Pengaruh Asam Klorogenat Terhadap Ekspresi mRNA HGF, BMP-7 dan Collagen type-1 pada Jantung Tikus Model Diabetes Mellitus

Latar Belakang: Kondisi hiperglikemik pada Diabetes Mellitus (DM) berkaitan erat dengan kerusakan organ terminal serta fibrosis terutama pada jantung. Fibrosis akibat kerusakan otot jantung merupakan kunci terjadinya kardiomiopati diabetikum. Hepatocyte Growth Factor (HGF) dan Bone Morphogenetic Protein-7 (BMP-7) memiliki peran dalam jalur antifibrosis untuk mencegah progresi fibrosis jantung. Sedangkan Collagen type-1 merupakan salah satu produk pada jalur fibrosis yang diperantarai oleh Transforming Growth Factor-β (TGF-β). Asam klorogenat (chlorogenic acid/CGA) yang terdapat pada biji kopi hijau memiliki peran kardioprotektif namun belum disebutkan secara jelas. Tujuan: Mengkaji pengaruh asam klorogenat terhadap ekspresi mRNA HGF, BMP-7, dan Collagen type-1 pada jantung tikus model diabetes mellitus. Metode: Penelitian ini menggunakan tiga puluh ekor tikus jantan berumur 2 bulan yang terbagi dalam 6 kelompok: kelompok kontrol, kelompok Diabetes Melitus 1,5 bulan, kelompok DM 2 bulan, kelompok DM 1,5 bulan dengan pemberian 3 dosis CGA. Model diabetes mellitus dibuat dengan injeksi streptozotocin 60 mg/kgBB. Hewan coba kemudian diterminasi lalu ekspresi marker fibrosis dan antifibrosis diukur melalui pengukuran densitometri pada mRNA Collagen type-1, HGF, dan BMP-7. Hasil: Ekspresi mRNA Collagen type-1 pada kelompok DM2 lebih tinggi dibandingkan kelompok kontrol dan berbeda bermakna. Ekspresi mRNA HGF tidak menunjukkan perbedaan antar kelompok meskipun terdapat adanya tren yang lebih tinggi pada kelompok CGA dibandingkan kelompok DM1,5 dan DM2. Selain itu, ekspresi mRNA BMP-7 juga tidak menunjukkan perbedaan antar kelompok walaupun terdapat kecenderungan ekspresi pada kelompok CGA1, CGA2 dan CGA3 yang dibandingkan kelompok DM1,5 dan DM2. Kesimpulan: Asam klorogenat tidak mempengaruhi ekspresi mRNA HGF, BMP-7, dan Collagen type-1.

Background: Hyperglycemic conditions in Diabetes Mellitus (DM) are closely related to end-organ damage and fibrosis, especially in the heart. Fibrosis due to damage to the heart muscle is the key to diabetic cardiomyopathy. Hepatocyte Growth Factor (HGF) and Bone Morphogenetic Protein-7 (BMP-7) have a role in the antifibrosis pathway to prevent the progression of cardiac fibrosis. While Collagen type-1 is one of the products in the fibrosis pathway mediated by Transforming Growth Factor-β (TGF-β). Chlorogenic acid (CGA) contained in green coffee beans has a cardioprotective role but has not been clearly stated. Purpose: To examine the effect of CGA on mRNA expression of HGF, BMP-7 and Collagen type-1 in the heart of a diabetic mellitus rat model. Method: This study used thirty 2-month-old male rats which were divided into 6 groups: control group, 1.5-month diabetes mellitus group, 2-month DM group, 1.5-month-old DM group with 3 doses of CGA. Diabetes mellitus model was made by injection of streptozotocin 60 mg/kgBW. The experimental animals were then terminated and the expression of fibrosis and antifibrosis markers was measured by densitometric measurements on Collagen type-1, HGF, and BMP-7 mRNA. Result: Collagen type-1 mRNA expression in the DM2 group was higher than the control group and was significantly different. HGF mRNA expression did not show a difference between groups where a higher trend was found in the CGA group than in the DM1,5 and DM2 groups. In addition, the expression of BMP-7 mRNA also did not show a difference between groups where a trend of expression was found in the CGA1, CGA2 and CGA3 groups which were lower than in the DM1,5 and DM2 groups. Conclusion: The CGA did not affect the expression of HGF, BMP-7 nor Collagen type-1 mRNA.
