Permintaan Masyarakat terhadap Asuransi Kesehatan Paket Katastrofik dan Faktor-faktor yang Mempengaruhinya

INTISARI Latar belakang. Hasil penelitian diberbagai negara menujukkan bahwa pengeluaran biaya kesehatan secara out-of pocket yang bersifat katastrofik akan menimbulkan kemiskinan. Pemerintah Indonesia, sejak Januari 2014 telah menerapkan Jaminan Kesehatan Nasional. Target pencapaian Universal Health Coverage dalam kepesertaan diharapkan bisa mencapai 100% pada akhir tahun 2019. Tetapi, kondisi sosiodemografik penduduk antar daerah di Indonesia sangat beragam. Tujuan penelitian. Untuk mengetahui faktor-faktor yang mempengaruhi permintaan masyarakat terhadap asuransi kesehatan yang menjamin penyakit katastrofik, termasuk pengaruh berbagai kondisi daerah di Indonesia. Metode penelitian. Data sekunder dari proyek penelitian Depkes 2008. Sampel diambil secara kluster purposif kuota sampling sejumlah 1.289 orang dari tiga provinsi yang berbeda katagori fiskal (DKI, Sulawesi Selatan dan NTT). Kuesioner terstruktur yang telah divalidasi digunakan sebagai instrumen penelitian. Variabel dependen atau wtp paket asuransi diperoleh melalui metode contingent valuation dengan 2 skenario, Model regresi logistik multivariat multilevel digunakan untuk menganalisis studi ini.Variabel independen terdiri atas 15 variabel untuk level individu-keluarga dan 4 variabel untuk level daerah. Hasil dan kesimpulan. WTP premi paket asuransi standar berkorelasi positif dan secara statistik signifikan dengan pendidikan tinggi, jarak ke pelayanan kesehatan rawat jalan dan tingkat kesejahteraan keluarga, tetapi berkorelasi negatif dengan kondisi kesehatan anggota keluarga yang tidak sehat, adanya balita, dan hambatan. Sedangkan wtppremi paket asuransi katastrofik berkorelasi postif dan secara statistik signifikan dengan pekerjaan dan jarak ke pelayanan kesehatan rawat jalan. Analisis multilevel WTP paket asuransi katastrofik level individu-keluarga terbukti dipengaruhi oleh perbedaan urban-rural. Masyarakat urban memiliki kesediaan membayar premi asuransi katastrofik 20x lebih tinggi dari rural. Kebijakan pembiayaaan kesehatan untuk pencapaian UHC sebaiknya difokuskan ke kelompok masyarakat yang memiliki WTP rendah yaitu masyarakat dengan pekerjaan berpenghasilan tidak tetap yang tinggal di rural.

ABSTRACT Background. Various studies around the world showed that catastrophic out-of-pocket health expenditure will lead to poverty. The Indonesian government, since January 2014 has implemented the National Health Insurance. The target of achieving Universal Health Coverage in the membership is expected to reach 100% by the end of 2019. However, socio-demographic condition of population between regions in Indonesia is very diverse. Objectives. Determine the factors that affecting the community demand for health insurance that guarantees catastrophic illness, including the influence of various local conditions in Indonesia. Methods. Secondary data from the Department of Health research project, 2008. Samples were taken by purposive quota sampling clusters as much as 1,289 people from three different provinces in fiscal categories (Jakarta, South Sulawesi and NTT). Structured questionnaire that has been validated is used as a research instrument. The dependent variable or WTP insurance package obtained through the Contingent Valuation method with two scenarios. Multilevel multivariate logistic regression model was used to analyze the study. The independent variables consisted of fifteen variables for individu-family level and four variables for the regional level. Results and conclusions. WTP premium standard insurance package is positively correlated and statistically significant with highh education background, distance to outpatient health care and family welfare levels, but negatively correlated with the health condition of a family member who is not healthy, the presence of a toddler, and barriers. Meanwhile, WTP premium catastrophic insurance package positively correlated and statistically significant with distance to jobs and outpatient health services. Multilevel analysis of WTP catastrophic insurance package of individual-family level shown to be affected by the urban-rural differences. Urban communities have a willingness to pay for catastrophic insurance premiums 20 times higher than rural. Health financing policy for the achievement of UHC should be focused to a community group that has a lower WTP is people with informal sector who live in rural areas.
