ASSESSING THE APPROPRIATENESS OF SAUDE NA FAMILIA PROGRAM FOR TB ACTIVE CASE FINDING IN BOBONARO MUNICIPALITY, TIMOR LESTE.

Latar belakang Sudah dua dekade sejak 2004 hingga 2019, angka kejadian Tuberkulosis di Timor Leste(TL) tetap sama, dengan perkiraan 498/100.000 populasi (kisaran 322-711), dan jumlah kematian adalah 106/100.00 populasi, meskipun ada upaya untuk mengurangi TB, dalam rencana strategis nasional baru untuk pengendalian TB dijelaskan fokusnya dalam memperkuat penemuan kasus aktif TB termasuk Saude na Familia (Kesehatan Keluarga) sebagai salah satu pendekatan. Penelitian ini bertujuan untuk menilai kesesuaian program Saude na Familia untuk penemuan kasus aktif TB. Metodologi Penelitian implementasi dengan desain paralel konvergen metode campuran untuk menilai kesesuaian Saude na Familia untuk penemuan kasus aktif TB. utama variabel untuk menilai kesesuaian adalah; variabel bebas terdiri dari Pengetahuan dan sikap petugas kesehatan, persepsi petugas tentang relevansi SnF dalam melakukan ACF, HWCs Persepsi tentang kompatibilitas SnF dan TB ACF Ketersediaan sumber daya, dan variabel hasil adalah kesesuaian implementasi, 75 lima petugas kesehatan direkrut untuk studi kuantitatif dan 12 kesehatan dipilih untuk melanjutkan wawancara mendalam. Hasil Tujuh puluh dua persen peserta selalu menerapkan Saude na familia program, dan 61 persen telah menerima informasi tentang pelaksanaan TB penemuan kasus aktif melalui Saude na Familia. 62,7 dan 33,3% dari peserta setuju dan sangat setuju bahwa SnF dapat meningkatkan penemuan kasus aktif TB, dan 46,7 dan 49,3% setuju dan sangat setuju bahwa SnF layak melakukan pelacakan kontak, aktif skrining, pengumpulan dahak dan juga pendidikan kesehatan. Keuangan dan teknis hambatan, dan kendala sumber daya disajikan sebagai tantangan untuk penerapan. Kesimpulan Sebagian besar tenaga kesehatan memiliki pengetahuan umum dan sikap positif sikap mendukung pelaksanaan program, petugas kesehatan juga mempersepsikan bahwa SnF kompatibel dengan penemuan kasus aktif TB, perlu diperkuat kerja organisasi dan memperkuat dukungan untuk meningkatkan implementasi di masa depan.

Background It is has been two decades since 2004 to 2019, Tuberculosis incidence rate in Timor Leste(TL) remains the same, with an estimation of 498 /100,000 populations (range 322-711), and the number of deaths is 106/100.00 populations, despite the efforts to reduce TB, in the new national strategic plan for TB control described the focus in strengthening TB active case finding which has included Saude na Familia (Family Health) as one of the approaches. This study aims to assess the appropriateness of the Saaude na Familia program for TB active case finding. Methodology Implementation research with mixed-method convergent parallel design to assess the appropriateness of Saude na Familia for TB active case finding. The main variables for assessing appropriateness were; independent variables comprise of HCWs Knowledge and attitude, HCWs perceptions of the relevance of SnF in doing ACF, HWCs Perception regarding the compatibility of SnF and TB ACF Availability of the resources, and the outcome variable is the appropriateness of the implementation, 75 five health workers were recruited to the quantitative study and 12 health was selected to continue to the in-depth interview. Result Seventy two percent of the participants always implement Saude na familia program, and 61 percent have received information about the implementation of TB active case finding through Saude na Familia. 62.7 and 33.3 % of the participants agree and strongly agree that SnF can improve TB active case finding, and 46.7 and 49.3 % agree and strongly agree that SnF is fit in conducting contact tracing, active screening, sputum collection and also health education. Financial and technical barriers, and resources constraint presented as the challenges for the implementation. Conclusions The majority of the health workers has general knowledge and posse a positive attitude to support program implementation, the health workers also perceived that SnF is compatible with TB active case finding, it is worth strengthening work organization and reinforce support for improving future implementation.
