HUBUNGAN ANTARA KADAR VITAMIN D (25 (OH) D) DENGAN FUNGSI FAGOSIT PADA PENDERITA SEPSIS

Angka kematian akibat sepsis cukup tinggi berkisar 30-70%. Respon imun tubuh terhadap sepsis ditandai dengan proinflamasi dan antiinflamasi. Beberapa penelitian terbaru menunjukan hubungan antara vitamin D dengan sepsis. Kekurangan vitamin D dikaitkan dengan kelainan fungsi makrofag dan neutrofil seperti kemotaksis, fagositosis, dan produksi sitokin pro inflamasi. Tujuan Penelitian: Untuk mengetahui apakah ada hubungan antara kadar vitamin D dengan fungsi fagosit pada penderita sepsis. Metode Penelitian: Observasional dengan desain penelitian cross-sectional. Dengan teknik pengambilan sampel menggunakan consecutive sampling. Dari 38 Subjek yang telah menandatangani inform concent dilakukan pengambilan darah untuk pemeriksaan kadar vitamin D dan tes Nitro Blue Tetrazolium (NBT). Nilai hasil tes NBT dikategorikan berdasarkan pada cut off optimal analisis kurva ROC yang didapatkan 0,215. Hubungan kadar vitamin D dengan fungsi fagosit dalam penelitian ini dianalisis menggunakan uji korelasi spearman dan analisis subgroup fisher exact dengan tingkat kepercayaa 95%. Hasil Penelitian: Dari 38 pasien sepsis yang terlibat dalam penelitian ini yang memiliki kadar vitamin D normal dengan fungsi fagosit >0,215 adalah 6 (35,5%), sedangkan yang memiliki fungsi fagosit �¢ï¿½�¤0,215 hanya 1 (4,8%). Pasien sepsis yang memiliki kadar vitamin D rendah dengan fungsi fagosit >0,215 sebanyak 11 (64,70%), sedangkan yang memiliki fungsi fagosit �¢ï¿½�¤0,215 sebanyak 20 (95,20%). Hasil analisis korelasi spearman menunjukan bahwa tidak ada hubungan antara kadar vitamin D dengan fungsi fagosit pada pasien sepsis (r=0,082; p>0,05). Sedangkan analisis subgroup dengan uji fisher exact menunjukkan bahwa pada pasien sepsis yang memiliki kadar vitamin D rendah akan berpeluang memiliki fungsi fagosit yang rendah yaitu �¢ï¿½�¤0,215, dan ini bermakna secara statistik P<0,05 dengan Relative Risk 10,91. Kesimpulan: Pada penelitian ini tidak terdapat hubungan antara kadar vitamin D dengan fungsi fagosit pada penderita sepsis.
Background: Sepsis is the leading cause of death in intensive care units. The death rate due to sepsis is quite high, ranging from 30-70%. The body's immune response to sepsis is characterized by pro-inflammatory and anti-inflammatory. Several recent studies have shown a link between vitamin D and sepsis. Vitamin D deficiency is associated with malfunctioning of macrophages and neutrophils such as chemotaxis, phagocytosis, and production of pro-inflammatory cytokines. Objective: To determine whether there is a relationship between vitamin D (25 (OH) D) levels and phagocyte function in patients with sepsis. Research method: Observational with cross-sectional research design. With the sampling technique using consecutive sampling. Of the 38 subjects who had signed the inform consent, blood was drawn for vitamin D levels and Nitro Blue Tetrazolium (NBT) tests. The value of the NBT test results is categorized based on the optimal cut-off of the ROC curve analysis obtained by 0.215. The relationship between vitamin D levels and phagocyte function in this study was analyzed using the Spearman correlation test and fisher exact test for subgroup analysis with a 95% confidence level. Results: Of the 38 septic patients who were involved in this study, only 1 (4.8%) had normal vitamin D levels with phagocyte function> 0.215. Sepsis patients who had low vitamin D levels with phagocyte function> 0.215 were 11 (64.70%), while those who had phagocyte function �¢ï¿½�¤0.215 were 20 (95.20%). The results of the Spearman correlation analysis showed that there was no relationship between vitamin D levels and phagocyte function in septic patients (r = 0.082; p> 0.05). Meanwhile, subgroup analysis with fisher exact test showed that sepsis patients who had low vitamin D levels had a chance to have low phagocyte function, namely �¢ï¿½�¤0.215, and this was statistically significant P <0.05 with a Relative Risk of 10.91. Conclusion: In this study, there was no relationship between vitamin D levels and phagocyte function in patients with sepsis.
