Hubungan Atopi dengan Dermatofitosis

Pendahuluan. Respon imun seluler terhadap infeksi dermatofita sebagian besar dipengaruhi oleh Th1. Atopi merupakan sindrom klinis yang ditandai dengan disfungsi barrier kulit, penurunan respon Th1 dan peningkatan respon Th2. Hubungan antara atopi dengan infeksi dermatofitosis masih kontroversial. Tujuan. Penelitian ini bertujuan untuk mengetahui apakah atopi merupakan faktor risiko terjadinya dermatofitosis pada populasi di Yogyakarta. Metode. Penelitian dilakukan dengan menggunakan desain kasus kontrol dengan matching pada populasi di Yogyakarta dan sekitarnya antara bulan Oktober hingga Desember 2016. Kasus adalah penderita dermatofitosis yang didiagnosis secara klinis dan laboratoris, sedangkan kontrol adalah individu yang tidak menderita dermatofitosis. Dilakukan matching pada usia dan jenis kelamin antara kasus dan kontrol. Kriteria atopi ditetapkan berdasarkan anamnesis adanya riwayat atopi diri dan keluarganya dengan/atau peningkatan kadar IgE total. Pemeriksaan IgE dilakukan dengan metode Enzym Linked Fluorescent Assay (ELFA). Analisis data dilakukan dengan uji statistik McNemar. Hasil. Didapatkan 61 subyek pada kelompok kasus dan 61 subyek pada kelompok kontrol, terdiri atas masing-masing dengan jenis kelamin perempuan 38 (62,3%) dan laki-laki 23 (37,7%). Rerata usia 31,84+18,14. Rerata kadar IgE kelompok dermatofitosis lebih tinggi dibandingkan kelompok kontrol (553,7+440,1 vs. 298,2+266,6, P<0,05). Digabung dengan riwayat atopi, pada kelompok kasus didapatkan atopi sebesar 13 orang (21,3%) dan non atopi 48 orang (78,7%) sedangkan kelompok kontrol dengan atopi 6 orang (9,8%) dan non atopi 55 orang (90,2%). Meskipun frekuensi atopi lebih tinggi pada kelompok dermatofitosis dibandingkan kontrol, tetapi secara statistik perbedaan tersebut tidak bermakna (RO=2,4; IK95%=0,787 - 8,695; P>0,05). Kesimpulan. Titer IgE serum lebih tinggi pada kelompok dermatofitosis dibanding kelompok kontrol, namun secara keseluruhan atopi bukan merupakan faktor risiko timbulnya dermatofitosis.

Background. The immune cellular mechanism of Th1 is predominant to defence against dermatophyte infections. Atopy is a syndrome characterized by skin barrier disfunction, decrease of Th1 response and increase of Th2 response. The association between atopy and dermatophytosis infection is still controversial. Objective. This study aims to determine whether atopy is as a risk factor of dermatophytosis in Jogjakarta. Patients and methods. A matched case control was designed in this study and conducted in Yogyakarta from October until December 2016. The case was subject with dermatophytosis, and the control was healthy subject without dermatophytosis. Matching was done in age and sex between case and control. Atopy was define based on history (self and family) and/or increased of serum IgE level. IgE examination by Enzym Linked Fluorescent Assay (ELFA) method in Clinical Laboratory of Sardjito Hospital. Atopy was person who had self or family history of atopy and elevated IgE levels. Data analysis used McNemar statistical tests. Result. There were 61 persons recruited in either case and control groups, with 38 (62.3%) females and 23 (37.7%) males. The mean age were 31,84+18,14. The mean IgE levels of the dermatophytosis group was higher than the control group (553.7+440.1 vs. 298.2+266.6, P< 0.05). In combination with history, atopy was found in 13 subjects in dermatophytosis group (21.3%) and only 6 subjects in control group (9,8%). While non-atopy was found in 48 dermatophytosis subject (78.7%) and 55 subjects in the control group (90,2%). However, this difference was not statistically significant (OR = 2.4, 95% CI = 0.787-8.695, P> 0.05). Conclusion. Although IgE level was found higher in dermatophytosis compared to healthy control group, atopy in general is not as a risk factor for dermatophytosis.
