Identifikasi Ketidakpatuhan Penggunaan Obat pada Pasien Penyakit Kronis

Pasien penyakit kronis menggunakan obat-obatan dalam jangka waktu yang cukup lama dan obat yang didapatkan cukup banyak juga kompleks. Oleh karena itu, untuk mencegah timbulnya keparahan dari penyakit kronis diperlukan adanya kepatuhan terhadap pengobatan. Tujuan dari penelitian ini adalah untuk mengidentifikasi dan menelaah penyebab, bentuk ketidakpatuhan dan faktor sosiodemografi yang mempengaruhi ketidakpatuhan penggunaan obat pada pasien penyakit kronis yang nantinya dapat dijadikan bahan evaluasi. Penelitian ini merupakan penelitian observasional cross-sectional dan narrative review. Pengambilan data penelitian observasional dengan cara conveinience sampling dan wawancara menggunakan kuesioner yang telah dibuat oleh tim peneliti pengembangan self-report adherence Fakultas Farmasi UGM. Pengambilan data dilakukan di Puskesmas Kotagede I dan hasil telaah dari berbagai artikel. Telaah dilakukan melalui tiga database terkenal yaitu Pubmed, proquest dan Google Scholar. Data yang diperoleh dari wawancara kemudian dianalisis frekuensi untuk memberikan gambaran bentuk dan penyebab ketidakpatuhan penggunaan obat dan data hasil telaah dari artikel-artikel dijabarkan secara deskriptif untuk mendapatkan faktor sosiodemografi yang mempengaruhi ketidakpatuhan. Kata kunci yang digunakan untuk narrative review yaitu adherence, compliance, concordance, kepatuhan, chronic disease, chronic illness, chronic, penyakit kronis, nonadherence factors, factors that affecting nonadherence, dan faktor yang mempengaruhi ketidakpatuhan. Jumlah respoden yang didapatkan adalah 31 responden dan jumlah artikel yang telah ditelaah adalah 23 artikel. Hasil dari penelitian dapat disimpulkan dari hasil observasi dan wawancara bahwa bentuk ketidakpatuhan yang paling sering dilakukan oleh responden adalah meminum obat tidak sesuai dengan aturan pakainya, baik tidak sesuai dosis, frekuensi, maupun interval penggunaan. Tiga penyebab ketidakpatuhan yang paling sering dilakukan responden adalah lupa (83,9%), terlambat kontrol (obat habis) (61,29%) dan sibuk karena pekerjaan (54,8%). Menurut hasil telaah penyebab ketidakpatuhan paling banyak disebabkan karena efek samping obat dan interaksi obat/adverse drug reaction. Hasil dari review faktor sosiodemografi yang dapat mempengaruhi ketidakpatuhan adalah usia, jenis kelamin, jarak rumah ke fasilitas kesehatan, tingkat pendidikan dan pengetahuan, pendapatan dan etnis/ras/suku.

Patients with chronic diseases use drugs for long periods and have many and complex drugs. Therefore, to prevent the onset of the severity of chronic diseases, it is necessary to have adherence of treatment. The purpose of this study is to identify and examine the causes, forms of non-adherence and sociodemographic factors that influence non-adherence drug use in patients with chronic diseases which can later be used as evaluation material. This research is an observational cross-sectional study and narrative review. Retrieval of observational research data by means of convenience sampling and interviews using a questionnaire that was created by a research team of self-report adherence development at the Faculty of Pharmacy UGM. Data was collected at the Kotagede I Health Center and the results of the study of various articles. The study was conducted through three well-known databases namely Pubmed, Proquest and Google Scholar. Data obtained from interviews were analyzed by frequencies to provide a description of the forms and causes of non-adherence drug use and the study data from articles were described descriptively to obtain sociodemographic factors that influence non-adherence. Keywords used for narrative review are adherence, compliance, concordance, kepatuhan, chronic disease, chronic illness, chronic, penyakit kronis, nonadherence factors, factors that affecting nonadherence, dan faktor yang mempengaruhi ketidakpatuhan. The number of respondents obtained was 31 respondents and the number of articles that were reviewed was 23 articles. The results of the study can be concluded from the results of observations and interviews that the most common form of non-compliance by respondents is taking drugs not in accordance with the rules of use, either not according to the dose, frequency, or interval of use. The three most common causes of non-compliance by respondents were forgetfulness (83.9%), late control (out of medicine) (61.29%) and busy due to work (54.8%). According to the results of the study the most causes of non-compliance are due to drug side effects and drug interactions / adverse drug reaction. The results of a review of sociodemographic factors that can affect non-compliance are age, sex, distance of home to health facilities, level of education and knowledge, income and ethnicity / race / ethnicity.
