Pengaruh anemia ibu hamil trimester III terhadap kejadian rendahnya skor APGAR bayi baru lahir

Tujuan: Penelitian ini bertujuan untuk mengetahui pengaruh anemia ibu hamil trimester III terhadap kejadian rendahnya skor Apgar 1 dan 5 menit Cara: Jenis penelitian observasional dengan menggunakan rancangan kohort retrospektif. Data diambil dari rekam medis pasien periode Januari 1997 – Desember 2001 di RS DR. Sardjito, Yogyakarta. Populasi ibu hamil trimester III sebanyak 8029, didapatkan 295 kasus dengan kadar hemoglobin kurang 11 g%. Setelah diseleksi menurut kriteria inklusi dan eksklusi diperoleh 149 kasus sebagai subyek kelompok terpapar. Dengan menggunakan metode simple random sampling diperoleh 149 kasus sebagai subyek kelompok tak terpapar (kadar hemoglobin lebih besar sama dengan 11 g%). Analisis yang digunakan adalah uji t, uji X2, Fischer Exact test, stratifikasi dengan uji Mantel Haenszel dan regresi logistik ganda. Hasil Penelitian: Pada analisis X2 diperoleh bahwa anemia pada ibu hamil trimester III meningkatkan kejadian rendahnya skor Apgar 1 menit dengan RR=1,805 (95% CI: 1,29-2,53). Setelah dianalisis regresi logistik ganda terhadap variabel-variabel bebas: anemia, cara kelahiran, presentasi janin, preeklamsi, paritas, kala I lama, kala II lama dan umur kehamilan diperole h hasil bahwa anemia pada ibu hamil trimester III secara statistik tidak meningkatkan kejadian rendahnya skor Apgar 1 menit (p=0,56)). Terhadap kejadian skor Apgar 5 menit, dengan analisis X2 diperoleh RR=1,11 (95% CI: 0,46-2,65) dan setelah dianalisis dengan regresi logistik ganda terhadap variabel bebas: anemia, cara kelahiran, presentasi janin, umur ibu, kala I lama dan preeklamsi, diperoleh hasil bahwa anemia pada ibu hamil trimester III tidak meningkatkan kejadian rendahnya skor Apgar 5 menit (p=0,24). Kesimpulan: Pada penelitian ini tidak ditemukan adanya pengaruh anemia pada ibu hamil terhadap kejadian rendahnya skor Apgar 1 dan 5 menit.

Objection: This research aim to know influence of anemia on the third trimester pregnancy woman on the incidence of low Apgar scores at the first minute and fifth minutes. Methods: This was an observational research with cohort retrospective design. Data werw obtained from medical record of patients period of Januari 1997 – December 2001 in DR. Sardjito Hospital, Yogyakarta. Pregnant population on the third trimester counted 8029, got 295 cases with hemoglobin rate less 11 g%. After selected according to criterion of inclusion and exclusion obtained 149 cases as group exposure. By using of method of simple random sampling obtained 149 cases as group unexposure (haemoglobin rate is equal more than 11 g%). The analysis used is t test, X2 test, Fischer Exact test, stratify with Mantel Haenszel test and binary logistic regression. Results: At analysis of X2 obtained that anemia on the third trimester pregnancy woman was increased on the incidence of low Apgar scores at the first minute with RR=1.805 (95% CI: 1,29 –2,53). After analysed by binary logistic regression to dependent variables: anemia, parity, preeclampsi, mode of birth, prolonged first stage, prolonged second stage, gestational age and presentation of foetus obtained by result of that anemia on the third trimester pregnancy woman statistically do not increased on the incidence of low Apgar scores at the first minute (p=0,56). While to occurrence of Apgar score five minutes, with analysis of X2 obtained by RR=1,11 (95% CI: 0,46-2,65) and obtained by binary logistic regression to dependent variables: anemia, mode of birth, fetus presentation, maternal age, prolonged first stage and preeclampsi that anemia on the third trimester pregnancy woman was not increase on the incidence of low Apgar scores at the fifth minutes (p=0,24). Conclusion: at this research do not found the influence of anemia on the third trimester pregnancy woman to incidence of low Apgar scores at the first minute and fifth minutes.
