ANALISIS KELENGKAPAN REKAM MEDIS RAWAT INAP DI PUSKESMAS TEMON I SEBAGAI PENUNJANG RE-AKREDITASI

Latar Belakang : Berkas rekam medis harus berisikan informasi kesehatan yang cukup lengkap dan rinci, akurat dan relevan, sehingga dapat digunakan untuk berbagai kepentingan. Di Puskesmas Temon I, dari 5 berkas, terdapat 4 berkas yang tidak lengkap dan 1 berkas yang lengkap. Ketidaklengkapan mayoritas terdapat pada identitas pasien dan informed consent. Tujuan : Penelitian ini bertujuan untuk mengetahui kelengkapan item, mengetahui persentase, mengetahui faktor penyebab ketidaklengkapan, dan mengetahui upaya yang dilakukan untuk mengatasi ketidaklengkapan berkas rekam medis. Metode : Jenis penelitian adalah deskriptif kualitatif. Rancangan penelitian menggunakan studi kasus. Subjek yang dipakai adalah dokter, perawat, bidan, petugas administrasi, dan koordinator rekam medis. Objek penelitian adalah rekam medis rawat inap periode kunjungan Bulan Januari 2019. Keabsahan data yang digunakan triangulasi sumber dan teknik. Hasil : Hasil analisis kelengkapan item di rekam medis rawat inap sesuai dengan Standar Akreditasi Puskesmas. Hasil analisis persentase kelengkapan item rekam medis rawat inap berdasarkan kriteria 7.2.2 EP 1, 7.4.4 EP 4 dan 8.4.4 EP 1 adalah 86,3%, 49%, dan 91,3%. Faktor penyebab meliputi, Man dan Method. Sudah terdapat upaya untuk mengatasi ketidaklengkapan. Kesimpulan : Kelengkapan item pada berkas rekam medis rawat inap sudah sesuai. Persentase kelengkapannya yaitu 86,3%, 49%, dan 91,3%. Faktor penyebab ketidaklengkapan meliputi Man dan Method. Sudah terdapat upaya untuk mengurangi ketidaklengkapan. Disarankan bagi puskesmas untuk menentukan pembagian tugas perekam medis, melakukan sosialisasin tentang SOP dan pentingnya kelengkapan rekam medis. Bagi petugas sebaiknya memperinci analisis kelengkapan dan meningkatkan koordinasi.

Background: The medical record file must contain health information that is quite complete and detailed, accurate and relevant, so that it can be used for various purposes. InTemon I Primary Health Center, out of 5 files, there are 4 incomplete files and 1 complete file. The majority of incompleteness are in patient identity and informed consent. Objective: This study aims to find out the completeness of the items, find out the percentage, find out the causes of incompleteness, and find out the efforts to overcome the incomplete medical record file. Method: This research used qualitative descriptive. The design of the study used case studies. The subjects used were doctor, nurse, midwive, administrative officer, and medical record coordinator. The object of the study was the inpatient medical record of the visit period in January 2019 with 104 files . The validity of the data use were source triangulation and techniques. Results: The results of the complete item analysis in the inpatient medical record were in accordance with the Primary Health Center Accreditation Standards. The results of the analysis of the percentage of completeness of inpatient medical record items based on criteria 7.2.2 EP 1, 7.4.4 EP 4 and 8.4.4 EP 1 were 86.3%, 49%, and 91.3%. Causes include, Man and Method. There have been efforts to overcome incompleteness. Conclusion: Completeness of items in the inpatient medical record file is appropriate. The percentage of completeness is 86.3%, 49%, and 91.3%. Factors that caused incompleteness include Man and Method. There have been efforts to reduce incompleteness. It is recommended for the Primary Health Center to determine the division of medical record assignments, conduct socialization on Operational Procedural Standard and the importance of completing medical records. For officers, they should specify the completeness analysis and improve coordination.
