Faktor Prognosis terhadap Kesintasan pasien Atresia Jejunoileal di RSUP Dr. Sardjito Yogyakarta

Latar Belakang: Kesintasan Atresia jejunoileal sangat beragam, dimana berat badan lahir rendah (BBLR), prematur dan sepsis merupakan faktor prognostik yang dapat mempengaruhi kesintasan. Terlebih lagi pada tipe kompleks menimbulkan lebih banyak masalah pasca operasi. Tujuan Penelitian: Untuk mengetahui perbedaan kesintasan antara pasien atresia jejunoileal simpel dan kompleks serta hubungan antara kesintasan dengan faktor prognosis pada pasien atresia jejunoileal simpel dan kompleks. Metode Penelitian: Penelitian dengan data sekunder yang dilakukan dengan desain kohort retrospektif. Subjek penelitian adalah semua penderita atresia jejunoileal yang dirawat di Bagian Bedah Anak FK UGM/ RSUP Dr. Sardjito Yogyakarta pada periode Januari 2017 sampai dengan April 2021, yang sudah dilakukan tatalaksana dan faktor prognostik berupa BBLR, prematur dan sepsis preoperasi . Uji statistik menggunakan Chi-Square test dan analisis multivariat untuk menghitung hubungan multivariat serta analisa kesintasan dengan Kapplan-Meier. Hasil Penelitian: Terdapat 25 subyek penelitian, 12 laki laki dan 13 perempuan. Kami menemukan 9(36%) pasien dengan BBLR, 7(28%) pasien dengan prematur dan 15(60%) pasien dengan sepsis preoperasi. Berdasar jenis, ada 11 pasien atresia jejunoileal simpel dan 14 pasien atresia jejunoileal kompleks, berdasar letak, 13 pasien atresia jejunum dan 12 pasien atresia ileum. Kami melakukan Reseksi anastomosis pada 13 pasien dan kami harus melakukan enterostomi pada 12 pasien. Pasca operasi, kami dapatkan komplikasi paling tinggi berupa sepsis yakni 22(88%) pasien. Kesintasan atresia jejunoileal kompleks adalah 4(28,6%) pasien, sedangkan tipe simpel adalah 6(54,5%) pasien, meski tidak signifikan (p=0,241). Kemudian tidak pula didapatkan signifikan pada faktor prognosis seperti BBLR (p=0,229), dimana kesintasan 2(22,2%) pasien, prematur (p=0,179) dimana hanya memiliki 1(14,3%) pasien hidup dan 5 diantaranya adalah tipe kompleks dimana adalah keseluruhan kelompok dan untuk sepsis preoperatif, 6(40%) pasien hidup. Kesintasan rerata berdasar Kapllan-meier untuk BBLR 39 (5-80 hari), prematur 43(3-82 hari) dan untuk sepsis preoperasi 46 (3-105 hari),menariknya kami menemukan peningkatan jumlah sepsis sekitar 7 pasien, dari 15 ke 22(88%) pasien dimana 14 diantaranya meninggal karena sepsis, Kesimpulan: Kesintasan pada atresia jejunoileal sangat rendah, terutama pada kasus kompleks. Temuan kami diperlukan penelitian lebih lanjut untuk meningkatakan edukasi dan menurunkan mortalitas

Background: Jejunoileal Atresia has a variable survival rate. Low Birth Weight(LBW), Small Gestasional Age (SGA), and sepsis are some of the prognostic variables that have influenced survival rates. Furthermore, complicated types provide a greater problem. We aimed to compare and identify prognostic factors in Simple and Complex Jejunoileal Atresia. Methods: This was a retrospective study that evaluated at the medical records of neonates with Jejunoileal atresia who were operated on at our hospital between January 2017 and April 2021. Results: We found a total of 25 neonates, 12 males and 13 females. We discovered 9 (36%) patients with LBW, 7 (28%) patients with SGA, and 15 (60%) patients with preoperative sepsis. In terms of type, 11 patients have simple jejunoileal atresia and 14 have complex jejunoileal atresia; in terms of anatomical location, 13 patients have Jejunal atresia and 12 patients have Ileal atresia. We perform primary anastomosis on 13 patients and we had to enterostomy on 12 other patient. After surgery, 22 patients (88%) developed sepsis as a result of a complication. In all groups, the survival rate is 40%. The survival rate for Complex Jejunoileal atresia is four patients (28.6%), whereas the survival rate for Simple Jejunoileal atresia is six patients (54,5%), although the difference is not significant (p=0.241). There is also no significant from prognostic factors such as LBW (p=0.229), which has a survival rate of 2 patients (22,2%), SGA (p=0.179) only has one survivor (14,3%), but 5 of them are in the complex type and all die, and survival rate for sepsis before surgery (p=1.000), which has a survival rate of 6 patients (40%). The median survival for LBW is 39 (5-80 days), for small gestasional age median survival is 43 (3-82 days), and for preoperative sepsis is 46 (3-105 days), according to Kapplan-Meier. Surprisingly, sepsis increases after around 7 patients, rising from 15 to 22 patients (88%) with 14 patients dying from sepsis. Conclusions: The survival rate for Jejunoileal atresia is very low, particularly for the Complex form of Jejunoileal Atresia. Our findings will need to be further investigated before they can be utilized to provide patient counseling and minimize mortality
