PREDICTIVE FACTORS ON THE INCIDENCE OF CHRONIC GLOMERULONEPHRITIS IN PEDIATRIC PATIENTS WITH POST STREPTOCOCCAL ACUTE GLOMERULONEPRHITIS (PSAGN)

Latar belakang. Glomerulonefritis akut pasca streptokokus (GNAPS) umumnya sembuh sempurna, namun GNAPS menjadi glomerulonefritis (GN) kronik terjadi sekitar 5-10%. Mengetahui faktor-faktor prediktor terjadinya GN kronis pada pasien anak dengan GNAPS akan dapat mencegah terjadinya GN kronis dan melakukan intervensi sehingga episode akut tidak berkelanjutan Tujuan. Mengetahui faktor prediktor GN kronis pada pasien anak dengan GNAPS Metode penelitian. Penelitian merupakan penelitian observational analitik dengan desain kohort retrospektif. Populasi terjangkau adalah anak usia 2 tahun sampai 18 tahun dirawat dengan GNAPS sejak Januari 2015 sampai Desember 2020 di RSUP Dr. Sardjito Yogyakarta. Analisis multivariat dengan metode regresi logistik multivariat dilakukan untuk mengetahui hubungan antara GN kronis dengan beberapa variabel independen. Hasil. Seratus dua puluh empat anak dengan GNAPS diambil dari data rekam medis. Di antaranya 65 subjek (52,4%) dengan luaran GN kronis dan 59 subjek (47,6%) tanpa luaran GN kronis. Faktor prediktor yang diduga telah dianalisis secara bivariat dengan hasil yaitu faktor proteinuria masif yang disertai hipoalbuminemia (RR 1,670, p=0,003 (p<0.05), 95%CI 1,199-2,326), oliguria (RR 1,517, p=0,028 (p<0,005), 95%CI 1,101-2,089) dan hematuria makroskopis (RR 1,647, p=0,013(p<0,005), 95% CI 1,061-2,555) secara signifikan lebih tinggi pada kelompok GNAPS dengan luaran GN kronis dibandingkan tanpa luaran GN kronis. Analisis multivariat dengan regresi logistik didapatkan faktor prediktor proteinuria masif yang disertai hipoalbuminemia (OR=3.545, 95%CI 1.606-7.827), p=0,002) dan hematuria makroskopis (OR=3.050, 95%CI 1.339-6.950, p=0,008) merupakan faktor yang paling memprediksi luaran GN kronis di antara subjek GNAPS. Kesimpulan. Proteinuria masif yang disertai hipoalbuminemia dan hematuria makroskopis merupakan faktor prediktor yang paling memprediksi luaran GN kronis di antara subjek GNAPS.

ABSTRACT Background. Most cases of post-streptococcal acute glomerulonephritis (PSAGN) make a full recovery. However, there is a slight chance ranging from 5-10% that PSAGN would progress into chronic glomerulonephritis (GN). To prevent the incidence of chronic GN and to perform intervention for avoiding prolonged acute episodes, the predictive factors on the incidence of chronic GN in pediatric patients with PSAGN should be recognized. Objective. This study was aimed to acknowledge the predictive factors on the incidence of chronic glomerulonephritis in pediatric patients with PSAGN. Study Method. This study was an analytical observational study with a retrospective cohort design. The accessible population was children with the age 2-18 years old that admitted with PSAGN between January 2015 and December 2020 in RSUP Dr. Sardjito Yogyakarta. Multivariate analysis was performed with the multivariate logistic regression method. Result. A total of one hundred twenty-four patients with PSAGN were obtained from medical record data. From 124 PSAGN subjects (65(52,4%) with chronic GN and 59(47,6) without chronic GN). Bivariate analysis was performed on assumed predictive factors with the result of massive proteinuria with hypoalbuminemia (RR 1,670, p=0,003 (p<0.05), 95%CI 1,199-2,326), oliguria (RR 1,517, p=0,028(p<0,005), 95%CI 1,101-2,089) and macroscopic hematuria (RR 1,647, p=0,013(p<0,005), 95% CI 1,061-2,555) were significantly higher in PSAGN group with chronic GN compared to those without. Multivariate analysis with logistic regression has shown that massive proteinuria with hypoalbuminemia (OR=3.545, 95%CI 1.606-7.827, p=0,002) and macroscopic hematuria (OR=3.050, 95%CI 1.339-6.950, p=0,008) would highly predict chronic GN in PSAGN subjects. Conclusion. Massive proteinuria with hypoalbuminemia and macroscopic hematuria was the predictive factor that highly predicts chronic GN in PSAGN.
