Application of Nonspecific Esterase Enzyme Microassay to Detect Resistance Status of Aedes aegypti Larvae in Kampung Inggris, Pare, Kediri, East Java

Latar Belakang: Pada Januari 2015, Kota Kediri dijuluki Kejadian Luar Biasa (KLB) oleh pemerintah daerah karena terdapat wabah demam berdarah dengue (DBD). Berdasarkan rekaman Dinas Kesehatan Kediri, jumlah kasus di tahun 2015 meningkat 9 kali dari tahun sebelumnya. Kecamatan Pare memiliki angka kasus tertinggi diantara 11 Kecamatan di Kediri pada awal 2015. DBD disebabkan oleh parasit yang ditransmisikan vektor nyamuk Aedes aegypti. Strategi primer untuk mengontrol DBD adalah dengan mengurangi kepadatan populasi vektor menggunakan insektisida sdari kelompok organofosfat dan pyrethroid, sesuai anjuran WHO. Malathion dan temephos dari kelompok organofosfat sudah lama digunakan di Indonesia sejak 1974 lewat fogging. Kedua insektisida diketahui memiliki sifat larvasidal. Penggunaan insektisida secara sering, saat wabah maupun tidak, telah menjadi faktor yang berkontribusi dalam berkembangnya resistensi. Paparan terus menerus terhadap bahan aktif insektisida akan menimbulkan modifikasi genetik pada Ae. aegypti. Meningkatnya aktivitas enzim esterase nonspesifik menandakan telah adanya resistensi pada Ae. aegypti terhadap insektisida organofosfat. Tujuan: Untuk mendeteksi potensi resistensi terhadap insektisida organofosfat pada larva Ae. aegypti di Kampung Inggris, Pare, Kediri, Jawa Timur berkaitan dengan aktivitas enzim eseterase nonspesifik. Metode: Penelitian ini adalah studi observational. Subyek penelitian adalah larva Ae aegypti yang diberi pakan glukosa pada generasi 3 atau 4. Generasi pertama ditangkap oleh Departemen Parasitologi Universitas Gadjah Mada berupa larva dari kontainer penduduk Kampung Inggris, yang berada di dalam maupun luar rumah pada 26 Januari 2017 yang kemudian di pelihara di Laboratorium Parasitologi Universitas Gadjah Mada. Bahan-bahan yang digunakan untuk enzymatic assay adalah alpha-naphthyl asetat dan reagen coupling. Resistansi ditentukan secara biokimia menggunakan uji microplate yang nantinya akan dibaca secara quantitatif dengan Microplate Reader. Hasil: Tiga puluh dua persen sampel larva Ae. aegypti dianggap sangat rentan, 52% merupakan resisten sedang, dan 16% merupakan resistent tinggi. Kesimpulan: Larva Ae. aegypti di Kampung Inggris, Pare, Kediri, Jawa Tmur telah resisten terhadap insektisida organofosfat.

Background: In January 2015, Kediri city was labeled Kejadian Luar Biasa (KLB) by local government due to an outbreak of dengue hemorrhagic fever (DHF). Based on record of Dinas Kesehatan Kediri, the number of case in 2015 increased 9 times compared to previous year. Kecamatan Pare had the highest number of case among 11 Kecamatan in Kediri in the beginning of 2015. Dengue hemorrhagic fever is caused by parasite transmitted by mosquito vector Aedes aegypti. Primary strategy to control DHF is by reducing population density of the vector using insecticide from organophosphate and pyrethroid groups, as suggested by WHO. Malathion and temephos from organophosphate group have long been used in Indonesia since 1974 for fogging. Both insecticides are known to have larvacidal property. The common use of the insecticides during outbreak and even non outbreak however, becomes contributing factor to development of resistance. Exposure to the insecticides active ingredient will induce genetic modification in Ae. aegypti. Increase of nonspecific esterase enzyme activity indicates that Ae. aegypti has developed resistance towards organophosphate insecticide. Objective: To detect potential organophosphate insecticide resistance of Ae. aegypti larvae in Kampung Inggris Pare, Kediri, East Java associated with nonspecific eseterase enzyme activity. Method: This research is observational study. Research subject is Ae. aegypti larvae that were fed with glucose at generation 3 or 4. The first generation is taken by Universitas Gadjah Mada Parasitology Departement as larvae from Kampung Inggris residents containers, inside and outside of their house on January 26, 2017 which then reared in Parasitology Laboratory of Universitas Gadjah Mada. The chemicals that were used for the enzymatic assay were alpha-naphthyl acetate and coupling reagent. Resistance was determined biochemistrically using microplate assay which later will be quantitatively scanned with Microplate Reader. Result: Thirty two percent of Ae. aegypti larvae sample are considered highly susceptible, 52% are moderately resistant, and 16% are highly resistant. Conclusion: Aedes aegypti larvae in Kampung Inggris, Pare, Kediri, East Java have developed resistancy towards organophosphate insecticide.
