical staining of PDGFR-Beta in the CGA group showed a reduced distribution of fibroblasts compared to the DM group. The mRNA expression of CGA 1 and CGA 2 groups was lower than that of DM 1 and DM 2 groups (p <= 0,05). There was no significant difference in the expression of Col-1 mRNA in CGA 1 and CGA 2 groups with DM. There was an increase in mRNA expression and it was statistically significant in the CGA 3 group compared to the DM group. Conclusion: Administration of CGA induced improvement in renal fibrosis compared to the DM group. Administration of CGA doses of 12.5 mg/kgBW and 25 mg/KgBW showed lower TGF-Beta mRNA expression than the DM group. Administration of CGA did not affect changes in Col-1 mRNA expression in the DM group.
