ANALISIS KELENGKAPAN PENGISIAN FORMULIR TRANSFER PASIEN TERHADAP PEMENUHAN ELEMEN PENILAIAN STANDAR ARK 3.3 AKREDITASI SNARS DI RSIA AISYIYAH KLATEN

Latar Belakang : Formulir transfer pasien digunakan untuk mencatat kondisi pasien saat dipindah, kelengkapan pengisiannya diperlukan untuk menunjang kualitas pelayanan yang diberikan kepada pasien. Berdasarkan hasil studi pendahuluan masih ditemukan ketidaklengkapan pengisian pada formulir transfer pasien. Tujuan : Penelitian ini bertujuan untuk mengetahui persentase kelengkapan formulir transfer pasien berdasarkan standar ARK 3.3 akreditasi SNARS, faktor penyebab dan upaya yang dilakukan untuk mengatasi ketidaklengkapan formulir transfer pasien. Metode : Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kualitatif dan rancangan penelitian studi kasus. Teknik pengumpulan data menggunakan teknik wawancara, observasi dan studi dokumentasi. Keabsahan data yang digunakan triangulasi sumber dan teknik. Hasil : Berdasarkan hasil analisis pada 163 formulir transfer pasien berdasarkan standar ARK 3.3 akreditasi SNARS di RSIA Aisyiyah Klaten kelengkapan pengisian pada EP 2 sebesar 83,4% (terpenuhi lengkap), EP 3 sebesar 42,9% (terpenuhi sebagian), EP 4 sebesar 97,6% (terpenuhi lengkap), EP 5 sebesar 98,8% (terpenuhi lengkap), EP 6 sebesar 98,8% (terpenuhi lengkap), EP 7 sebesar 24,5% (terpenuhi sebagian). Faktor penyebab meliputi banyaknya pasien, kurangnya petugas jaga, petugas lupa untuk melengkapi dan tidak ada SPO atau petunjuk teknis pengisian formulir transfer pasien. Upaya yang telah dilakukan adalah saling mengingatkan, petugas rekam medis mengembalikan ketidaklengkapan kepada petugas terkait dan sosialisasi penggunaan formulir dengan format baru. Kesimpulan : Pengisian formulir transfer pasien terhadap pemenuhan elemen penilaian standar ARK 3.3 akreditasi SNARS belum terpenuhi lengkap. Penyebab dan upaya untuk mengatasi ketidaklengkapan pengisian pada formulir transfer pasien dapat dilihat dari unsur man dan method. Background : The internal patient transfer form is used to record the patients condition when transferred, the completeness of filling is needed to support the quality of servive provided to the patient. Based on the results of the preliminary study, there was still an incomplete filling in the patient transfer form. Objective : This study aims to determine the percentage of completeness of patient transfer forms based on the SNARS accreditation standard 3.3 ARK, the causal factors and efforts made to overcome incompleteness in the patient transfer form. Method : The type of this research was descriptive research with qualitative approach and research design was case study. The data was taken by interview, observation and documentation studies. The validity of the data used triangulation of sources and techniques. Results : Based on the results of the analysis on 163 internal patient transfer forms to fulfill the ARK 3.3 SNARS accreditation standard at RSIA Aisyiyah Klaten completeness filling in EP 2 amounting to 83.4% (complete fulfillment), EP 3 at 42.9% (partially fulfilled), EP 4 amounting to 97.6% (complete fulfillment), EP 5 amounting to 98.8% (complete fulfillment), EP 6 amounting to 98.8% (complete fulfillment), EP 7 amounting to 24.5% (partially fulfilled). The causal factors include the number of patients, the lack of a guard officer, the officer forgets to complete and there is no SPO or technical instructions for filling out the internal patient transfer form. Efforts have been made to overcome this by reminding each other, medical record officers return incompleteness to relevant officials and socialize the use of forms with the new format. Conclusion :Completion of the internal patient transfer form to fulfill the SNARS accreditation standard 3.3 evaluation element elements has not been fully fulfilled. The causes and efforts to overcome the incompleteness of filling in the internal patient transfer form can be seen from the man and method elements.
