PENGARUH PENGGUNAAN SABUN ANTISEPTIK BERBAHAN AKTIF KOMBINASI THYMOL DAN EKSTRAK Perilla frutescens TERHADAP PERTUMBUHAN Staphylococcus epidermidis PADA KULIT

Latar Belakang: Antiseptik adalah zat kimia yang menonaktifkan atau menghambat pertumbuhan mikroorganisme. Penggunaan antiseptik kovensional telah banyak menyebabkan munculnya resistensi silang terhadap berbagai antibiotik padahal perkembangan penemuan antibiotik jenis baru tidak sepadan dengan resistensi bakteri yang ada. Badan Pengawas Obat dan Makanan Amerika Serikat (FDA) melarang 24 bahan antiseptik perawatan kesehatan yang dijual bebas termasuk triklosan dan beberapa senyawa berbasis yodium. Hal ini memunculkan permintaan pasar akan antiseptik jenis baru. Minyak esensial yang berasal dari tanaman merupakan senyawa yang paling banyak diteliti sifat antimikrobanya sebagai pengganti anseptik konvensional. Thymol dan ekstrak Perilla fructescens diketahui memiliki efek antibakteri terhadap gram positif dan negatif. Penggunaan sabun sehari-hari dapat menyebabkan rusaknya fungsi sawar kulit yang ditandai dengan peningkatan derajat keasaman, peningkatan Trans Epidermal Water Loss (TEWL), dan penurunan hidrasi kulit. Kerusakan fungsi sawar kulit akibat penggunaan sabun disebabkan oleh kandungan surfaktan. Bahan aktif dari ekstrak tumbuh-tumbuhan memiliki aktivitas antibakteri dengan mengganggu lipid bilayer pada dinding sel bakteri dan memiliki target yang sama pada lipid interselular pada kulit Tujuan: Penelitian ini bertujuan untuk menginvestigasi efek penggunaan antiseptik yang berbahan aktif kombinasi thymol dan ekstrak Perilla frutescens terhadap S. epidermidis serta fungsi sawar kulit. Metode: Penelitian ini merupakan penelitian eksperimental. Subjek menjalani periode wash out selama 1 minggu. Setelah menyelesaikan periode wash out, setiap subjek akan dilakukan pengukuran TEWL, pH, hidrasi kulit, dan kultur bakteri dari daerah lengan pada fossa cubiti. Subjek kemudian akan menggunakan sabun plasebo dan antiseptik dengan bahan aktif kombinasi thymol 0,1% dan ekstrak Perilla frutescens 0,1% sebanyak 1,5 ml sebanyak dua kali sehari pada pukul 07.00-08.00 WIB dan 15.00-16.00 WIB. Setelah 2 minggu, masing-masing subjek akan dilakukan pengukuran TEWL, pH, hidrasi kulit, dan kultur bakteri kembali pada daerah yang sama dengan kultur yang pertama. Analisa data dilakukan untuk mengetahui perubahan koloni S. epidermidis dari waktu ke waktu, selisih penurunan koloni S. epidermidis antara kelompok plasebo dan antiseptik, serta perubahan fungsi sawar kulit dari hari ke-0 sampai hari ke-14. Hasil: Hasil penelitian menunjukkan penurunan koloni S. epidermidis yang signifikan setelah aplikasi sabun antiseptik pada 1 jam dan 6 jam setelah aplikasi sabun pertama pada hari ke-1, serta pada hari ke-14 setelah perlakuan (p< 0,001). Hasil penelitian menunjukkan penurunan koloni S. epidermidis yang signifikan setelah aplikasi sabun plasebo pada 1 jam dan 6 jam setelah aplikasi sabun pertama pada hari ke-1, serta pada hari ke-14 setelah perlakuan (p< 0,001). Selisih penurunan jumlah koloni pada kelompok sabun antiseptik berbeda signifikan dengan penurunan koloni S. epidermidis pada kelompok plasebo pada 6 jam setelah aplikasi sabun pada 6 jam setelah aplikasi sabun pertama pada hari ke-1. Tidak terdapat perbedaan yang signifikan pada penurunan koloni S. epidermidis pada kelompok antiseptik dan plasebo pada 1 jam setelah aplikasi sabun pertama pada hari ke-1 dan pada hari ke-14 setelah perlakuan. Penggunaan sabun antiseptik dengan bahan aktif thymol dan ekstrak Perilla frutescens meningkatkan pH kulit, meningkatkan TEWL, dan menurunkan hidrasi kulit secara signifikan (p<0,05) Kesimpulan: Sabun antiseptik dengan bahan aktif thymol dan Perilla frutescens mempengaruhi kepadatan koloni S. Epidermidis pada kulit. Sabun antiseptik dengan bahan aktif thymol dan Perilla frutescens menurunkan koloni S. Epidermidis secara signifikan lebih banyak dibanding sabun plasebo pada 6 jam setelah aplikasi sabun pertama pada hari ke-1. Sabun antiseptik dengan bahan aktif thymol dan Perilla frutescens mempengaruhi fungsi sawar kulit yang meliputi peningkatan pH, peningkatan TEWL, dan penurunan hidrasi kulit.

