HUBUNGAN JUMLAH LEKOSIT RESIDU PADA KONSENTRAT TROMBOSIT DENGAN CORRECTED COUNT INCREMENT (CCI) PADA PASIEN ANAK TROMBOSITOPENIA

Latar Belakang: Kebutuhan transfusi konsentrat trombosit semakin meningkat terutama pada pasien anak. Kemanfaatan transfusi trombosit dapat dinilai dengan corrected count increment (CCI) yang dapat dipengaruhi oleh kondisi klinis pasien dan kualitas konsentrat trombosit yang ditransfusikan. Lekosit residu diduga dapat mempengaruhi kualitas konsentrat trombosit selama penyimpanan komponen. Hubungan jumlah lekosit residu pada konsentrat trombosit dengan CCI sebagai indikator kemanfaatan transfusi konsentrat trombosit masih perlu dilakukan penelitian. Tujuan: Mengetahui hubungan jumlah lekosit residu pada konsentrat trombosit dengan CCI pada pasien anak dengan trombositopenia. Metode: Penelitian ini merupakan penelitian observasional analitik prospektif yang melibatkan 51 subjek anak trombositopenia yang dipilih secara konsekutif. Penelitian dilakukan di Unit Pelayanan Transfusi Darah, Instalasi Laboratorium Klinik dan Bangsal Perawatan Anak RSUP Dr. Sardjito. Jumlah lekosit residu yang terkandung dalam konsentrat trombosit dihitung dan nilai CCI ditetapkan 1 jam pasca transfusi konsentrat trombosit. Analisis korelasi antara jumlah lekosit residu dan nilai CCI dilakukan menggunakan uji Spearman. Hasil: Median CCI adalah 14.568/µL (0-63.875). Sebelas subjek (21,6%) menunjukkan CCI 7500/mcL. Pada subjek yang menunjukkan CCI 7500/mcgL menerima transfusi konsentrat trombosit dengan jumlah lekosit residu lebih tinggi (5,6+-3,3x10 6/unit) dan berbeda bermakna secara statistik dibandingkan pada subjek penelitian dengan CCI pasca transfusi >7500/mcgL (3,3+-2,1 x106/unit)(p<0,01). Jumlah lekosit residu memiliki korelasi negatif dengan nilai CCI pasca transfusi(r=-0,34;p=0,01). Pada strata subjek penelitian dengan kondisi tanpa splenomegali, tanpa demam, perdarahan sedang hingga berat atau pada subjek yang menerima transfusi konsentrat trombosit dengan lama simpan >2 hari, jumlah lekosit residu pada konsentrat trombosit mempengaruhi respon transfusi berupa CCI [RR4,88(IK95%1,08-22,12)],[RR2,9(IK95%1,03-8,17)],[RR 10(IK95%1,55-64,19)],[RR7(IK95%1,9-25,2)].Pada subjek penelitian tanpa kondisi splenomegali, perdarahan sedang hingga berat atau demam, jumlah lekosit residu memiliki korelasi negatif yang lebih baik dengan nilai CCI pasca transfusi(r=-0,5;p=0,02). Simpulan: Jumlah lekosit residu pada konsentrat trombosit berbanding terbalik dengan nilai CCI pada pasien anak trombositopenia pada strata subjek dengan lien tidak teraba, tidak demam, mengalami perdarahan sedang hingga berat atau pada subjek yang menerima transfusi konsentrat trombosit dengan lama simpan lebih dari 2 hari.

Background: The need of platelet concentrate transfusion is increased significantly especially in pediatric patients. Effectiveness of platelet transfusion can be monitored by corrected count increment (CCI). It is affected by clinical condition of recipient and quality of platelet concentrates that have been transfused. Residual leukocyte count/unit influences quality of platelets concentrates duri ng storage. Correlation between residual leukocyte count in platelet concentrate with CCI as an indicator of platelet concentrates efficacy is still unclear. Aim: To determine whether residual leukocyte count in platelet concentrates correlates with CCI level in pediatric patients with thrombocytopenia. Methods: This was an observational prospective study, 51 children with thrombocytopenia were recruited in this study and has been choosen consecutively. Research was conducted at the Blood Transfusion Service Unit (BTSU), Clinical Laboratory Unit and Pediatric Ward of Dr. Sardjito Hospital. The residual leukocyte counts that contained was measured and CCI value was calculated at 1 hour after transfussion. Correlation analysis between residual leukocyte count and CCI value was performed by Spearman test. Results: Median CCI in this study was 14,568/µL (0-63,875). Eleven (21.6%) of 51 subjects showed CCI 7500/mcgL. Subjects who showed CCI 7500/mcgL turned out got platelet concentrate with higher recidual leukocyte count (5.6+-3.3 x106 /unit)+-2.1 x10 /unit). This difference was statistically significant (p<0.01). Residual leukocyte count had negative correlation with CCI (r =-0.34;p=0.01). Among subject in conditions without splenomegaly, without fever, with moderate-severe bleeding or among subject that received platelet transfusions with storage time > 2 days, residual lekocyte in platelet concentrate affected transfusions response showed with CCI [RR 4.88(IK95%1.08-22.12)], [RR2.9(IK95%1.03-8.17)],[RR10(IK95%1.5564.19)],[RR 7(IK95%1.9-25.2)]. On subject without splenomegaly, moderatesevere bleeding or fever conditions, residual leukocyte count had better negative correlation with CCI (r=-0.5; p=0.02). Conclusion: Residual leukocyte count was inversely proportional with CCI level in pediatric patients with thrombocytopenia among subject without splenomegaly, without fever, with moderate-severe bleeding or among subject that received
