Optimasi Dosis Radiasi Terapi Proton pada Glioblastoma Multiforme (GBM) Menggunakan Particle and Heavy Ion Transport Code System (PHITS)

Dari seluruh tumor primer susunan saraf pusat, glioblastoma multiforme (GBM) adalah jenis tumor otak yang paling ganas dengan kecepatan pertumbuhan yang sangat tinggi. Salah satu radioterapi yang dapat digunakan untuk glioblastoma adalah terapi proton. Tujuan dari radioterapi adalah memberikan dosis maksimal pada tumor dan dosis minimal pada jaringan sehat. Penelitian ini dilakukan dengan tujuan untuk mengetahui dosis optimal terapi proton pada glioblastoma dan dosis jaringan sehat di sekitarnya. Pada penelitian ini, simulasi terapi proton pada glioblastoma disimulasikan menggunakan program Particle and Heavy Ion Transport Code System (PHITS). Simulasi terapi proton disimulasikan pada kedalaman 4,27 cm dengan volume glioblastoma 33,52 cm3. Energi yang digunakan pada simulasi terapi proton dalam penelitian ini adalah 70 MeV hingga 120 MeV. Pemberian variasi berkas dilakukan pada terapi proton dengan menggunakan berkas uniform dan berkas pencil. Setiap variasi berkas dilakukan penentuan grafik SOBP untuk melihat distribusi dosis pada daerah target. Selain itu, pembagian fraksinasi dilakukan 30 fraksi dengan 2 Gy/fraksi pada setiap variasi berkas. Hasil penelitian menunjukkan bahwa dosis optimal dari terapi proton untuk kasus glioblastoma multiforme adalah 60,1891 Gy (RBE) menggunakan berkas pencil. Berkas pencil memiliki dosis yang lebih mendekati dengan dosis total acuan pada penelitian. Dosis total yang diterima oleh organ sehat disekitar tumor adalah 1,3871 Gy (RBE) untuk otak, 1,3902 Gy (RBE) untuk jaringan lunak, 1,0261 Gy (RBE) untuk kranium dan 0,8832 Gy (RBE) untuk kulit. Hasil tersebut menunjukkan dosis yang diterima otak, jaringan lunak, kranium dan kulit lebih rendah daripada dosis toleransi maksimum sehingga aman bagi organ sehat.

Among primary central nervous system tumors, glioblastoma multiforme (GBM) is the most cancerous type of brain tumor with a remarkably high growth rate. One of radiotherapy techniques that can be used for glioblastoma is proton therapy. The goal of radiotherapy is to provide maximum dose to the tumor while minimizing the dose to healthy tissue. This research aimed to obtain the optimal dose of proton therapy in cancerous volume and minimizing the dose to healthy tissues around it. In this research, a simulation of proton therapy in glioblastoma was simulated using the Particle and Heavy Ion Transport Code System (PHITS) program. Proton therapy simulation was simulated by defining a spherical volume of 33,52 cm3 at a depth of 4,27 cm. The energy used in the simulation in this research was 70 MeV to 120 MeV. Variation of the beam was applied to proton therapy using a uniform beam and a pencil beam. For each beam variation, the SOBP curves was determined to show the dose distribution in the target area. Besides, a fractionation scheme of 30 fractions and 2 Gy/fraction was used for each beam variation. The results showed that the optimal dose of proton therapy in glioblastoma multiforme was 60,1891 Gy (RBE) using a pencil beam. The pencil beam dose was closer to the total dose reference in the research. The total dose received by the healthy organs around the tumor is 1,3871 Gy (RBE) for the brain, 1,3902 Gy (RBE) for soft tissue, 1,0261 Gy (RBE) for the cranium and 0,8832 Gy (RBE) for skin. These results indicate that the dose received by the brain, soft tissue, cranium, and skin is lower than maximum tolerance dose so that it is safe for healthy organs.
