Hubungan Derajat Kalsifikasi Knob Aorta Pada Citra Scanogram Dengan Derajat Skor Kalsium Arteri Koroner Menggunakan Metode Agatston Pada Pemeriksaan Ct Scan-Kardiak

Latar Belakang : Penyakit kardiovaskular (PK) masih menjadi masalah dan merupakan penyakit yang berperan utama sebagai penyebab kematian nomor satu di dunia. Salah satu spektrum terbanyak PK adalah penyakit jantung koroner (PJK). PJK sering disebabkan karena penumpukan plak kalsifikasi yang disebut aterosklerosis. Aterosklerosis merupakan proses kelainan sistemik. CT-kardiak merupakan teknik pencitraan untuk menilai aterosklerosis, baik arteri koroner maupun arkus aorta. Salah satu tahapan pemeriksaan CT-kardiak adalah pembuatan scanogram yang hasilnya menyerupai citra radiografi polos dada. Radiografi polos dada masih sering digunakan untuk evaluasi nyeri dada dan dapat digunakan untuk menilai kalsifikasi knob aorta. Tujuan : Untuk mengetahui hubungan antara derajat kalsifikasi knob aorta pada citra scanogram dengan derajat skor kalsium arteri koroner menggunakan metode Agatston pada pemeriksaan CT-kardiak. Metode : Penelitian observasional, metode analisis korelatif, desain cross-sectional, dengan pengambilan data secara retrospektif. Menggunakan data sekunder citra scanogram dada untuk mengetahui derajat kalsifikasi knob aorta dan ekspertise CT-kardiak untuk mengetahui derajat skor kalsium arteri koroner menggunakan metode Agatston. Uji korelasi antar variabel menggunakan uji Spearman. Hasil : Hasil uji Spearman menunjukkan adanya hubungan yang bermakna antara derajat kalsifikasi knob aorta pada citra scanogram dengan derajat skor kalsium arteri koroner menggunakan metode Agatston pemeriksaan CT-kardiak (p=0,001, koefisien korelasi r=0.421). Kesimpulan : Terdapat hubungan yang bermakna antara derajat kalsifikasi knob aorta pada citra scanogram dengan derajat skor kalsium arteri koroner menggunakan metode Agatston pemeriksaan CT-kardiak, dengan kekuatan hubungan sedang dan arah korelasi postifi yang berarti semakin tinggi derajat kalsifikasi knob aorta pada citra scanogram, maka semakin tinggi derajat skor kalsium arteri koroner menggunakan metode Agatston pemeriksaan CT-kardiak.

Background : Cardiovascular disease (CD) are still a problem and disease that plays a major role, as number one cause human death in the world. One of the largest spectrums of cardiovascular disease is coronary heart disease (CHD). CHD is often caused due to the accumulation of calcified plaque called atherosclerosis. Atherosclerosis is a systemic process. Cardiac-CT is one of the imaging techniques used to assess atherosclerosis in both, coronary arteries and aortic knob. Making scanogram is one of the stages on the Cardiac-CT examination, whose the results, similar with plain chest radiograph image. Plain chest radiograph are still frequently used for evaluation of chest pain and can be used to assess aortic knob calcification. Objective : To determine the relationship between degree of aortic knob calcification on the scanogram image with degree of coronary artery calcium score using Agatston methods on Cardiac-CT examination. Methods : Observational research with correlative analysis method, cross-sectional design, and retrospective data collection. This study used secondary data from chest scanogram images to determine the degree of calcification in the aortic knob and Cardiac-CT expertise to determine the degree of coronary artery calcium score using the Agatston methods. This study used Spearman test to test the correlation between variables. Results : The results of the Spearman test showed a significant relationship between the degree calcification of the aortic knob on the scanogram image and the degree of coronary artery calcium score using the Agatston methods in Cardiac-CT examination (p=0.001, correlation coefficient r=0.421). Conclusion : There is a significant relationship between the degree of calcification of the aortic knob on the scanogram image and the degree of coronary artery calcium score using the Agatston methods in Cardiac-CT examination, with a moderate strength relationship and positive direction, which means that, the higher degree of calcification of the aortic knob on the scanogram image, the higher degree of coronary artery calcium score using the Agatston methods on Cardiac-CT examination.
