Analisis Distribusi Dosis Terapi Proton pada Kasus Kanker Medulloblastoma dengan PHITS 3.24

Salah satu jenis radioterapi yang sedang dikembangkan adalah terapi radiasi proton. Sementara itu, terbatasnya jumlah fasilitas terapi proton yang ada saat ini membuat riset yang berkaitan dengan terapi proton tidak dapat dilakukan dengan mudah. Oleh karena itu, perlu adanya alternatif simulasi terapi proton menggunakan program selain yang ada di fasilitas terapi proton. Penelitian ini ditujukan untuk menyimulasikan kanker otak medulloblastoma yang sering dialami oleh anak-anak. Program yang digunakan dalam penelitian ini adalah PHITS versi 3.24. Tubuh manusia dimodelkan dengan fantom revised ORNL-MIRD anak usia 10 tahun. Skema terapi yang disimulasikan adalah whole posterior fossa boost sebesar 19,8 Gy. Teknik proton passive scattering disimulasikan dengan melewatkan berkas proton yang seragam melalui bukaan dan kompensator dengan variasi energi. Teknik proton pencil beam scanning disimulasikan dengan berkas-berkas kecil berbentuk silinder dengan jari-jari 0,5 cm yang disesuaikan dengan target terapi dengan variasi layer. Waktu total pemberian dosis perencanaan selama 550 detik dengan teknik passive scattering dan 605 detik dengan teknik pencil beam scanning. Pada teknik passive scattering OAR dengan persentase dosis serap terbesar adalah kulit, tulang tengkorak, dan otot, masing-masing sebesar 8,22 ± 0,15 %, 5,51 ± 0,05 % dan 1,39 ± 0,04 % dibandingkan dengan dosis toleransi maksimum, sedangkan pada teknik pencil beam scanning, OAR dengan persentase dosis serap terbesar adalah kulit, tulang tengkorak dan tulang belakang dengan masing-masing sebesar 5,42 ± 0,08 %, 4,43 ± 0,05 % dan 0,51 ± 0,05 % dari dosis toleransi maksimum. Distribusi dosis serap dengan teknik passive scattering relatif lebih baik dibandingkan dengan teknik pencil beam scanning dalam parameter homogenitas dosis menggunakan analisis cuplikan dosis di beberapa titik sembarang dalam volume target terapi.

One of the developments in particle therapy is proton radiation therapy. Meanwhile, limited number of proton therapy facilities currently available makes research related to proton therapy difficult. Therefore, there is a need for alternative proton therapy simulations using programs other than those in proton therapy facilities. This research was aimed to simulate medulloblastoma brain cancer which is often experienced by children. The program used in this research was PHITS version 3.24. The human body was modeled with the revised ORNL-MIRD phantom for a 10 year-old child. The therapy scheme was a whole posterior fossa boost of 19.8 Gy. The proton passive scattering was simulated by passing a uniform proton beam through the aperture and compensator with energy variations. The proton pencil beam scanning was simulated with small cylindrical beams with a radius of 0.5 cm which were adjusted to the planning target volume with layers variations. The total duration to give the prescription dose was 550 seconds with passive scattering and 605 seconds with pencil beam scanning. In passive scattering, the largest absorbed dose of OARs were skin, skull bones, and muscles, i.e. 8,22 ± 0,15 %, 5,51 ± 0,05 % and 1,39 ± 0,04 % respectively to their maximum tolerated dose, while in the pencil beam scanning, the largest absorbed dose of OARs were the skull, skin, and spine, i.e. 5,42 ± 0,08 %, 4,43 ± 0,05 % and 0,51 ± 0,05 % respectively to their maximum tolerated dose. Dose distribution in passive scattering is relatively better than in pencil beam scanning in term of dose homogeneity using dose sampling analysis at some points within the planning target volume.
