Background: The incidence of low birth weight (LBW) is one of the main causes of infant mortality. Health literacy of pregnant women will affect knowledge and attention to smoking behavior, as well as its consequences to pregnancy and birth. This study aims to determine the relationship between health literacy of pregnancy and SHS effects with LBW. Method: This study was a nested case control design. The data analyzed was secondary data of pre-post Peer Health SHS-LBW study intervention. The study population was pregnant women four months (baseline data) till childbirth (birth data). The cases group consists of 73 postnatal women with LBW, and the control group consists of 219 postnatal women with non-LBW. Dependent variable was LBW incidence, independent variable were pregnancy health literacy measured by exposure to pregnancy care information and pregnancy care, as well as the SHS effects health literacy that measured by exposure to SHS effects information, knowledge of SHS effects and SHS exposure. Bivariate analysis using chi-square test and multivarite analysis using logistic regression test, with a significance level of p <0.05 and 95% confidence level Results: Pregnancy care and exposure to SHS effects information had a significant relationship with the incidence of LBW (p <0.05). Exposure to SHS effects information and SHS exposure had a dominant relationship with the incidence of LBW. The possibility of why these variables became the dominant factor was because of the increasing of health literacy level that obatained from exposure to health information and health knowledge during the intervention. Conclusion: There was a relationship between pregnancy care and exposure to SHS effects information, but there was no significant relation between exposure of pregnancy care information, knowledge of the SHS effects and SHS exposure to the incidence of LBW. Efforts to improve health literacy through health promotion for pregnant women, can improve pregnancy health and prevent LBW.
