COST EFFECTIVENESS ANALYSIS DALAM UPAYA PENINGKATAN MUTU PELAYANAN KLINIS PENDERITA PROSTAT JINAK

Latar belakang: Terapi medikamentosa di Rumah Sakit ST Carolus saat ini berdasarkan panduan praktik klinik (PPK) adalah terapi kombinasi yakni metode standar dan metode modifikasi. Hingga saat ini belum ada upaya dan penelitian untuk mengurangi beban biaya pelayanan klinik BPH tanpa mengurangi mutu pelayanan. Penelitian ini ingin mengetahui bagaimana mutu klinik dan kualitas hidup dari terapi metode modifikasi dibandingkan dengan terapi metode standar, serta cost effective analysis dari kedua metode terapi yang ada di RS St Carolus Jakarta. Metodologi: Desain penelitian yang dipakai adalah retrospektif kohort. Data diambil dari rekam medis dan wawancara. Hasil luaran klinis dan biaya dibandingkan dari model terapi modifikasi dan standar. Wawancara dilakukan untuk mendapat data biaya dari persfektif societal. Penelitian ini menggunakan analisis model decision tree dengan parameter pembiayaan dan luaran klinis. Data juga kemudian dianalisis dengan menggunakan program SPSS. Hasil: Terdapat 100 penderita BPH yang memenuhi kriteria inklusi. Rerata usia kelompok metode terapi standar 66,92 (SD 6,67) tahun dan metode terapi modifikasi 67,10 (SD 8,49) tahun. Pada awal terapi rerata IPSS kelompok metode standar lebih rendah dari pada kelompok modifikasi (15vs17), rerata Qmax kedua kelompok metode terapi sama 9 ml/s, rerata QoL pada kedua kelompok metode terapi standar adalah 4(tidak puas). Pada akhir terapi terdapat perbaikan mutu klinik IPSS, Qmax dan QoL pada kedua kelompok. Uji statistik dengan Mann Whitney, tidak ada perbedaan bermakna antara IPSS dan QoL kelompok metode terapi standar dan terapi modifikasi (p = 0,07 dan p = 0,498). Pada uji T tidak berpasangan didapatkan perbedaan bermakna antara Qmax kelompok metode standar dengan modifikasi (p = 0,039,95%CI -3,20529 sd -0,8769). Efektivitas metode terapi standar lebih baik dari metode terapi modifikasi. Rerata biaya metode terapi standar lebih besar dari rerata biaya metode terapi modifikasi per kunjungan. ACER terapi metode modifikasi lebih kecil dari terapi metode standar. ICER untuk terapi metode modifikasi adalah - 1.290.715. Kesimpulan: Terapi metode modifikasi lebih baik pada mutu klinik pancaran urin maksimal dibandingkan terapi modifikasi. Terapi metode modifikasi juga lebih cost effective dibandingkan terapi metode standar. Kajian ini dapat menjadi dasar standar pelayanan di rumah sakit dan lebih lanjut penilaian teknologi kesehatan secara nasional sebagai arah kebijakan paket manfaat jaminan kesehatan nasional.

Backgrounds: Medical therapy for Benign Prostate Hyperplasia (BPH) at ST Carolus Hospital currently based on clinical practice guidelines (PPK) is a combination therapy, which are the standard method and the modified method. Until now, there has been no effort and research to reduce the cost burden of BPH medication without reducing the quality of service. This study wanted to know the clinical outcome and quality of life of the modified method therapy in comparison to the standard method therapy, as well as the cost effective analysis of the two therapeutic methods available at St Carolus Hospital, Jakarta. Methods: The study design used is a retrospective cohort. Data were taken from medical records and interviews. Decision tree analysis was used for this study with the parameter of clinical outcomes and costs. Clinical outcomes and costs were compared between standard and modified therapy models. Interviews were conducted to obtain cost data from a societal perspective. The data then analyzed using SPSS statistics program. Results: There were 100 BPH patients who met the inclusion criteria. The mean age in the standard therapy and modified therapy method group was 66.92 (SD 6.67) and 67.10 (SD 8.49) years, respectively. At the start of therapy, the mean IPSS in the standard method group was lower than that in the modified group (15vs17), the mean Qmax in both groups was the same (9 ml/s), and the mean QoL score in both groups was 4 (not satisfied). At the end of therapy, there was an improvement in the quality of the IPSS, Qmax and QoL in both groups. In the Mann Whitney statistical test, there was no significant difference between IPSS and QoL in the standard therapy method group and the modified therapy method group (p = 0.07 and p = 0.498). In the unpaired T test, there was a significant difference between Qmax in the standard method group and the modified method group (p = 0.039, 95%CI, -3.20529 - -0.8769). The effectiveness of modified therapy methods better than standard therapy methods. The average cost of standard therapy methods is greater than the average cost of modified therapy methods per visit. ACER modified method therapy is smaller compared to standard method therapy. ICER modified method theraphy is - 1.290.715. Conclusions: Modified therapy method has better results regarding Qmax compared to the standard therapy method. The modified therapy method is also more cost effective than the standard therapy method. This study can be used as the basis for service standards in hospitals and national health technology assessments as a policy direction for the national health insurance benefit package.
