Uji Stabilitas Kadar Albumin Pooled Sera sebagai Alternatif Kontrol Serum di Laboratorium Klinik

Albumin merupakan protein paling melimpah (sekitar 50% dari total protein) dalam serum dan menggambarkan kualitas nutrisi pasien. Albumin menjadi salah satu parameter pemeriksaan di Laboratorium Klinik. Spektrofotometer sebagai alat pengukur kadar albumin di laboratorium klinik membutuhkan kalibrasi harian dengan standar serum komersial. Pengadaan serum komersial yang mahal menjadi kendala bagi laboratorium dasar di puskesmas, laboratorium daerah sehingga diperlukan alternatif serum untuk kalibrasi alat laboratorium. Pooled sera memiliki potensi menggantikan serum kontrol komersial karena memiliki komposisi yang mirip, bersifat homogen dan stabil pada penyimpanan -20 derajat selsius dengan pengawet NaN3 1%. Tujuan penelitian ini adalah mengetahui kestabilan kadar albumin pooled sera pada suhu -20 derajat selsius dalam satu periode grafik kontrol Spektrofotometer. Penelitian ini menggunakan metode eksperimen dengan desain Pre and Post Test serta sampel yang digunakan adalah pooled sera yang didapatkan dengan mengambil plasma segar dari proses apheresis darah donor dan ditambah bahan pengawet NaN3. Analisis data menggunakan uji homogenitas dan stabilitas dengan perhitungan menurut ISO 13528:2015. Sampel serum dikatakan homogen apabila nilai Ss < 0,3 sigma dan disebut stabil apabila |Xr-Yr| < 0,3 sigma. Hasil penelitian menunjukkan bahwa pooled sera dengan penambahan NaN3 1% memiliki nilai Ss 0,03765 < 0.3 sigma 0,97335 pada uji homogenitas, dan serta nilai |Xr-Yr| 0,089 < 0.3 sigma 0,97335 sehingga dapat dikatakan bahwa bahwa pooled sera dengan penambahan pengawet NaN3 1% bersifat homogen dan stabil pada penyimpanan -20 derajat selsius selama satu periode grafik spektrofotometer yaitu 30 hari. Hal ini menunjukkan pool sera berpotensi sebagai bahan untuk kalibrasi alat pengukur albumin di laboratorium.

Albumin is the most abundant protein (about 50% of the total protein) in serum and describes the nutritional quality of the patient. Albumin is one of the measurement parameters in the Clinical Laboratory. The spectrophotometer as a tool for measuring albumin levels in the clinical laboratory requires daily calibration with commercial serum. Procurement of expensive commercial serum is an obstacle for basic laboratories at health center, and regional laboratories so the alternative serum is needed for calibration of laboratory equipment. Pooled sera have the potential to replace commercial control serum because they have a similar composition, are homogeneous, and are stable at -20 degrees celsius storage with 1% NaN3 as a preservative. The purpose of this research was to determine the stability of albumin pooled sera levels at a temperature of -20 degrees celsius in one period of the spectrophotometer control chart. This research used an experimental method with a Pre and Post Test design and the sample used was pooled sera obtained by taking fresh plasma from the apheresis process of donor blood and adding NaN3 as a preservative. Data analysis used homogeneity and stability tests with calculations according to ISO 13528:2015. The serum sample is said to be homogeneous if the value of Ss < 0.3 sigma and is called stable if |Xr-Yr| < 0.3 sigma. The results showed that the value of Ss pooled sera added with 1% NaN3 was 0.03765 which was lower than 0.97335 (value 0.3 sigma from the homogeneity test), and the value of |Xr-Yr| is 0.089 which is lower than 0.97335 (0.3 sigma value of the 30-day stability test). So it can be said that pooled serum with the addition of 1% NaN3 preservative is homogeneous and stable at - 20 degrees celcius storage for one period of the spectrophotometer graph, which is for 30 days. This shows that the sera pool has potential as a material for the calibration of albumin measuring instruments in the laboratory.
