Pengaruh Edukasi Oleh Apoteker Menggunakan Metode Brief Counseling Terhadap Tingkat Pengetahuan, Kepatuhan, dan Luaran Klinik Pada Pasien Hipertensi di Puskesmas di Kota Pangkalpinang

Upaya BPJS Kesehatan dalam meningkatkan pemeliharaan kesehatan dan meningkatkan pengetahuan pasien hipertensi adalah menyelenggarakan program prolanis. Salah satu aktivitas prolanis yaitu pemberian edukasi kelompok. Namun selama pandemi COVID 19, program ini ditiadakan sementara dalam rangka pengendalian penularan infeksi, sehingga diperlukan strategi edukasi dengan metode konseling yang efektif. Penelitian ini bertujuan untuk mengetahui pengaruh edukasi oleh apoteker menggunakan metode brief counseling terhadap tingkat pengetahuan, kepatuhan, dan luaran klinik pada pasien hipertensi di puskesmas di Kota Pangkalpinang. Penelitian ini merupakan quasi experimental pretest and posttest with control group. Bentuk intervensi berupa konseling singkat dengan alat bantu poster dan follow up melalui whatsapp atau telepon pada pasien hipertensi peserta prolanis yang memenuhi kriteria inklusi dengan tekanan darah lebih dari sama dengan 140/90 mmHg. Penelitian dilaksanakan selama bulan September sampai Desember 2021 di 6 puskesmas diantaranya 3 puskesmas intervensi dan 3 puskesmas kontrol. Sampel berjumlah 90 orang yaitu 46 kelompok intervensi dan 44 kelompok kontrol. Data diambil sebanyak dua kali yaitu pada waktu awal sebelum intervensi (pretest) dan satu bulan setelah intervensi (posttest). Instrumen yang digunakan kuesioner HKLS dan MGLS. Analisis data secara statistik menggunakan Wilcoxon test, Independent sampe t test, dan Mann Whitney test. Setelah dilakukan intervensi brief counseling menunjukkan bahwa kelompok intervensi memiliki nilai yang lebih baik dibandingkan kelompok kontrol. Pada kelompok intervensi terdapat peningkatan tingkat pengetahuan yaitu 14,93 kurang lebih 1,756 menjadi 20,57 kurang lebih 1,601 (p kurang dari 0,001), peningkatan kepatuhan 1,54 kurang lebih 1,168 menjadi 0,80 kurang lebih 0,910 (p kurang dari 0,001), penurunan tekanan darah sistolik 162,67 kurang lebih 14,618 menjadi 146,80 kurang lebih 15,809 (p kurang dari 0,001), serta penurunan tekanan darah diastolik 98,96 kurang lebih 7,674 menjadi 87,04 kurang lebih 9,897 (p kurang dari 0,001). Pada kelompok kontrol terjadi perubahan tingkat pengetahuan, kepatuhan serta penurunan tekanan sistolik dan diastolik namun tidak signifikan (p lebih dari 0,05). Dapat disimpulkan bahwa edukasi oleh apoteker menggunakan metode brief counseling dapat meningkatkan tingkat pengetahuan, kepatuhan, dan luaran klnik pasien hipertensi secara bermakna.

BPJS Kesehatan efforts in improving health maintenance and knowledge level of hypertension patients are organizing program called prolanis. One of the prolanis activities is providing group education. However, during the COVID 19 pandemic, this program was temporarily eliminated in order to control transmission on infection, so an educational strategy with effective counseling methods was needed. This study aimed to determine the effect of education by pharmacist using brief counseling methods on knowledge level, adherence, and clinical outcomes of hypertension patients at Pangkalpinang Public Health Center. This study was a quasi experimental pretest and posttest with control group. The form of intervention is brief counseling with posters devices and follow up via whatsapp or telephone for hypertension patients who are prolanis participants that entered inclusion criteria with blood pressure more than equal to 140/90 mmHg. The study was conducted during September until December 2021 in 6 public health centers, such as 3 for intervention and 3 for control groups. The sample total was 90 people, consist of 46 intervention and 44 control groups. Data were taken twice, namely at the beginning before intervention (pretest) and one month after intervention (posttest). The instruments used were HKLS and MGLS questionnaires. The data were statistically analyzed using Wilcoxon test, Independent sample t test, and Mann Whitney test. After brief counseling showed that in the intervention group had better scores than the control group. In the intervention group there was an increased on knowledge level 14,93 more or less 1,756 to 20,57 more or less 1,601 (p less than 0,001), an increased adherence 1,54 more or less 1,168 to 0,80 more or less 0,910 (p less than 0,001), a decrease on systolic blood pressure 162,57 more or less 14,618 to 146,80 more or less 15,809 (p less than 0,001), and a decreased on diastolic blood pressure 98,96 more or less 7,674 to 87,04 more or less 87,04 more or less 9,897 (p less than 0,001). In the control group there was a change on knowledge level, adherence, and decrease systolic and diastolic blood pressure but not significant (p more than 0,05). So, it can be concluded that an education by pharmacist using brief counseling methods can significantly increase the knowledge level, adherence, and clinical outcomes of hypertension patients.
