DAMPAK EKONOMI PENYAKIT PADA PASIEN DIABETES MELITUS TIPE 2: STUDI KASUS DI 3 PUSKESMAS DI KOTA YOGYAKARTA DAN KABUPATEN SLEMAN

Diabetes Melitus (DM) tipe 2 merupakan penyakit kronis yang dapat membebani pasien secara ekonomi. Penelitian ini bertujuan untuk mengetahui total biaya akibat penyakit DM tipe 2, pengaruh karakteristik pasien terhadap biaya, dan kemauan cost-sharing pasien dengan BPJS (Badan Penyelenggara Jaminan Sosial). Penelitian menggunakan rancangan cross-sectional secara observasional dengan perspektif pembiayaan adalah perspektif pasien. Data diambil dari hasil wawancara pasien DM tipe 2 di Puskesmas Mergangsan, Puskesmas Gondomanan, dan Puskesmas Mlati I pada Januari-Maret 2020 dengan pengambilan sampel menggunakan metode convenience sampling. Uji Mann-Whitney dan Kruskal-Wallis digunakan untuk mengetahui pengaruh karakteristik pasien terhadap biaya. Berdasarkan hasil penelitian, terdapat 96 pasien yang memenuhi kriteria inklusi. Rata-rata total biaya akibat penyakit DM tipe 2 dalam 3 bulan sebesar Rp 95.458 per pasien yang meliputi obat (19%), jasa pelayanan medis (11%), penunjang medis (4%), transportasi (12%), makan (11%), dan produktivitas yang hilang riil (42%). Ada perbedaan bermakna biaya tidak langsung riil dengan karakteristik jenis kelamin (p=0,39), status pekerjaan (p=0,00), jenis pekerjaan (p=0,00), dan penghasilan (p=0,00). Sebanyak 54% pasien tidak mau melakukan cost-sharing dan 46% pasien mau melakukan cost-sharing dengan tingkat kemauan cost-sharing terbanyak 10%.

Type 2 Diabetes Mellitus (DM) is a chronic disease that can be patients economic burden. This study aims to investigate the total cost of type 2 DM, the correlation between patient caracteristics toward cost, and willingness to cost-sharing by patient with BPJS (Social Insurance Administration Organization). The study was conducted cross-sectional and observational design and the financial perspective used is patients perspective. The data were obtained by interviewing type 2 DM patient in Puskesmas Mergangsan, Puskesmas Gondomanan, dan Puskesmas Mlati I on Januari-March 2020. The samples were taken using convenience sampling. Mann-Whitney and Kruskal-Wallis test were used to investigate the correlation between patients characteristic toward cost. Based on this study, there was 96 patient includes inclusion criteria. The average of total cost of type 2 DM within 3 month is Rp 95.458 per patient there are medicine (19%), medical services (11%), medical support (4%), transportation (12%), food ( (11%), and real loss productivity (42%). There are significant differences between real indirect cost and characteristic of sex (p=0,39), work status (p=0,00), type of work (p=0,00), and income (p=0,00). As much 54% patients unwilling to cost-sharing and 46% patient wiliing to cost-sharing with the highest level of willingness to cost-sharing is 10%.
