JUMLAH LIMFOSIT ABSOLUT DARAH TEPI PADA SAAT DIAGNOSIS SEBAGAI FAKTOR PROGNOSTIK LUARAN PENGOBATAN LEUKEMIA MIELOBLASTIK AKUT ANAK

Latar Belakang: Leukemia mieloblastik akut (LMA) merupakan kanker anak dengan prevalensi terbesar kedua di Indonesia setelah leukemia limfoblastik akut (LLA). Analisis kesintasan terhadap pasien-pasien LMA anak di Indonesia menunjukkan rendahnya event-free survival (EFS) dan overall survival (OS) serta tingginya treatment-related mortality (TRM). Hal ini diduga disebabkan oleh ketiadaan stratifikasi kelompok risiko luaran pengobatan akibat keterbatasan sumber daya manusia, infrastruktur, maupun finansial untuk melakukan analisis mutasi gena dan pemeriksaan sitogenetik. Oleh karena itu, perlu dilakukan penelitian-penelitian yang bertujuan untuk menentukan parameter-parameter sederhana yang dapat digunakan dalam stratifikasi kelompok risiko luaran pengobatan. Beberapa penelitian terhadap pasien-pasien LMA dewasa menunjukkan hubungan kenaikan jumlah limfosit T regulator (Treg) dan penurunan jumlah limfosit T sitotoksik (TC) maupun natural killer cell (sel NK) dengan limfositosis dalam sirkulasi darah tepi dan progresivitas penyakit. Tujuan: Membandingkan EFS dan OS serta menentukan hazard ratio (HR) kematian, relaps, maupun kegagalan pencapaian complete remission (CR) dalam kurun waktu satu tahun pertama antara pasien-pasien dengan jumlah limfosit absolut darah tepi pada saat diagnosis (ALC0) lebih dari 4,5 x 109 sel/L dan pasien-pasien dengan ALC0 kurang dari 4,5 x 109 sel/L. Metode: Sebuah penelitian kohort retrospektif dilakukan terhadap pasien-pasien baru yang didiagnosis LMA pada usia kurang dari 18 tahun dan menjalani pengobatan di Rumah Sakit Umum Pusat (RSUP) Dr. Sardjito, Yogyakarta, dalam kurun waktu tahun 2011-2019. Pasien-pasien dengan sindrom Down, sindrom mielodisplastik, atau fase krisis blastik leukemia mielositik kronik (LMK), pasien-pasien dengan mixed phenotypic leukemia, maupun pasien-pasien dengan rekam medis yang tidak lengkap tidak diikutsertakan dalam analisis statistik. Analisis kesintasan Kaplan-Meier dan uji log-rank dilakukan untuk membandingkan EFS dan OS, sedangkan analisis regresi Cox dilakukan untuk menentukan HR. Hasil: Penelitian ini melibatkan 60 pasien yang terdiri dari 37 pasien dengan ALC0 lebih dari 4,5 x 109 sel/L dan 23 pasien dengan ALC0 kurang dari 4,5 x 109 sel/L. Analisis Kaplan-Meier dan uji log-rank tidak menemukan perbedaan EFS (p = 0,06) maupun OS yang bermakna (p = 0,05) antara pasien-pasien dengan ALC0 lebih dari 4,5 x 109 sel/L dan pasien-pasien dengan ALC0 kurang dari 4,5 x 109 sel/L. Analisis kesintasan menggunakan nilai ambang ALC0 yang berbeda, yaitu terhadap 35 pasien dengan ALC0 lebih dari 4,7 x 109 sel/L dan 25 pasien dengan ALC0 kurang dari 4,7 x 109 sel/L, menunjukkan bahwa pasien-pasien dengan ALC0 lebih dari 4,7 x 109 sel/L memiliki EFS (p = 0,02) dan OS (p = 0,01) yang lebih tinggi daripada pasien-pasien dengan ALC0 kurang dari 4,7 x 109 sel/L. Dalam analisis regresi Cox multivariat pasien-pasien dengan ALC0 lebih dari 4,7 x 109 sel/L memiliki risiko kematian, relaps, dan kegagalan pencapaian CR yang lebih rendah dibandingkan pasien-pasien dengan ALC0 kurang dari 4,7 x 109 sel/L (HR 0,30; interval kepercayaan [IK] 95% 0,14-0,64). Simpulan: EFS maupun OS tahun pertama pada pasien-pasien LMA anak dengan ALC0 lebih dari 4,5 x 109 sel/L tidak berbeda dari pasien-pasien dengan ALC0 kurang dari 4,5 x 109 sel/L. Pasien-pasien dengan ALC0 lebih dari 4,7 x 109 sel/L memiliki EFS dan OS yang lebih tinggi serta risiko kematian, relaps, dan kegagalan pencapaian CR yang lebih rendah daripada pasien-pasien dengan ALC0 kurang dari 4,7 x 109 sel/L.

