Potensi Mikro Rna 122 Sebagai Penanda Kejadian Sirosis Dekompensasi Pada Pasien Dengan Latar Belakang Infeksi Virus Hepatitis B Kronik Di RSUP Dr Sardjito Yogyakarta

Latar belakang. Sirosis hati dekompensasi akan menyebabkan morbiditas dan mortalitas tinggi. Saat ini di Indonesia penyebab utama sirosis hati adalah infeksi virus Hepatitis B yang jumlahnya sekitar 7,1% dari seluruh populasi. Belum ada penanda yang mampu memprediksi kejadian sirosis dekompensasi. miR 122 merupakan mikro RNA yang dominan di hati dan juga dapat dideteksi di sirkulasi dengan kadar yang sebanding dengan tingkat kerusakan hati. Menarik untuk mengetahui potensi miR 122 sebagai penanda kejadian sirosis dekompensasi. Tujuan penelitian. Penelitian ini bertujuan untuk mengetahui potensi miR 122 sebagai penanda kejadian sirosis hati dekompensasi. Metode. Penelitian ini adalah penelitian potong lintang. Dilakukan pengambilan plasma pasien yang telah didiagnosis sirosis hati dengan infeksi Hepatitis B kronik di rawat jalan maupun rawat inap RSUP dr Sardjito Yogyakarta pada bulan juli sampai Oktober 2020. Penderita dikelompokkan menjadi sirosis dekompensasi bila didapatkan perdarahan, asites, ensefalopati, HRS atau SBP pada pemeriksaan saat ini atau riwayat terdahulu. Kuantifikasi miR 122 dilakukan dengan metode RT PCR dan disajikan dalam bentuk fold change dengan perhitungan menggunakan rumus Livax. Uji perbedaan antara dua kelompok dilakukan dengan menggunakan uji Mann Whitney. Potensi miR 122 sebagai penanda kejadian sirosis hati dekompensasi dihitung menggunakan chi square test dengan terlebih dulu mencari nilai cut off dari kurva ROC. Hasil. Didapatkan 62 subjek penelitian, 41 orang (66,1%) diantaranya adalah sirosis dekompensasi. Usia rata-rata 54,89�±9,09 tahun. Median ekspresi miR 122 sirosis kompensasi vs dekompensasi 4,46 (-2,84-31,23) vs 9,95(-1,88-81,17) fold change (p=0,0058). Ekspresi miR122 lebih tinggi pada pasien dengna HBVDNA terdeteksi (p=0,010), memiliki korelasi positif dengan SGPT (r=0,334,p=0,008) dan korelasi negatif dengan terapi antiviral (r=-0,272,p=0,034). Penentuan cut off dengan kurva ROC mendapatkan hasil miR 122= 9,23 foldchange. Analisis bivariat dengan chi square menunjukkan hasil p=0,040 dengan PR=1,45, CI(1,01-2,09). Kesimpulan. miR 122 mempunyai potensi untuk digunakan sebagai penanda biomolekuler kejadian sirosis dekompensasi pada pasien dengan latar belakang infeksi Hepatitis B kronik. Background. Decompensated liver cirrhosis will cause high morbidity and mortality. Currently in Indonesia, the main cause of liver cirrhosis is Hepatitis B virus infection that infected about 7,1% of the population. There are no markers that can predict the incidence of decompensated cirrhosis. MiR 122 is the dominant micro-RNA in the liver and can also be detected in the circulation at a level comparable to the level of liver damage. It is interesting to know the potential of miR 122 as a marker for the incidence of decompensated cirrhosis. Goal. This study aims to determine the potential of miR 122 as a marker of decompensated liver cirrhosis. Method. This is a cross sectional study. Plasma collection of patients who have been diagnosed with cirrhosis of the liver with chronic Hepatitis B infection is carried out in outpatient and inpatient care at Dr Sardjito Hospital Yogyakarta from July to October 2020. Patients were classified as decompensated cirrhosis if there was bleeding, ascites, encephalopath, HRS or SBP at the current examination or previous history. The quantification of miR 122 was carried out using the RT PCR method and presented in fold change using the Livax formula. The difference test between the two groups was carried out using the Mann Whitney Test. The potential of miR 122 as a marker of decompensated liver cirrhosis was calculated using the chi square test by first looking for the cut off value of the ROC curve. Result. There were 62 study subjects, 41 people (66.1%) were decompensated cirrhosis. The mean age was 54.89 �± 9.09 years. Median expression of miR 122 in compensated vs decompensated cirrhosis 4.46 (-2.84-31.23) vs 9.95 (-1.88-81.17) fold change (p = 0.0058). Expression of miR122 was higher in patients with detectable HBVDNA (p = 0.010), had a positive correlation with ALT (r = 0.334, p = 0.008) and a negative correlation with duration of antiviral therapy (r = -0.272, p = 0.034). Determination of the cut off with the ROC curve results in miR 122 = 9.23 foldchange. Bivariate analysis in patients above and below the cutt off with chi square showed the result p = 0.040 with PR = 1,45, CI (1.01-2,09). Conclusion. miR 122 has the potential to be used as a marker for the incidence of decompensated cirrhosis in patients with chronic hepatitis B infection.
