Evaluasi Kesesuaian Penggunaan Antibiotik Empiris Dan Definitif Pada Pasien Health-Care Associated Pneumonia (HCAP) Rawat Inap di RSUP Dr. Sardjito Yogyakarta

Health-care-Associated Pneumonia (HCAP) adalah pneumonia yang berkembang pada pasien yang dirawat di rumah sakit selama dua hari atau lebih, dalam 90 hari terakhir, menjalani hemodialisis jangka panjang, atau perawatan di rumah sakit 30 hari sebelumnya. Penelitian ini bertujuan untuk mengetahui kesesuaian penggunaan antibiotik empiris dan definitif serta hubungannya terhadap clinical outcome berdasarkan profil farmakokinetik dan faktor-faktor yang berpengaruh terhadap clinical outcome pada pasien rawat inap dengan HCAP di RSUP Dr. Sardjito Yogyakarta. Penelitian ini menggunakan rancangan cohort retrospektif dengan subyek penelitian pasien usia �18 tahun dengan diagnosa HCAP berdasarkan hasil pemeriksaan x-ray thorax dari 1 Januari 2012-Desember 2016. Dilakukan evaluasi terhadap kesesuaian jenis, dosis, frekuensi dan durasi penggunaan antibiotik serta penyusunan antibiogram. Uji Fisher's Exact dan Chi-square digunakan untuk melihat hubungan kesesuaian antibiotik empiris, definitif berdasarkan profil farmakokinetik dan variabel perancu terhadap clinical outcome. Didapatkan 78 pasien dan 47 pasien merupakan pasien definitif. Hasil dari penelitian ini menunjukkan kesesuaian penggunaan antibiotik empiris pada pasien rawat inap dengan HCAP di RSUP Dr. Sardjito Yogyakarta 75,6% tidak sesuai. Untuk kesesuaian penggunaan antibiotik definitif menunjukkan 55,3 % tidak sesuai. Berdasarkan analisis statistik diketahui tidak adanya hubungan antara kesesuaian antibiotik empiris dan definitif berdasarkan profil farmakokinetik terhadap clinical outcome pasien HCAP dengan nilai p= 1,000 dan 0,352 (p>0,05). Faktor yang berpengaruh terhadap clinical outcome pasien HCAP adalah infeksi penyerta dengan nilai p=0,000 (0<0,05). Gambaran antibiogram pada pasien HCAP adalah: pola bakteri yang menginfeksi pasien HCAP yaitu Gram negatif sebesar 55,4% dan bakteri Gram positif 44,6% dimana antibiotik yang mempunyai sensitivitas tertinggi terhadap Gram negatif dan positif adalah imipenem dengan persentase sensitivitas masing-masing 91%, dan 86%.

Health-care-Associated Pneumonia (HCAP) is a developing pneumonia in hospitalized patients for two days or more, within the last 90 days, undergoing long-term hemodialysis, or hospitalization 30 days earlier. This study aims to determine the suitability of the use of empirical and definitive antibiotics and their relationship to clinical outcomes based on pharmacokinetic profiles and factors that influence clinical outcome in inpatients with HCAP in Dr. Sardjito Yogyakarta. This study used a retrospective cohort design with study subjects aged �18 years old with HCAP diagnoses based on x-ray thorax examination results from January 1, 2012-December 2016. An evaluation of the suitability of type, dose, frequency and duration of antibiotic use and antibiogram preparation was performed. Fisher's Exact and Chi-square tests are used to look at the relationship of empirical, definitive adherence based on pharmacokinetic profiles and confounding variables to clinical outcomes. There were 78 patients and 47 patients were definitive patients. The results of this study indicate the suitability of empirical antibiotic use in inpatients with HCAP in Dr. Sardjito Yogyakarta 75.6% is not suitable. For the appropriateness of definitive use of antibiotics showed 55.3% were not appropriate. Based on statistical analysis, there is no correlation between the compatibility of antibiotic and definitive based on pharmacokinetic profile to clinical outcome of HCAP patients with p = 1,000 and 0,352 (p> 0,05). Factors that influence clinical outcome of HCAP patients are comorbid infections with p = 0,000 (0 <0.05). The antibiogram features in HCAP patients are: the pattern of bacteria infecting HCAP patients is Gram negative of 55.4% and Gram positive bacteria 44,6% where antibiotics having highest sensitivity to Gram negative and positive are imipenem with percentage of sensitivity respectively 91% , and 86%.
