HUBUNGAN PENGETAHUAN DAN SIKAP IBU DENGAN STATUS IMUNISASI MEASLES RUBELLA PADA ANAK USIA 12-24 BULAN DI PUSKESMAS SIMPANG EMPAT KABUPATEN PASAMAN BARAT

Latar belakang: Campak merupakan salah satu penyakit akibat virus yang paling mudah menular pada manusia. Sementara itu infeksi virus rubella paling berpengaruh pada awal kehamilan dan dapat menyebabkan keguguran, lahir mati, dan cacat lahir yang parah pada bayi. Risiko Congenital Rubella Syndrome (CRS) paling tinggi ketika seorang wanita tertular rubella selama 12 minggu pertama kehamilan. Imunisasi MR merupakan salah satu program utama Pemerintah Indonesia terkait komitmen terhadap Global untuk berpartisipasi dalam program eliminasi campak dan rubella tahun 2020. Namun cakupan Imunisasi MR di provinsi Sumatera Barat, khususnya di Puskesmas Simpang Empat Kabupaten Pasaman Barat, masih rendah. Maka dengan dasar inilah kajian lebih lanjut hubungan pengetahuan dan sikap dengan status imunisasi MR pada bayi usia 12-24 bulan di Puskesmas Simpang Empat Kabupaten Pasaman Barat sangat diperlukan, agar efek buruk secara tidak langsung berupa meningkatnya angka morbiditas dan mortalitas pada anak dapat dicegah. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan pengetahuan dan sikap dengan status imunisasi MR pada bayi usia 12-24 bulan di Puskesmas Simpang Empat, Kabupaten Pasaman. Metode: Metode penelitian menggunakan observasional analitik dengan desain penelitian cross sectional. Pengumpulan data pada penelitian ini menggunakan teknik purposive sampling dengan total subjek sebanyak 112 orang ibu di Simpang Empat. Hasil: Berdasarkan hasil penelitian rasio bayi usia 12-24 bulan yang tidak melakukan imunisasi MR dengan yang sudah melakukan imunisasi MR adalah 1:1,5. Karakteristik responden yang telah melakukan imunisasi MR, 67% ibu memiliki pengetahuan yang baik dan 63% memiliki sikap positif terhadap imunisasi MR. Adapun karakteristik responden yang tidak melakukan Imunisasi MR pada anaknya adalah 51% ibu dengan pengetahuan baik; 67% ibu memiliki sikap positif terhadap imunisasi. Hasil multivariat, diperkirakan ibu dengan pengetahuan buruk berpeluang 2,6 kali lebih banyak tidak melakukan imunisasi MR dibanding ibu dengan pengetahuan baik, dengan mempertimbangkan variabel akses pelayanan kesehatan dan akses informasi kesehatan. Sedangkan sikap ibu baik analisis bivariat dan analisis multivariat konsisten menunjukkan tidak ada hubungan antara sikap ibu dengan imunisasi MR (p value 0.67> 0.05). Kesimpulan: Hubungan yang signifikan antara pengetahuan ibu dengan status imunisasi MR dan tidak terdapat hubungan sikap ibu dengan status imunisasi MR dengan mempertimbangkan variabel akses pelayanan kesehatan dan akses informasi kesehatan, dimana Ibu dengan pengetahuan buruk berpeluang 2,6 kali tidak melakukan imunisasi MR dibandingkan ibu dengan pengetahuan baik.

Background: Measles is one of the most easily transmitted viral diseases in humans. Meanwhile, rubella virus infection is most influential in early pregnancy and can cause miscarriage, stillbirth, and severe birth defects in babies. The risk of Congenital Rubella Syndrome (CRS) is highest when a woman contracts rubella during the first 12 weeks of pregnancy. MR immunization is one of the main programs of the Government of Indonesia regarding its commitment to Global to participate in the measles and rubella elimination program in 2020. However, the coverage of MR immunization in the province of West Sumatra, especially at the Simpang Empat Health Center, West Pasaman Regency, is still low. So based on this issue, further studies of the relationship between knowledge and attitudes with MR immunization status in infants aged 12-24 months at Simpang Empat Health Center, West Pasaman Regency are needed, so that indirect adverse effects in the form of increasing morbidity and mortality in children can be prevented. Objective: This study aims to determine the relationship between knowledge and attitudes with MR immunization status in infants aged 12-24 months at Simpang Empat Health Center, Pasaman Regency. Method: The research method used observational analytic with a cross sectional research design. Collecting data in this study using a purposive sampling technique with a total subject of 112 mothers in Simpang Empat. Results: Based on the results of the study, the ratio of infants aged 12-24 months who did not have MR immunization to those who had MR immunization was 1:1.5. Characteristics of respondents who have done MR immunization, 67% of mothers have good knowledge and 63% have a positive attitude towards MR immunization. The characteristics of respondents who did not carry out MR immunization for their children were 51% mothers with good knowledge; 67% of mothers have a positive attitude towards immunization. Multivariate results estimated that mothers with poor knowledge had 2.6 times more chance of not immunizing MR than mothers with good knowledge, taking into account the variables of access to health services and access to health information. Meanwhile the mother's attitude, both bivariate analysis and multivariate analysis consistently showed that there was no relationship between mother's attitude and MR immunization (p value 0.67> 0.05). Conclusion: There is a significant relationship between mother's knowledge and MR immunization status and there is no relationship between mother's attitude and MR immunization status by considering the variables of access to health services and access to health information, where mothers with poor knowledge have a 2.6 times chance of not immunizing MR compared to mothers with good knowledge.
