INFEKSI HUMAN IMMUNODEFICIENCY VIRUS, EPILEPSI, GLOBAL DEVELOPMENTAL DELAY DAN GIZI KURANG

Pasien anak dengan infeksi Human Immunodeficiency Virus (HIV) rentan memiliki komplikasi baik yang disebabkan oleh kondisi infeksi HIV itu sendiri, efek samping terapi, infeksi oportunistik atau komorbid. Komplikasi tersebut dapat menyebabkan gangguan pertumbuhan, gangguan perkembangan, gangguan intelektual dan gangguan sistem organ lain. Tujuan pengamatan dan pemantauan secara berkala adalah mendeteksi secara dini gangguan tersebut, memberi intervensi dini untuk setiap gangguan, Pasien laki laki dengan riwayat premature 32 minggu dengan berat badan lahir sangat rendah, dan mengalami keterlambatan di seluruh aspek perkembangan. Sejak usia 1 tahun pasien mengeluhkan demam, diare berulang dan batuk berulang. Ayah pasien meninggal saat terdiagnosis terinfeksi HIV saat pasien berusia 1,5 tahun, dan kemudian pasien terdiagnosis terinfeksi HIV stadium III dengan bukti virologis. Sesaat sebelum terdiagnosis terinfeksi HIV, pasien kejang disertai demam, didiagnosis sebagai ensefalitis HIV DD ensefalopati HIV DD epilepsi. Pemeriksaan PCR DNA dari LCS tidak dapat dilakukan karena adanya kendala teknis (tidak tersedianya reagen). Pemantauan dilakukan pada bulan Maret 2019 saat pasien berusia 2 tahun 7 bulan, dengan pertimbangan kejang yang masih berulang, kemungkinan interaksi obat antiretrovirus (ARV) dengan obat anti epilepsy (OAE) dan masalah tumbuh kembang pada pasien ini. Luaran utama pada pasien ini adalah, tidak ada gangguan kardiovaskular, tercapainya status gizi normal, dan perbaikan kualitas hidup pasien. Hal yang masih belum tercapai pada pasien adalah masih didapatkannya gangguan perkembangan dan gangguan kognitif. Faktor prediktor pada pasien adalah CD4, viral load, infeksi oportunistik, frekuensi kejang, kepatuhan terapi ARV dan OAE, serta gangguan hematologi dan gastrointestinal.

Child with Human Immunodeficiency Virus (HIV) infection is in risk to having complication such as related with HIV infection itself, side effect of therapy, opportunistic infection or other comorbidities. Those complications make growth and developmental disturbance, intellectual disability, and other system organ malfunctions. The objective of this routine observation was to detect the disturbance earlier, and give early interventions on each problem. Male, which had history of premature birth of 32 weeks, very low birth weight and delay of all developmental aspects. Since he was 1 years old, the patient suffered from recurrent fever, diarrhea and cough. The father already died and identified having HIV infection, when the patient 1.5 years old, then the patient also diagnosed as stage III infection of HIV with virologic evidence. A moment before establish the diagnosis, patient had seizure accompanied with fever then he suspected of encephalitis DD HIV encephalopathy DD epilepsy. The PCR DNA HIV from LCS regiment did not performed due to lack of availability. The observation started in March 2019 when the patient was 2 years and 7 months old, with the considerations of persistent of seizure, probability of reaction between antiretroviral therapy (ARV) - epileptic drug, growth and developmental problems. The main outcome in this observation were no cardiovascular problem, achievement of normal growth and quality of live improvement. Another outcome that did not achieve yet during this observation were cognitive and developmental problem still persisted. The predictor that we already had identified was, CD4 level, viral load, opportunistic infection, seizure frequency, therapy compliance hematologic and gastrointestinal problems.
