Pengaruh Pursed-Lips Breathing Terhadap Skor Ansietas Pada Penderita Penyakit Paru Obstruktif Kronik Stabil Yang Cemas

Latar belakang. Ansietas merupakan salah satu komorbid pada PPOK dengan prevalensi yang cukup tinggi. Ansietas pada PPOK menurunkan kualitas hidup dan meningkatkan morbiditas dan mortalitas. Pursed lips breathing (PLB) merupakan teknik pernafasan yang sangat mudah dan dapat memperbaiki kualitas hidup, pola pernafasan, memperbaiki volume tidal dan menurunkan sesak nafas. Penelitian mengenai rehabilitasi pulmoner terhadap perbaikan skor ansietas ataupun kualitas hidup telah dilakukan di beberapa negara, namun khusus mengenai PLB terhadap skor ansietas belum pernah diteliti. Tujuan Penelitian.Tujuan utama penelitian ini adalah untuk menganalisis pengaruh rehabilitasi pulmoner dengan menggunakan teknik pursed-lips breathing (PLB) terhadap perbaikan skor ansietas pada pasien PPOK stabil yang cemas. Metode.Penelitian ini menggunakan desain randomized controlled trial, open label pada pasien PPOK rawat jalan yang dinilai skor ansietas saat awal dengan kuesioner Beck Anxiety Inventory (BAI) kemudian dilakukan randomisasi. Subyek dibagi menjadi 2 kelompok, yaitu kelompok perlakuan yang mendapat latihan teknik PLB dan kelompok kontrol. Terapi standar tetap dilanjutkan. Setelah 4 minggu dilakukan evaluasi ulang skor BAI. Hasil Penelitian. Sebanyak 44subyek penelitian yang dianalisis pada akhir penelitian, terdiri dari 24 orang kelompok PLB dan 20 orang kelompok kontrol. Rerata umur kelompok PLB 63+-7 tahun dan kelompok kontrol 64+-9 tahun. Terjadi penurunan skor BAI yang lebih besar pada kelompok PLB sebesar 5,08+-6,26 (p=0.001) dibandingkan kelompok kontrol sebesar 3,7+-4,34 (p=0.001). Namun tidak didapatkan perbedaan rerata delta BAI pada kedua kelompok (p = 0,409) Kesimpulan.Meskipun tidak terdapat penurunan skor ansietas yang bermakna secara statistik pada kedua kelompok, namun penurunan skor ansietas lebih banyak terjadi pada kelompok PLB. Dapat dipertimbangkan latihan pursed-lips breathing sebagai terapi tambahan pada pasien PPOK stabil dan cemas yang sudah mendapat terapi medikamentosa standar.

Background. Anxiety is highly prevalent comorbid complications in chronic obstructive pulmonary disease (COPD). Anxiety in COPD patients impact quality of life. Pursed lips breathing (PLB) is a simple breathing technique and improves quality of life, breathing pattern, tidal volume and dicrease dyspneu. Its effect on anxiety by improving ability of the inspiratory muscle, dicrease patients tiredness, reduce hyperinflation, recruitment of the accesory muscles and it would lead to breath more efficiently and consume less oxygen Objective. This study aimed to evaluate the effect of pursed lips breathing on beck anxiety inventory scale with stable COPD patients Methods. The research method used is randomized controlled trials study and conducted in patients with COPD in Respira Hospital, Bantul, Yogyakarta. Anxiety is measured using Beck Anxiety Inventory (BAI). The intervention group was treated with usual care plus PLB excercises in the form of 8 minutes for 28 days. The control group was treated with only the usual care. The assesment of anxiety was repeated in both groups after 28 days Results : Out of total 44 subjects, 24 subjects were in the PLB group and 20 subjects were in the control group. The average age of subjects in the PLB group was63+-7 years old and in the control group was 64 +-9 years old. There was a bigger decrease of BDI score in the PLB group 5,08+-6,26 (p=0.001) compare to the control group 3,70+-4,34 (p=0.002). But, there was no difference of the mean of delta between these two groups (p = 0,409). Conclusion :Despite no significant difference in the symptoms of depression between two groups statistically, the improvement of the symptoms of depression were much more to be found in the PLB group. As a result, pursed-lips breathing exercise may be considered as an adjunctive therapy for stable COPD patient that already receiving standard medical treatment
