EFIKASI KOMBINASI CAPTOPRIL DAN 5-FLUOROURASIL PADA PROLIFERASI DAN DEPOSISI KOLAGEN FIBROBLAS KELOID

Latar Belakang: Keloid adalah pertumbuhan jaringan fibroproliferatif jinak yang melebihi batas tepi luka awal. Keloid terjadi karena gangguan fase penyembuhan luka yang ditandai dengan peningkatan proliferasi fibroblas dan deposisi kolagen. Stimulasi angiotensin II diketahui dapat meningkatkan proliferasi fibroblas kulit dan meningkatkan produksi kolagen. Fibroblas keloid juga memiliki laju apoptosis yang lebih rendah dibanding fibroblas normal. Hingga saat ini belum ada modalitas terapi paling efektif untuk tatalaksana keloid. Captopril, obat golongan angiotensin converting enzyme inhibitor (ACEI), secara in vitro dapat menurunkan produksi angiotensin II pada biakan fibroblas keloid sehingga berpotensi menjadi modalitas terapi keloid. 5-fluorourasil (5-FU) telah digunakan pada tatalaksana keloid. 5-fluorourasil bekerja dengan menghambat sintesis DNA sehingga menyebabkan apoptosis fibroblas keloid dan menginhibisi produksi kolagen tipe I. Tujuan: Mengetahui efikasi pemberian kombinasi captopril dan 5-FU dibanding pemberian captopril atau 5-FU sebagai agen terapi tunggal pada proliferasi dan deposisi kolagen biakan fibroblas keloid Metode: Penelitian ini merupakan penelitian in vitro dengan rancangan eksperimental. Jaringan keloid dibiakkan hingga pasase 4-7. Penelitian ini terdiri dari kelompok kontrol, captopril dalam berbagai konsentrasi (10-2, 10-3, 10-4, dan 10-5 mol/L), 5-FU 1 mg/ml, dan kombinasi captopril dalam berbagai konsentrasi dengan 5-FU 1 mg/ml. Setelah perlakukan selama 144 jam, indeks proliferasi fibroblas diukur dengan metode 3-(4,5-dimethylthiazol-2-yl)-2,5- diphenyltetrazolium bromide (MTT assay) dan indeks deposisi kolagen diukur dengan Sirius red assay. Analisis dilakukan menggunakan ANOVA dan dinyatakan signifikan secara statistik jika nilai p<0,05. Hasil: Penelitian ini menunjukkan penurunan rerata indeks proliferasi fibroblas dan deposisi kolagen pada kelompok yang mendapat captopril dalam berbagai konsentrasi (10-2, 10-3, 10-4, dan 10-5 mol/L) dan kelompok 5-FU 1 mg/ml terhadap kelompok kontrol secara signifikan (p<0,05). Pada pemberian dosis kombinasi, captopril dengan konsentrasi 10-2 mol/L dan 5-FU 1 mg/ml menunjukkan penurunan proliferasi fibroblas dan deposisi kolagen secara signifikan dibanding kelompok 5-FU 1 mg/ml maupun captopril pada dosis yang sama sebagai agen terapi tunggal (p<0,05). Kombinasi captopril dengan konsentrasi 10-3, 10-4, dan 10-5 mol/L dan 5-FU 1 mg/ml tidak menunjukkan perbedaan signifikan terhadap kelompok 5-FU 1 mg/ml. Kesimpulan: Kombinasi captopril 10-2 mol/L dan 5-FU 1 mg/ml lebih baik dalam menurunkan proliferasi fibroblas dan deposisi kolagen pada biakan fibroblas keloid dibanding pemberian captopril atau 5-FU sebagai agen terapi tunggal.

Background: Keloid is a benign fibroproliferative tissue growth that exceeds the initial wound margins. Keloid is caused by the disruption in the wound healing phase with increased fibroblast activity and excess collagen deposition. Angiotensin II stimulation is known to increase proliferation of skin fibroblasts and increase collagen production. Keloid fibroblasts also have lower apoptotic rates than normal fibroblasts. To date there is no effective therapeutic modality for keloid management. Captopril, one of the angiotensin converting enzyme inhibitor (ACEI) drug, has been tested in vitro to reduce angiotensin II production in keloid fibroblast cultures so it has potential to become a modality for keloid therapy in the future. 5- fluorouracil (5-FU) has been used in keloid management. 5-fluorouracil works by inhibiting DNA synthesis thereby causing apoptosis of keloid fibroblasts and inhibit the production of collagen type I. Objective: To determine the efficacy of the combination of captopril and 5-FU compared to captopril or 5-FU as a single therapeutic agent in fibroblast proliferation and collagen deposition in keloid fibroblast cultures. Methods: This research is an in vitro study using an experimental design. Keloid tissues were cultured up to passages 4-7. The study consisted of a control group, captopril in various concentrations (10-2, 10-3, 10-4, and 10-5 mol/L), 5-FU 1 mg/ml, a combination of captopril with various concentrations and 5-FU 1 mg / ml. After 144 hours of treatment, fibroblast proliferation was measured by the 3-(4,5- dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT assay) method and the collagen deposition was measured by Sirius red assay. ANOVA is used to analyze the data and stated statistically significant if p <0.05. Results: This study showed a decrease in the mean index of fibroblast proliferation and collagen deposition in the group receiving captopril in various concentrations (10-2, 10-3, 10-4, and 10-5 mol/L) and the 5-FU 1 mg/ml group against control group significantly (p<0.05). In the combined dose group, captopril with a concentration of 10-2 mol/L and 5-FU 1 mg/ml showed a significant reduction in fibroblast proliferation and collagen deposition compared to the 5-FU 1 mg/ml group and the captopril at the same dose (p<0,05). The combination of captopril with concentrations of 10-3, 10-4, and 10-5 mol/L and 5-FU 1 mg/ml showed no significant difference against the 5-FU 1 mg/ml group. Conclusion: The combination of captopril 10-2 mol/L and 5-FU 1 mg/ml is better at reducing fibroblast proliferation and collagen deposition in keloid fibroblast cultures than captopril or 5-FU as a single therapeutic agent.
