PENENTUAN LETAK IMPAKSI GIGI KANINUS MAKSILA PADA RADIOGRAF PANORAMIK DENGAN MENGGUNAKAN METODE GARIS SEKTOR

Gigi kaninus maksila merupakan gigi yang sering mengalami impaksi setelah gigi molar ketiga. Perawatan gigi impaksi kaninus maksila sangat berhubungan dengan letak mahkotanya. Penentuan letak mahkota impaksi kaninus maksila dapat ditentukan dengan menggunakan radiograf panoramik. Radiograf ini memiliki kelebihan sehingga terdapat beberapa metode untuk menentukan letak mahkota impaksi gigi kaninus maksila. Salah satu metode yang menggunakan radiograf panoramik adalah metode analisa garis sektor. Penelitian sebelumnya menunjukkan bahwa metode ini memiliki hasil yang baik dalam menentukan letak bukal palatal mahkota impaksi gigi kaninus maksila. Penelitian ini bertujuan untuk mengetahui ketepatan letak impaksi kaninus maksila dengan metode garis sektor menggunakan radiograf panoramik. Terdapat 36 kasus yang dapat dijadikan sebagai subjek penelitian karena memiliki data rekam medis dan radiograf panoramik yang lengkap, dimana 25 kasus telah dieksklusi. Penelitian dilakukan dengan membandingakan letak mahkota gigi impaksi kaninus antara hasil analisa garis sektor pada radiograf panoramik dengan hasil operasi yang tercatat pada rekam medis (retrospektif). Awal penelitian dilakukan uji Cohens Kappa pada 9 sampel untuk melihat kesesuaian intra pengamat dan antar pengamat. Hasil uji Cohens Kappa adalah 1,000. Setelah dilakukan uji Cohens Kappa, dilakukan uji diagnostik untuk menentukan akurasi, sensitivitas dan spesifisitas metode garis sektor. Hasil penelitian akurasi 91,7%, sensitivitas 100% dan spesifisitas 80%. Kesimpulan dari penelitian ini adalah metode garis sektor merupakan metode yang baik dalam menentukan letak mahkota gigi impaksi kaninus maksila, metode ini memiliki kelebihan praktis dan parameter anatomisnya jelas dalam menetukan garis sektornya.

The maxillary canines are the second most frequently impacted after the third molar tooth. The treatment of the impacted maxillary canines is closely associated with the location of the crown, where a panoramic radiograph is used to determine its position. With the panoramic radiograph, several methods can be used to determine the location of the impacted maxillary canine crown, one of which is the sector line analysis method. As shown by the previous research, this method resulted in high accuracy in in determining the buccal palatal position of the impacted maxillary canine crowns. This study aimed to observe the exact position of the impacted maxillary canine tooth with the panoramic radiograph-assisted sector line method at RSGM UGM Prof. Soedomo. The research subjects consisted of 36 cases with their complete data of medical records and panoramic radiographs, 25 cases of which have been excluded. This study compared the position of the impacted maxillary canine crowns between the result of the sector line analysis on the panoramic radiograph to that of the operation recorded on the medical records (retrospectively). Cohen’s Kappa test on 9 samples was initially performed to see the consistency between the intra- and interraters, which resulted in 1.000. Subsequently, the diagnostic test was done to determine the accuracy, sensitivity and specificity of the sector line method. This study resulted in 91.7%, 100% and 80% for the value of accuracy, sensitivity and specificity respectively, which concluded that the sector line method was acceptable for determining the position of the impacted maxillary canine crowns. Furthermore, this method has practical advantages with clear anatomical parameters in determining the sector line.