Background: Antiseptics are chemical substances that inactivate or inhibit the growth of microorganisms. The use of conventional antiseptics has led to the emergence of cross-resistance to various antibiotics even though the development of the discovery of new types of antibiotics is not commensurate with the resistance of existing bacteria. The United States Food and Drug Administration (FDA) has banned 24 over-the-counter health care antiseptics including triclosan and some iodine-based compounds. This gives rise to market demand for a new type of antiseptic. Essential oils derived from plants are the compounds that have been the most studied for their antimicrobial properties as a substitute for conventional antiseptics. Thymol and Perilla fructescens extract are known to have antibacterial effects against gram-positive and gram-negative. Daily use of soap can cause damage to the skin barrier function which is characterized by an increase in acidity, an increase in Trans Epidermal Water Loss (TEWL), and a decrease in skin hydration. Damage to the skin barrier function due to the use of soap is caused by the content of surfactants. The active ingredients of plant extracts have antibacterial activity by disrupting the lipid bilayer of the bacterial cell wall and having the same target on intercellular lipids in the skin. Objective: This study aimed to investigate the effect of using an antiseptic with the active ingredients of a combination of thymol and Perilla frutescens extract on S. epidermidis and skin barrier function. Methods: This research is an experimental study. Subjects underwent a wash out period of 1 week. After completing the wash out period, each subject will be measured for TEWL, pH, skin hydration, and bacterial culture from the arm area in the cubital fossa. Subjects will then use placebo soap and antiseptic with the active ingredient combination of 0.1% thymol and 0.1% Perilla frutescens extract as much as 1.5 ml twice a day at 07.00-08.00 WIB and 15.00-16.00 WIB. After 2 weeks, each subject will be measured for TEWL, pH, skin hydration, and bacterial culture again in the same area as the first culture. Data analysis was carried out to determine the changes in S. epidermidis colonies from time to time, the difference in the decline in S. epidermidis colonies between the placebo and antiseptic groups, and changes in skin barrier function from day 0 to day 14. Results: The results showed a significant decrease in S. epidermidis colonies after antiseptic soap application at 1 hour and 6 hours after the first soap application on day 1, and on day 14 after treatment (p < 0.001). The results showed a significant decrease in S. epidermidis colonies after placebo soap application at 1 hour and 6 hours after the first soap application on day 1, and on day 14 after treatment (p < 0.001). The difference in the decrease in the number of colonies in the antiseptic soap group was significantly different from the decrease in S. epidermidis colonies in the placebo group at 6 hours after soap application at 6 hours after the first soap application on day 1. There was no significant difference in the decrease in S. epidermidis colonies in the antiseptic and placebo groups at 1 hour after the first soap application on day 1 and on day 14 after treatment. The use of antiseptic soap with the active ingredient thymol and Perilla frutescens extract increased skin pH, increased TEWL, and significantly decreased skin hydration (p<0.05) Conclusion: Antiseptic soap with active ingredients thymol and Perilla frutescens affects the density of S. Epidermidis colonies on the skin. Antiseptic soap with active ingredients thymol and Perilla frutescens reduced S. Epidermidis colonies significantly more than placebo soap at 6 hours after the first soap application on day 1. Antiseptic soap with active ingredients thymol and Perilla frutescens affects the function of the skin barrier which includes an increase in pH, an increase in TEWL, and a decrease in skin hydration.
