Efektivitas Nanopartikel Ekstrak Etanol Daun Sirih Merah (Piper Crocatum) Terhadap Kadar Glukosa Darah, Ekspresi Reseptor Insulin, GLUT4 dan Hsp70 Intraseluler pada Tikus Diabetes Mellitus Tipe 2

Diabetes melitus tipe 2 (DMT2) merupakan penyakit degenerative yang memiliki karakteristik terjadinya hiperglikemia dan resistensi insulin. Salah satu teknologi yang kini terus dikembangkan dengan adalah teknologi nanopartikel dengan kandungan bahan herbal yang bertujuan untuk meningkatkan bioavailabilitas, absorbsi maupun waktu paruh obat herbal tersebut. Salah satu tanaman herbal yang telah diketahui memiliki bahan aktif yang bersifat antidiabetic adalah sirih merah. Penelitian ini bertujuan untuk mengetahui efek nanopartikel ekstrak etanol daun sirih merah terhadap kadar glukosa darah, ekspresi reseptor insulin, GLUT4 dan Hsp70 intraseluler (iHsp70) pada tikus model DMT2. Tiga puluh ekor tikus Wistar jantan dewasa dikelompokan secara acak menjadi 5 kelompok , masing-masing 6 ekor tikus; KN: kontrol normal, KD: kontrol diabetes yang diinduksi STZ-NA, Perlakuan 1: P1, Perlakuan 2: P2 dan Perlakuan 3: P3 yang diinduksi STZ-NA dan diberi nanopartikel ekstrak etanol daun sirih merah dosis 30, 60 dan 90 mg/Kg BB masing-masing kelompok. Perlakuan dilakukan selama 28 hari berturut-turut, sekali sehari. Pengambilan darah untuk pemeriksaan kadar glukosa darah dilakukan sebelum perlakuan, hari ke 7 dan 29. Hari ke-29 perlakuan, tikus dietanasi untuk dikoleksi sampel darah, organ hepar, otot skelet, otot jantung dan ginjal. Hasil pengukuran glukosa darah puasa tikus dianalisis menggunakan metode twoway Anova dan dilanjutkan dengan uji Duncan Multiple Range Test serta diuji dengan uji regresi linear berganda. Ekspresi reseptor insulin (IR), GLUT4 dan iHsp70 diwarnai dengan metode imunohistokimia. Seluruh hasil pengamatan imunoreaktivitas pada organ dianalisis menggunakan uji Kruskal-Wallis dan dilanjutkan dengan uji Mann-Whitney. Hasil penelitian ini menunjukkan adanya penurunan kadar glukosa darah puasa tikus yang signifikan (p < 0,05) akibat pemberian nanopartikel ekstrak etanol daun sirih merah dosis 30, 60 dan 90 mg/KgBB dibandingkan dengan kelompok kontrol diabetes. Kadar glukosa darah puasa terendah dicapai dengan pemberian nanopartikel dosis 90 mg/Kg BB. Pemberian nanopartikel dosis 30, 60 dan 90 mg/Kg BB juga dapat meningkatkan ekspresi reseptor insulin pada otot skelet, GLUT4 pada otot skelet dan jantung dan iHsp70 pada hepar dan ginjal. Berdasarkan hasil penelitian ini dapat disimpulkan bahwa pemberian nanopartikel ekstrak etanol daun sirih merah berpotensi memperbaiki kondisi DMT2 pada tikus dengan menurunkan kadar glukosa darah puasa serta meningkatkan ekspresi reseptor insulin, GLUT4 dan iHsp70.

Diabetes type 2 (DMT2) is a degenerative disease characterized by hyperglycemia and insulin resistance. Nanoparticle technology with herbal substances is currently being explored with the objective of enhancing the bioavailability, absorption, and half-life of these herbal medicines. Red betel is one of the herbal plants recognized to provide anti-diabetic bioactive components. The aim of the research is to see how nanoparticles of red betel leaf ethanol extract affect blood glucose levels, insulin receptor expression, GLUT4, and intracellular Hsp70 (iHsp70) in the DMT2 rat model. Thirty adults male Wistar rats were randomly assigned to one of five groups of six rats each: KN: normal control, KD: STZ-NA-induced diabetes control, Treatment 1: P1, Treatment 2: P2, and Treatment 3: P3 induced by STZ-NA and given nanoparticles of ethanol extract of red betel leaves at doses of 30, 60, and 90 mg/Kg BW. The treatment was administered once a day for 28 consecutive days. Before the treatment, on days 7 and 29 blood samples were collected to measure the fasting blood glucose levels prior to the treatment. Rats were sacrificed on the 29th day of treatment to collect blood, liver, skeletal muscle, cardiac muscle, and kidney samples. The data of fasting blood glucose levels in rats were analyzed using the two-way Anova test, followed by the Duncan Multiple Range Test, and verified using multiple linear regression. Immunohistochemistry was used to determine the expression of insulin receptors (IR), GLUT4, and iHsp70. The Kruskal Wallis test was used to assess all observations of immunoreactivity in organs, followed by the Mann-Whitney test. The administration of red betel leaf ethanol extract nanoparticles at dosages of 30, 60, and 90 mg/KgBW resulted in a significant reduction of fasting blood glucose levels of rats (p < 0.05) compared to KD. The lowest fasting blood glucose level was observed of administering nanoparticles at a dose of 90 mg/Kg BW. Nanoparticles at doses of 30, 60, and 90 mg/kg BW have also been shown to increase the expression of insulin receptors in skeletal muscle, GLUT4 in skeletal and cardiac muscle, and iHsp70 in liver and kidney. Based on the findings of this research, it is possible to conclude that administration red betel leaf ethanol extract nanoparticles have the potential to improve DMT2 conditions in rats by lowering fasting blood glucose levels and increasing the expression of insulin receptors, GLUT4 and iHsp70.
