Pengeluaran Biaya Kesehatan Pada Masyarakat Dengan Penyakit Hipertensi Berdasarkan Pemilihan Fasilitas Kesehatan

Hipertensi meningkatkan mortalitas, morbiditas dan biaya kesehatan. Penelitian ini bertujuan untuk mengetahui hubungan dan perbedaan rata-rata biaya kesehatan berdasarkan faktor-faktor yang mempengaruhi pemilihan fasilitas kesehatan. Penelitian ini merupakan penelitian observasional dengan desain cross-sectional yang dilakukan pada masyarakat dengan hipertensi dan menjadi peserta Program Pengelolaan Penyakit Kronis (Prolanis) di Puskesmas kabupaten Madiun, kabupaten Gunung Kidul, kota Surakarta dan kota Surabaya. Responden sebanyak 120 orang dipilih menggunakan teknik convenience sampling. Penelitian dilakukan pada bulan September-November 2021. Uji Chi-square dilakukan untuk mengetahui hubungan faktor-faktor yang mempengaruhi pemilihan fasilitas kesehatan. Uji Mann Whitney dilakukan untuk mengetahui perbedaan rata-rata pengeluaran biaya kesehatan dengan faktor-faktor yang mempengaruhi pemilihan fasilitas kesehatan. Hasil penelitian menunjukkan persepsi sistem layanan kesehatan berpengaruh signifikan terhadap pemilihan fasilitas kesehatan (p=0,013). Usia (p=0,037), persepsi sistem layanan kesehatan (p=0,032) dan pemilihan fasilitas kesehatan (p<0,001) berpengaruh signifikan terhadap rata-rata pengeluaran biaya medis langsung. Jarak ke fasilitas kesehatan (p<0,001) dan persepsi penyakit (p=0,023) berpengaruh signifikan terhadap rata-rata pengeluaran biaya non medis langsung. Total cost of illness periode 3 bulan untuk hipertensi adalah Rp 78.052,18 dan biaya non medis langsung merupakan komponen terbesar dari cost of illness (Rp 23.505,83). Kunjungan ke fasilitas kesehatan lain menyebabkan peningkatan beban ekonomi pada pasien. Oleh karena itu, penelitian ini berupaya mencegah pengeluaran biaya langsung (medis dan non medis) dan produktivitas yang hilang akibat penyakit hipertensi.

Hypertension increases mortality, morbidity and health costs. This study aims to determine the relationship and the difference in average health costs based factors that influence the selection of health facilities. This study is an observational study with a cross-sectional design conducted on people with hypertension and participating in the Chronic Disease Management Program (Prolanis) at Madiun, Gunung Kidul district, Surakarta and Surabaya city. Total of respondents is 120 people selected using convenience sampling technique. The research was conducted in September-November 2021. Chi-square test was conducted to determine the relationship of factors that influence the selection of health facilities. The Mann Whitney test was conducted to determine the difference between the average health expenses and the factors that influence the selection of health facilities. The results showed that sociodemographic characteristics and disease perception had no significant effect on the selection of health facilities. The perception of the health care system had a significant effect on the selection of health facilities (p=0.013). Age (p=0,037), perception of the health care system (p=0,032) and the selection of health facilities (p<0,001) had a significant effect on the mean of direct medical cost. Distance to health facilities (p<0,001) and perception of disease (p=0,023) had a significant effect on the mean of direct non-medical cost. The total cost of illness for a 3-month period is IDR 78.052,18 and direct non-medical cost are the largest component of the cost of illness (IDR 23,505.83). Visiting other health facilities cause an increase in the economic burden on patients. Therefore, this study seeks to prevent direct cost (medical and non-medical) and lost productivity due to hypertension.
