HUBUNGAN ANTARA KATETER URIN DENGAN INFEKSI SALURAN KEMIH (ISK) KARENA UROPATOGEN PEMBENTUK BIOFILM

Latar belakang: Infeksi saluran kemih (ISK) merupakan penyakit infeksi tersering kedua setelah infeksi pernapasan. Biofilm adalah komunitas mikroorganisme yang saling menempel satu sama lain atau ke permukaan dan terbungkus dalam substansi polimer ekstraselular (EPS). Biofilm juga merupakan penyebab kegagalan terapi ISK terutama pada pasien dengan pemakaian kateter urin yang cukup signifikan, sehingga diperlukan analisis terhadap risiko penyebab ISK yang dikarenakan oleh biofilm. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara kateterisasi urin pada ISK karena uropatogen pembentuk biofilm dan ISK karena uropatogen bukan pembentuk biofilm. Metode: Penelitian ini merupakan penelitian observasional analitik dengan desain case control pada pasien ISK di RSUP Dr. Sardjito. Kriteria inklusi adalah pasien rawat inap dengan diagnosis ISK dan kultur urin tumbuh bakteri yang signifikan sebagai patogen. Analisis univariat untuk data deskriptif. Analisis bivariat digunakan untuk melihat hubungan antar variabel. Variabel katagorik menggunakan uji Chi Square dan variabel numerik menggnakan uji t Independent bila data berdistribusi normal, bila tidak normal digunakan uji Mann Whitney. Uji normalitas menggunakan uji Shapiro Wilk. Data dianalisis dengan menggunakan software SPSS versi 24.0. Hasil: Diperoleh 50 subjek terdiri dari ISK yang disebabkan oleh patogen pembentuk biofilm sebanyak 27 subjek (kasus) dan bukan pembentuk biofilm 23 subjek (kontrol). Enterococcus faecalis merupakan isolat terbanyak yang membentuk biofilm (40,7%) diikuti E. coli (25,9%). Subjek dengan kateter urin memiliki patogen pembentuk biofilm sebesar 48,1% dibandingkan bukan pembentuk biofilm (13%) dengan p=0,008 berbeda signifikan secara statistik. Lama pemakaian kateter urin >5 hari terdapat 76,9% pada patogen pembentuk biofilm dengan p=0,036 berbeda signifikan. Kateter urin merupakan faktor risiko terjadinya ISK yang disebabkan oleh patogen pembentuk biofilm dengan OR=6,19, IK95% 1,48-25,84. Kesimpulan: Tindakan kateterisasi urin dapat meningkatakan risiko terjadinya ISK yang disebabkan oleh patogen pembentuk biofilm sebesar 6,19 kali.

Background: Urinary tract infection (UTI) is the second most common infectious disease after respiratory infection. Biofilms are communities of microorganisms that stick to each other or to surfaces and are encased in an extracellular polymer substance (EPS). Biofilm is also a cause of failure of UTI therapy, especially in patients with urinary catheter use, so it is necessary to analyze the risk of UTI caused by biofilm. Objective: This study aims to determine the relationship between urinary catheters in UTI due to biofilm-forming uropathogens and UTI due to biofilm-forming uropathogens. Method: This study is an analytic observational study with a case control design in UTI patients at Dr. Sardjito. Inclusion criteria were hospitalized patients with a diagnosis of UTI and urine culture growing significant bacteria as pathogens. Univariate analysis for descriptive data. Bivariate analysis was used to see the relationship between variables. The categorical variables used the Chi Square test and the numerical variables used the Independent T test if the data was normally distributed, if not normal, the Mann Whitney test was used. The normality test used the Shapiro Wilk test. The data were analyzed using SPSS software version 24.0. Results: There were 50 subjects consisting of UTI caused by biofilm-forming pathogens as many as 27 subjects (cases) and 23 subjects (controls) non-biofilm-forming. Enterococcus faecalis was the most isolate that formed biofilms (40.7%) followed by E. coli (25.9%). Subjects with urinary catheter had 48.1% biofilm-forming pathogens compared to non-biofilm-forming (13%) with p=0.008 statistically significant difference. Urinary catheter use >5 days was found in 76.9% of biofilm-forming pathogens with p=0.036 significantly different. Urinary catheter is a risk factor for UTI caused by biofilm-forming pathogens with OR=6.19, 95% CI 1.48-25.84. Conclusion: Urinary catheterization can increase the risk of UTI caused by biofilm-forming pathogens by 6.19 times
