HUBUNGAN VARIASI GENETIK RS1421085 PADA GEN FAT MASS AND OBESITY-ASSOCIATED (FTO) DENGAN PROFIL LEMAK TUBUH PADA PENDERITA OBESITAS POPULASI ETNIS JAWA

Latar belakang: Prevalensi obesitas mengalami peningkatan di dunia termasuk Indonesia. Peningkatan prevalensi obesitas turut meningkatan risiko terjadinya komplikasi penyakit kardiometabolik. Selain faktor gaya hidup, variasi genetik juga berperan terhadap terjadinya obesitas. Variasi genetik pada intron pertama gen Fat Mass and Obesity-Associated (FTO) rs1421085 (substitusi T > C) merupakan salah satu faktor yang mempengaruhi kejadian obesitas, peningkatan lingkar pinggang dan lemak tubuh pada berbagai populasi. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan variasi gen FTO rs1421085 terhadap obesitas, lingkar pinggang dan profil lemak tubuh pada populasi etnis Jawa. Metode: Rancangan penelitian ini menggunakan kasus kontrol. Sampel merupakan etnis Jawa yang tinggal di Yogyakarta, Indonesia. Identifikasi variasi gen FTO rs1421085 dilakukan pada 94 orang dalam kelompok obesitas dengan Indeks Massa Tubuh (IMT) >= 27 kg/m2 dan 94 orang pada kelompok kontrol dengan IMT < 27 kg/m2. Kedua kelompok sudah dilakukan matching berdasarkan jenis kelamin. Metode Polymerase Chain Reaction - Restriction Fragment Length Polymorphism digunakan untuk genotyping. Dilakukan pengukuran profil lemak tubuh menggunakan metode Bioelectrical Impedance Analysis. Pengukuran asupan makanan dilakukukan menggunakan metode Food Record. Analisis bivariat digunakan untuk mengetahui hubungan antara variasi gen FTO rs1421085 dengan obesitas, lingkar pinggang dan profil lemak tubuh. Nilai signifikansi p < 0,05. Hasil: Karakteristik asupan makanan antara kelompok obesitas dan kontrol tidak menunjukkan perbedaan bermakna. Pada kelompok obesitas, frekuensi genotip CC lebih tinggi dibandingkan genotip TT. Genotip CC meningkatkan risiko obesitas pada seluruh subyek (OR = 2,71; 95% CI = 1,03-7,05; nilai p = 0,038) dibanding genotip TT. Genotip CC juga memiliki risiko terhadap peningkatan pesrsentase total lemak tubuh hanya pada subyek laki-laki menggunakan uji Pearson Chi-square dengan continuity correction (nilai p = 0,037). Analisis bivariat tidak menunjukkan adanya hubungan yang bermakna terhadap lingkar pinggang dan lemak viseral pada seluruh subyek maupun setelah dilakukan stratifikasi jenis kelamin. Simpulan: Terdapat hubungan antara variasi gen FTO rs1421085 dengan obesitas pada seluruh subyek dan terhadap total lemak tubuh tinggi pada subyek laki-laki, dengan genotip CC sebagai genotip risiko.

Background: The prevalence of obesity has increased globally, including Indonesia. Increased prevalence of obesity also increases the complications risk of cardiometabolic disease. In addition to lifestyle factors, genetic variation plays a role in obesity. Genetic variation rs1421085 in the first intron of Fat Mass and Obesity-Associated (FTO) gene (T> C) is one of the factors that influence the incidence of obesity, increased waist circumference and body fat in various populations. Objective: This study aimed to investigate the association of FTO gene variation rs1421085 with obesity, waist circumference and body fat profile in Javanese populations. Method: The study used a case-control design. All samples were Javanese who live in Yogyakarta, Indonesia. In this research FTO gene variation rs1421085 was genotyped in 94 obese peoples with a Body Mass Index (BMI) >= 27 kg/m2 and 94 healthy controls with a BMI <27 kg/m2. Both groups have matched by gender. The Polymerase Chain Reaction - Restriction Fragment Length Polymorphism method was used for genotyping. Body fat profile was measured by Bioelectrical Impedance Analysis scale. Food Record form was used to assess the food intake. Bivariate analysis was used to assess the association between FTO gene variation rs1421085 with obesity, waist circumference and body fat profile with p<0,05. Results: Characteristics of food intake between the obese and control groups did not show significant differences. In the obese group, the CC genotype frequency was higher than the TT genotype. The CC genotype increases the risk of obesity in all subjects (OR=2,71; 95%CI=1.03 - 7.05; p=0,038) compared to the TT genotype. CC genotype also a risk of increasing the percentage of total body fat only in male subjects using the Pearson Chi-square test with continuity correction (p=0,037). Bivariate analysis did not show a significant association with waist circumference and visceral fat in all subjects and after gender stratification. Conclusion: There is association between FTO gene variation rs1421085 and obesity in all subjects and with high total body fat in male subjects, with CC genotype as risk genotype.