Background: Acute myeloblastic leukemia (AML) constitutes the second most prevalent cancer among Indonesian children. Survival analysis on Indonesian children with AML revealed the poor event-free survival (EFS), overall survival (OS), and treatment-related mortality (TRM). It has widely been accepted that the lack of human, infrastructure, and financial resources to routinely perform molecular and cytogenetic analyses, which leads to the absence of risk group stratification, may contribute to this situation. It is, therefore, reasonable to conduct a study, which aims to determine the applicable risk stratification-directed parameters. A number of studies on adult AML patients showed the correlation of increased regulatory T lymphocytes (Treg), decreased cytotoxic T lymphocytes (TC), and natural killer (NK) cell counts with peripheral blood lymphocytosis and disease preogressivity. Objectives: To compare the one-year EFS and OS and to determine the hazard ratio (HR) of death, relapse, and failure to achieve complete remission (CR) between patients with the diagnostic peripheral blood absolute lymphocyte counts (ALC0) of more than 4.5 x 109 cells/L and patients with the ALC0 of less than 4.5 x 109 cells/L. Methods: A retrospective cohort study was conducted on patients less than 18 years of age with the newly diagnosed AML, who had been admitted to Dr. Sardjito General Hospital, Yogyakarta, during the period of 2011-2019. Patients with Down syndrome, myelodysplastic syndrome, or the blast crisis of chronic myeloblastic leukemia (CML), patients with mixed phenotypic leukemia, or patients with an incomplete medical record were excluded from this study. Kaplan-Meier survival analysis and log-rank test were performed to compare EFS and OS between two groups of patients, while the Cox regression analysis was performed to determine HR. Results: This study involved 60 patients, who consisted of 37 patients with the ALC0 of more than 4.5 x 109 cells/L and 23 patients with the ALC0 of less than 4.5 x 109 cells/L. No significant EFS (p = 0.06) or OS difference (p = 0.05) was detected on Kaplan-Meier analysis and log-rank test. When the cut-off point of ALC0 was changed to 4.7 x 109 cells/L, there were 35 patients with the ALC0 of more than 4.7 x 109 cells/L and 25 patients with the ALC0 of less than 4.7 x 109 cells/L. Kaplan-Meier analysis and log-rank tests revealed that patients with the ALC0 of more than 4.7 x 109 cells/L had the higher EFS (p = 0.02) and OS (p = 0.01) than those of patients with the ALC0 of less than 4.7 x 109 cells/L. The Cox regression analysis demonstrated that patients with the ALC0 of more than 4.7 x 109 cells/L had a lower risk of death, relapse, and failure to achieve CR than those of patients with the ALC0 of less than 4.7 x 109 cells/L (HR, 0.30; 95% confidence interval [CI], 0.14-0.64). Conclusion: There is no significant one-year EFS or OS difference between childhood AML patients with the ALC0 of more than 4.5 x 109 cells/L and patients with the ALC0 of less than 4.5 x 109 cells/L. Patients with the ALC0 of more than 4.7 x 109 cells/L have the higher EFS and OS and a lower risk of death, relapse, and failure to achieve CR than those of patients with the ALC0 of less than 4.7 x 109 cells/L.
