IDENTIFIKASI MASALAH DAN SOLUSI TERKAIT PEMBERIAN KODE DIAGNOSIS PADA REKAM MEDIS DI PUSKESMAS SEWON I BANTUL

Latar Belakang: Penyelenggaraan rekam medis khususnya pada pemberian kode diagnosis di Puskesmas Sewon I Bantul masih menjadi masalah. Berdasarkan hasil studi pendahuluan dari 30 berkas rekam medis yang telah peneliti amati, ketidakakuratan kode diagnosis berdasarkan pada rekam medis dengan ICD-10 di Puskesmas Sewon I Bantul mencapai 71,66%. Selain itu terdapat beberapa masalah lain yang peneliti dapati di Puskesmas Sewon I Bantul. Dimana sering terjadinya error pada komputer, terlebih aplikasi I-HIS serta P-care pada masa masa sibuk Puskesmas. Tujuan: Mengetahui dan mengidentifikasi masalah serta solusi yang dapat digunakan pada masalah tidak tepatnya kode diagnosis pada berkas rekam medis pasien di Puskesmas Sewon I Bantul. Metode: Jenis penelitian ini menggunakan metode penelitian deskriptif secara kualitatif dengan pendekatan fenomenologis. Hasil: Penelitian menunjukkan terdapat beberapa permasalahan pada kegiatan coding rekam medis. Dimana petugas tidak berlatar belakang rekam medis, tidak adanya Prosedur tetap, seringnya terjadi masalah pada komputer serta aplikasi didalamnya dan tidak adanya ICD-10 disetiap poliklinik yang menyebabkan terhambatnya proses coding. Setelah dilakukan analisis lebih lanjut, didapatkan daftar solusinya. Dengan solusi yaitu mengadakan pelatihan untuk petugas coding, membuat prosedur tetap terkait kegiatan di bagian coding rekam medis, mengupgrade komputer serta menambah ICD-10. Kesimpulan: Pada bagian coding diagnosis di Puskesmas Sewon I Bantul terdapat masalah yaitu terjadinya ketidaktepatan dalam proses pemberian kode diagnosis dengan solusi terbaik mengadakan pelatihan untuk petugas coding.

Background: The organization of medical record, especially on the diagnosis code at Puskesmas Sewon I Bantul still a problem. Based on preliminary study of 30 medical record files that researchers have observed, the inaccuracy of diagnostic codes based on ICD-10 at Puskesmas Sewon I Bantul reached 71.66%. In addition there are several other problems that researchers found in Puskesmas Sewon I Bantul. Where is the frequent occurrence of errors on the computer, especially IHIS applications and P-care during busy period of Puskesmas. Objectives: knowing the problem and solutions that can be used on of not accurate diagnosis code in patient medical record file at Puskesmas Sewon I Bantul. Methods: This research uses descriptive research method qualitatively with fenomenologis approach. Result: Research shows there are some problems in the activity of coding record medical. Where the officer does not have a medical record background, the ansence of fixed procedures, frequent computer problem and the absence of ICD in every poly. Conclusion: In the coding of diagnosis at Puskesmas Sewon I Bantul there is a problem that is the occurrence of diagnosing patient code at Puskesmas Sewon I Bantul with the best solution hold training for coding officers.
