Aspek Psikososial Pada Anak Dengan Duschenne Muscular Dystrophy

Latar belakang. Duschenne muscular dystrophy (DMD) adalah penyakit distrofi muskular yang bersifat herediter melalui X-linked resesif akibat mutasi gen distrofin. Pendekatan multidisiplin adalah pintu utama menuju manajemen optimal pada pasien DMD. Kolaborasi multidisiplin yang dilakukan harus berfokus pada pemantauan secara berkala dan terus-menerus yang bersifat antisipatif, preventif, dan kuratif. Presentasi kasus. Anak laki-laki usia 10 tahun datang ke RS Sardjito dengan keluhan kelemahan kedua tungkai. Sebelumnya, anak masih mampu berdiri dengan merayap / menyentuh lutut hingga panggulnya sebagai tumpuan, meskipun agak sulit. Keluhan kelemahan anggota gerak ini, dirasakan terus menerus dan makin lama, makin memberat. Dari pemeriksaan fisik awal, didapatkan kelemahan anggota gerak tungkai dengan kekuatan motorik 3-4. Hasil biopsi otot menunjang diagnosis DMD. Foto thorax dan lumbal di awal pemantauan menunjukkan hasil normal. Kemudian, perkembangan penyakit pasien diikuti selama 1,5 tahun. Pasca 1,5 tahun pengamatan, dengan pemberian steroid rutin, dapat menurunkan kecepatan progresivitas penyakit. Meskipun akhirnya tetap didapatkan skoliosis lumbal, dan penurunan aktivitas. Kesimpulan. Kualitas hidup pasien dengan DMD dapat meningkat dengan manajemen komprehensif dari berbagai bidang.

Background. Duschenne muscular dystrophy (DMD) is a hereditary X-linked recessive muscular dystrophy caused by mutations in the dystrophin gene. A multidisciplinary approach is the main door to optimal management of DMD patients. The multidisciplinary collaboration that is carried out must focus on periodic and continuous monitoring that is anticipatory, preventive, and curative. Case presentation. A 10-year-old boy came to Sardjito Hospital with complaints of weakness in both legs. Previously, the child was still able to stand by crawling / touching the knee to the pelvis as a support, although it was a bit difficult. Complaints of weakness of this limb, felt continuously and over time, getting worse. From the initial physical examination, limb weakness was found with 3-4 of motor strength. Muscle biopsy results support the diagnosis of DMD. Chest and lumbar radiographs at the start of monitoring showed normal results. Then, the patient's disease progression was followed for 1.5 years. After 1.5 years of observation, with routine steroid administration, it can reduce the speed of disease progression. Although eventually still obtained lumbar scoliosis, and decreased activity. Conclusion. The quality of life of patients with DMD can be improved by comprehensive management of various fields.
